Medical Director Of Surgery

The Medical Director of Surgery at Intermountain Health St Vincent Regional Hospital will work collaboratively in a (dyad/triad-Surgery, Anesthesia, OR Manager) partnership to define, establish, and oversee the work of Surgical Services at the facility. The Medical Director provides the respective facility with leadership, oversight, and support for the development of a culture of high reliability and the measurement of care, identifying opportunities, building relationships, executing on strategies to drive the performance and measured improvement in surgical care delivery. They will oversee performance all tasks necessary to provide safe, efficient, high-quality patient care. Responsible in tandem with operations and nursing officers for the administrative, financial, and clinical performance of the operating rooms of the facility. The Medical Director will assist in maintaining an appropriate operating room environment. Where possible, the Department Chair will fill the role of Medical Director of Surgery at the facility to help better align facility surgical administration with surgical operations.

Scope: This is a facility-level leadership role that requires involvement at a system level within the One Intermountain structure. Success in the role requires key organizational relationships across Intermountain Health with a specific focus on leading the performance and continuous improvement of surgery at the respective facility. The Medical Director of Surgery will report to the hospital Chief Medical Officer and strongly partner with the Senior Medical Director of Intermountain Surgical Services. Where applicable, the Department Chair role will report to the Hospital Medical Director and Medical Executive Committee of the chair's respective facility.

Leadership

  • Leads Physician/Advanced Practice Provider collaborations and defines and establishes care models that best meet our patient's needs and our system fundamentals
  • Where applicable, fulfill Department Chair duties and responsibilities as outlined in the Medical Staff Bylaws and in the Department/Division Rules and Regulations
  • Ensures that evidence-based clinical care standards for Surgical Services are established and effectively implemented in the respective Intermountain facility
  • Responsible for execution of policies and procedures related to operating room environments
  • Assists in educating compliance with appropriate standards required by all governmental and regulatory agencies
  • Champions quality initiatives and policies and reports infractions, collaborating to eliminate future infractions
  • Address opportunities and maintain successes in patient and provider experience
  • Conduct regular Operating Room Council meetings, in conjunction with dyad/triad partners, to address safety, efficiencies, outcomes, policies, scheduling, utilization, system initiatives, and facility-specific needs
  • Regular attendance of meetings, which include but are not limited to: Facility Operating Room Council, Surgical Services Council, facility-specific councils/committees

GENERAL OVERVIEW:
The hospital-based Wound Ostomy Nurse serves as a clinical resource and consultant for the acute and rehabilitative care of patients with selected disorders of the gastrointestinal, genitourinary, and integumentary systems such as acute and chronic wounds (e.g., complex draining wounds, fistulae, pressure ulcers, vascular ulcers), ostomies, percutaneous tubes, and continence disorders. Professional responsibilities are carried out through direct patient care, clinical consultation, education, research, and administration.
ESSENTIAL RESPONSIBILITIES:
+ Utilizes the nursing process when directing or delivering care to patients with WOC needs, including assessment, diagnosis, outcomes identification, planning, and implementation (e.g., coordination of care delivery, health teaching and promotion, consultation), and evaluation
+ Utilizes and maintains current physical, psychosocial, clinical assessment, and therapeutic communication skills to determine individual patient needs and expected outcomes
+ Participates in committees or task forces related to wound, ostomy, and continence issues
+ Provides consultation for individuals with altered skin integrity (e.g., pressure ulcers, draining wounds, fistulae) to develop individualized plans of care and attain expected outcomes
+ Provides guidance to staff in implementing protocols to identify, control, or eliminate etiologic factors for skin breakdown, including selection of appropriate support surfaces
+ As applicable, with a physician's order, provides appropriate debridement of devitalized tissue (e.g., conservative sharp debridement, silver nitrate (AgNO3) cauterization of nonproliferative wound edges, hypergranulation tissue, or to control minor bleeding)
+ Provides appropriate education to patients, caregivers, and staff regarding skin care, wound management, care of percutaneous tubes, and draining wound/fistulae management
+ Validates pressure ulcer data collection for nursing quality indicators (e.g., pressure ulcers and restraint use)
+ Provides pre- and postoperative education to patients who are undergoing ostomy surgery (and their families), with consideration of the need for physical and psychological adaptation
+ As applicable, marks the stoma site preoperatively by determining the appropriate site for stoma placement with consideration of the patient's anatomical markings, physical capabilities, and lifestyle
+ Assesses the stoma and stoma functioning and initiates appropriate procedures for stoma care (e.g., removes rod/sutures, measures/fits a pouching system, teaches care of stoma/peristomal skin and pouch)
+ Provides appropriate educational information to patients, including postoperative and discharge instructions about care, referrals, and supply needs.
+ Serves as a resource for patients after discharge through telephone consultation or outpatient services
+ Identifies risk factors for urinary or fecal continence disorders
+ Assesses patients with urinary and/or fecal continence disorders
+ Establishes an appropriate management program to include dietary and fluid management; bowel training or stimulated defecation program; bladder retraining, prompted voiding, or a scheduled voiding program; pelvic muscle reeducation without biofeedback; indwelling catheter management; recommendations regarding containment/absorptive products and devices and skin care; and education and counseling for patients/caregivers.
+ Identifies patients requiring referral for assessment/management of complex urinary or fecal continence disorders.
+ Participates in developing and implementing procedures and protocols, based on current national guidelines, to deliver care to patients with the following: Urinary or intestinal disorders resulting in the need for an ostomy; Conditions requiring wound management; Impaired or altered skin integrity; Urinary and/or fecal continence issues
+ Assists the nursing and medical staff in maintaining current knowledge and competence in WOC care by providing the following: Formal or informal continuing education programs; Clinical rounds; Lunch and learn sessions; Orientation sessions; Collaborative practice committees; Nursing councils

**GENERAL OVERVIEW:**
This job performs nursing care during cardiac catheterization procedures, including precise instrument handling, accurate medication administration, and proficient monitoring of hemodynamic parameters. + Participates in the care of patients undergoing Cardiac Cath Lab, Interventional Radiology, Electrophysiology, Hybrid Operating Room procedures and surgeries. Prepares patients for cardiac catheterization procedures, including explaining the procedure, obtaining informed consent, and verifying allergies and medical history.
+ Integrates knowledge of cardiovascular and peripheral vascular physiology, pathophysiology, and procedural techniques being performed, and assists accordingly.
+ Positions patients, selects anatomical and technical parameters accurately for independent imaging of special procedures per established protocol. Prepares for and assists the physician and/or physician assistant while performing procedures.
+ Maintains the competency to administer and monitor Moderate Sedation, vasoactive medications, and anticoagulants as directed by a physician.
+ Anticipates adverse reactions / conditions when monitoring the effects of all procedural activities and responds appropriately.
+ Demonstrates competent technical skills and may perform scrub functions related to the intra-operative phase. Assists cardiologist during cardiac catheterization procedures by preparing equipment, managing instrumentation, and maintaining a sterile field.
+ Demonstrates knowledge of accepted procedures performed during the intra-operative phase utilizing proper aseptic techniques and practices standard precautions. Follows policy for medication and specimen handling.
+ Prepares contrast media, practices aseptic technique pre-, during, and post- special procedures.
+ Completes required documentation for procedures e.g. regulatory, billing protocols and image storage and archival.
+ Maintains adequate supplies for efficient section/department operations. Participates in inventory control and monitoring.
+ Orients inexperienced staff on the operation of laboratory equipment; troubleshoots malfunctions and is knowledgeable of proper support personnel and instances when they need to be contacted.
+ Provides accurate and through nurse-to-nurse handoff to receiving unit.
+ Available to cross-train as appropriate in various areas.

**QUALIFICATIONS:**
Minimum
+ Act 34 Criminal Background Clearance Certificate
+ Act 33 Child Abuse Clearance Certificate
+ Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
+ Current State of PA RN licensure OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).
+ BLS and ACLS Certification
Preferred
+ Certification in area of specialty
+ RCIS Certification encouraged within five years
