**Responsibilities:**
GENERAL PURPOSE
Primarily responsible for providing Targeted Case Management (CM) and Psychiatric Rehabilitation services within Strong Recovery for Safe Court Clients. This hybrid model of care is intended to support the wellness and recovery goals of individuals with complex and/or chronic behavioral health issues and needs by implementing targeted case management services and interventions at Safe Court and Strong Recovery designed to provide timely, high-quality, and efficient care. The CM will provide case management services to support patients around goals aimed at providing self-sufficiency. Additionally, the CM performs psychosocial rehabilitation services designed to assist the individual to reduce trauma, mental health symptoms and substance abuse barriers. The intent of this role is to assist the individual to restore the individual's functional level to the fullest possible. The primary focus of these services with the individuals includes the following:
- Maximize success through developing and maintaining rapport with clients during all phases of Sate Court participation
- Meet personally and make telephone contacts with clients on a regular basis to monitor compliance to court and/or intervention requirements.
- Monitor the client's progress in the course of treatment/Intervention and make appropriate alterations to plans when indicated
- Make telephone and personal contacts with defense counsel, judges, district attorneys, courts and jail personnel when needed
- Make collateral contacts by phone or writing for the purpose of verifying compliance
- Perform urine screening as needed
- Make accurate and timely reports to the Safe Court Coordinator on clients' compliance with conditions, both in writing and, periodically, in person at court appearances
- Maintain accurate and up to date records both manually and on the agency data base regarding client compliance and contacts
- Develop and maintain cooperative working relationships with all components of the criminal justice system, allied systems, and Strong Recovery community-based intervention programs
- Maintain strict confidentiality of protected information
- Make contact with clients in the Monroe County Jail, when indicated
- Perform any other tasks as deemed appropriate by the Safe Court Coordinator at the Court
- Must demonstrate trauma-informed, broad cultural sensitivity and a non-judgmental approach to clients
- Must demonstrate skill in identifying primary treatment/intervention needs, such as mental health problems, drug/alcohol abuse, and other areas of concern
- Must demonstrate ability to work independently in a variety of settings
- Must demonstrate ability to manage multiple tasks competently while meeting deadlines and maintain positive interactions
- Must demonstrate computer literacy
- Must demonstrate ability to work well in a team environment and to work cooperatively to fulfill the goals of the agency and the program
- Must be able to obtain and maintain a jail pass issued through the Monroe County Sheriff's Department
Under general direction and with considerable independence, performs several care management and psychiatric rehabilitation services consistent with CCBHC and the Department of Psychiatry Policy. Establishes and maintains cooperative working relationships with community providers and resources to obtain needed services and support for enrolled patients. Works as part of an interdisciplinary team to support activities to engage patients in care, connection to care management, vocation and education resources and ensures necessities are met.
Utilizes community and family resources to create sustainable support systems for patients. Reviews and discussed plan with patient and the treatment team, focusing on linking individuals to clinical and social services within the system and community providers.
**ESSENTIAL FUNCTIONS**
+ Conducts assessment of identified patients for psychiatric rehabilitation and care management services. Designs appropriate treatment plan goals according to patients' psychiatric rehabilitation needs. Completes and maintains professional documentation
+ Conducts evidence informed treatment with patients. Assesses successful completion of goals and plans termination of psychiatric rehab services and care management services accordingly.
+ Provide case management needs as indicated to ensure access to healthcare and meeting immediate needs or patients.
+ Coordinates outreach and engagement activities focused on finding, connecting and retaining patients in appropriate community services.
+ Completes all Health Home Care management referrals, community agency referrals, and self-referrals for CCBHC patients who are not yet connected to Health Home Care Management services.
+ Maximize community integration and normalization
+ Mobilizes family and natural support networks when necessary.
+ Provides milieu management in reception areas at Strong or the court, supporting patients to engage with substance use treatment and mental health care, connection patients to Addiction and Mental Health services and other support services.
+ Available to assist clinicians as needed, to support case management needs.
+ Liaison responsibilities: collaborates with referral sources and other professionals within Safe Court and the CCBHC throughout patient care activities.
+ Establishes and regularly maintains cooperative working relationships with community and health care agencies and professional and administrative staff of the Medical Center and University.
+ Works as part of the Safe Court Team and Strong Recovery CCBHC team (Peer specialists, Rehabilitation Therapists)
+ Maintains professional standard by reading, attending seminars, workshops and conferences.
+ Attends weekly supervision/clinical meetings
+ Participates in departmental and programmatic diversity presentations
+ Available to triage case management needs for CCBHC program recipients on an urgent basis, including walk-ins
+ Committed to understanding and meeting the unique needs of external and internal customers.
+ Contributes to the vision and mission of the Safe Court and Strong Recovery CCBHC
+ Maintains a thorough knowledge of related policies and procedures.
+ Coordinates with support staff regarding patient scheduling, fees, and insurance issues.
+ Promote hope and recovery by using strengths based, culturally appropriate, and person-centered practices.

**MINIMUM EDUCATION & EXPERIENCE**
+ 2-3 years of relevant experience working with the mentally ill.
+ Or an equivalent combination of education and experience preferred.
+ Experience working as a case manager in a Safe Court or A Master's degree in the mental health or addictions field preferred.
+ Experience working with adults and/or adolescents with severe mental illness with or without co-occurring substance use disorders preferred.
**KNOWLEDGE, SKILLS AND ABILITIES**
+ Experience/Education focused on assisting individuals with vocation/education goals preferred.
**LICENSES AND CERTIFICATIONS**
+ Must possess and maintain a valid New York state License, have a satisfactory driving record required.

6.Submits required documentation in accordance with the policies and procedures of the department.
7.Participates in quality improvement chart reviews and activities.
8.Attends family meetings/conferences and interacts with members of the interdisciplinary staff pertaining to the welfare and management of assigned patients.
9.Makes periodic inspections of equipment, identifies equipment malfunctions and initiates repairs by contacting appropriate service departments.

Job Qualification
1.Bachelor's Degree required, or equivalent combination of education and related experience.
2.Current License to practice as a Physical Therapist, Occupational Therapist, or Speech-Language Pathologist in New York State required, plus specialized certifications as needed.
3.1-3 years of relevant experience and training, required.

The salary range for this position is $85,090-$147,220/year
