- Plan, conduct and participate in In-service Education Programs. Demonstrate (hands on) regarding the resident needs as identified by the IDT, including the use of appropriate materials and equipment.
- Assist in implementing and maintaining an effective orientation program that orients the new employee to the facility, his/her department and its policies and procedures for active treatment.
- Attend and participate in continuing education programs.
- Recommend to the Administrator the equipment and supply needs of the department.

- Ensures that appropriate adaptive equipment is present, in good repair, is appropriately used.
- Provides positive reinforcement to both staff and clients.

- Ensures that each resident has a current IPP and active treatment goals monthly.

- Ensures that all assessments are completed in a timely fashion prior to the IPP annually and as needed.
- Writes monthly notes regarding progress on resident goals as well as family and school contacts and social service needs.
- Update goals based on review of data as appropriate and in-service staff upon revisions.
- Notifies appropriate professional staff when goals are met, regression noted, or no progress noted.
- Ensures that special staffings are held within 3 days following hospital discharges, changes in medical status, or any update occurs which would affect programming.
- Assure that all clients are well groomed, clean, dressed appropriately in their own clothing, nails clipped, shaved appropriately, hair groomed, etc.
- Participates in the development, maintenance, and implementation of the facility's Quality Assurance meetings, etc., as directed.
- Attends community outings as assigned by the Administrator and/or designee.

**Responsibilities**
Assist clients by assessing their social services needs, solving problems, setting goals and obtaining required services.
+ Conduct initial intake procedures.
+ Assess needs and eligibility of potential clients.
+ Participate in the initial assessment process, service plan of care development, and ongoing monitoring of each client's care.

**Requirements**

Position Summary:
Provides social services support to the community while functioning as an advocate for the residents, family members, and continued success of the organization.
Social Services Essential Duties and Responsibilities:
1. Completes documentation in the resident's charts regarding pertinent information to conform to organizational policy, industry standards, along with required state and federal regulations and protocol.
2. Discusses with and informs the appropriate department(s), residents, and resident families of room changes / transfers in a timely manner.
3. Initiates and participates in family meetings to discuss a resident's level of care, family concerns, or new diagnosis.
4. Attends and participates in departmental, organizational, and other requested or required trainings and meetings.
5. Completes MDS requirements in an effective and efficient manner, in accordance with appropriate and standard protocols.
6. Participates in and attends resident care plans to create and ensure a team approach to resident desires, needs, concerns.
7. Communicates effectively with referral sources, such as home health agencies, other nursing homes, hospitals, etc, regarding residents or potential residents on our campus.
8. Assists in supporting other departments as needed and requested to benefit residents and families.
9. Using superior customer service and problem solving skills, addresses and assists in solving resident and family concerns, including unaccounted items, changes in care levels, and facilitating relationships with other staff members.

Discharge Planner Essential Duties and Responsibilities:
1. Complete all required social service documentation in a timely manner.
2. Responsible for timely completion of MDS 3.0.
3. Complete a discharge plan and assessment within a 24 hour guideline.
4. Competent in written clinical documentation.
5. Able to care plan/ service plan behavior/mood or other issues with admissions.
6. Meet Medicare/Medicaid standards of Care Plans meetings and Notice of Medicare provider non-coverage.
7. Able to work well with families and resident during crisis and LTC adjustments.
8. Address and explain resident rights.
9. Able to assist and facilitate advance directives
10. Ability to make referrals and communicate with physicians and hospitals.
11. Able to complete paperwork as needed for completion of discharge.
12. Facilitate the facility Bed hold policy and Care Plan letters.
13. Able to intervene and liaison between staff, family, and resident.
14. Able to problem solve and defuse issues with residents.
15. A willingness to support and assist Social Services, Admissions, and other related disciplines.

Qualifications:
