Job Summary:

Manages the daily operations for the Care Coordination department, which includes RN Case Managers, Social Workers, and administrative team members.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Develops and implements specific departmental goals, standards, and objectives which support the strategic plan and vision.
  • Assesses quality of patient care and coordinates patient care services with patients, staff, physicians, and other departments. Provides bidirectional communication between leadership and frontline staff,
  • Collaborates with others regarding orientation and ongoing education. Provides coaching and mentoring of staff . Ensures staff compliance with departmental and organizational policies, procedures, and standard work.
  • Interacts with staff daily, identify gaps/barriers to process, escalates and brings gaps/barriers to resolution .
  • Participates with other members of team regarding opportunities for improvement in standard work .
  • Monitors departmental budgets, regulatory compliance, departmental contracts and vendor relations.
  • Manages staff relations including performance management, staff satisfaction and conflict management. Performs and oversees scheduling, recruitment, and payroll .
  • Applies the existing body of evidence-based practice and scientific knowledge in health care to nursing practice, ensuring that nursing care is delivered based on patient’s age-specific needs and clinical needs as described in the department's Scope of Service
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.

EDUCATION

  • Bachelor’s degree in nursing or Masters in social work

EXPERIENCE

  • Five years’ experience, with two years’ progressive leadership  

PHYSICAL REQUIREMENTS

  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.

**Here's what you can expect working in Patient Registration (Customer Service):**
+ Working on computers and answering phones to intake information in a busy environment. You'll work across multiple screens collecting details that must be input correctly between physician and nurse check-ins. You should feel comfortable working across multiple programs and typing as quickly as patients can talk. You will work around clinicians interrupting the registration process for emergent clinical care.
+ Helping people who may be very ill or worried. We're not human computers intaking information. Patients need you to be there for them and listen carefully to ease their anxiety. You'll ask follow-up questions and build rapport in real time.
+ Fast-paced work environment, often on your feet assisting patients complete their registration.
