Provide a short description of the Position: OPWDD Modernization and Remediation Program (HHS_OPW_PRJ0251326) within the OPWDD TABS (Tracking and Billing System) Application. Modify TABS to use a web-based GUI and consolidate the TABS databases.

Provide a list of the day-to-day tasks to be performed by the Selected Candidate: 1. MUMPS/Cache' programming; 2. Responsible for attending and participating in meetings related to projects and tasks impacting the Tracking and Billing System (TABS) 3. Gathering, documenting, and implementing technical requirements surrounding the analysis and design of new MUMPS/Cache'solutions to support the strategic vision of OPWDD 4. **Narrative:**
Grant funded position.
A Community Health Worker (CHW) is a frontline public health worker, who is a trusted member of and/or has a remarkable understanding of the community served. This trusting relationship with the community enables the CHW to serve as a liaison/link/intermediary between the SPHP and the community to facilitate access to services and improve the quality and cultural competence of service delivery. In this position, the CHW will serve as a "success coach" for newly hired front line colleagues at St. Peter's Health Partners. The CHW/Success Coach will provide support to ensure colleagues feel supported in their new role. A CHW builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as mentoring, providing community resources and advocacy for colleagues.
Acts as a mentor and resource to newly hired colleagues, in frontline positions, in order to navigate the onboarding process. Builds and maintains strong relationships with newly hired colleagues to facilitate successful onboarding and job retention. Facilitates or conducts enrollment in available internal and community services, unmet social needs, such as food assistance, transportation and access to health care, educational opportunities.
Serves as a liaison between human resources and colleagues to facilitate open lines of communication.
Provides in-service education to service community-based organizations on the role of a CHW/Success Coach.
Continuously expands knowledge and understanding of community services and resources; maintains frequent contact with community agencies serving vulnerable populations and participates in community meetings.
Uses Social Care tools via The Community Resource and Unite Us platforms for coordination of services.
Completes timely and accurate documentation of services provided to colleagues via electronic platform. Documents notes, activities, service plans, and outcomes achieved by individuals in an effective manner.
Assists with data collection and interpretation relevant to understanding outcomes of services provided.
**MINIMUM QUALIFICATIONS:**
1. High School Diploma / associate's degree preferred. One to three (1-3 years) of community involvement experience. Ability to earn a CHW certificate is preferred. Must be able to operate effectively in a collaborative, shared leadership environment.
2. Must possess a personal presence that is characterized by a sense of honesty, integrity and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals and values of Trinity Health.
3. Must be able to operate effectively in a collaborative, shared leadership environment.
4. Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively engage with colleagues. Ability to effectively present information in one-on-one and small group situations to colleagues.
Work requires ability to communicate orally and to hear or communicate with target population. Bi-lingual skills to work with patients in their native language are preferred; certification as a medical interpreter/translator is a plus
Work requires proofreading and checking documents for accuracy.
Basic Life Support (BLS) certification is preferred.
Must be comfortable working and serving in a diverse and inclusive environment, and operating in a collaborative, shared leadership environment.
PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS
1. Must be able to adapt to frequently changing work priorities and be able to prioritize and balance the requirements of working with the System Office and Regional Health Ministries (RHMs).
2. Must be able to travel to various SPHP sites and other locations as needed.
3. Requires working in a normal office environment where there are relatively few physical discomforts due to dust, dirt, noise, and the like.
4. Requires spending time in other agencies and environments within the community where there may be some disruptions due to noise, interruptions and the like.
5. Must possess good manual dexterity in order to operate a computer keyboard.
6. Must possess good hearing in order to be effective on extensive telephone conference calls and for in person communication.

This grant-funded position plays a central role in coordinating and advancing a regional initiative focused on physical activity and nutrition. As the project lead, you will serve as the primary point of contact for school districts and ECE providers, fostering partnerships that promote wellness at a systems level.
**Key Responsibilities:**
+ Provide tailored education and technical assistance to strengthen wellness policies in schools and ECE programs
+ Craft and deliver impactful training for school and ECE staff

+ If no Master's degree: Minimum of 4 years of relevant experience in a public health or community engagement role

+ Valid driver's license and reliable transportation for regional travel (mileage reimbursed at GSA rate)
