Thriveworks is currently seeking Licensed Clinicians to provide a mix of telehealth and face-to-face sessions in Framingham, MA.

Qualifications:

All eligible candidates must have one of the following independent licenses in Massachusetts:

  • LICSW or LMFT or LMHC

  • Licensed Clinical Psychologists (PhD or PsyD)

Clinicians need to be licensed and living in the state in which they will be practicing.

Qualified candidates will have experience primarily working within a specialty (e.g., couples therapy, children, trauma) and must also be comfortable working with individuals of all ages and populations (Behavioral Health Generalists)

Compensation: Up to $ 137,200 depending on licensure, sessions, and bonus opportunities

Position Summary:

Join PACE! As part of a dynamic Interdisciplinary Team (IDT) in an innovative model of care, you will work closely with participants and their caregivers to help them continue to live safely in their homes and communities as independently as possible. You will work in an integrated primary care and behavioral health setting to support participants in their homes and at one of our PACE centers.

Social services case management position in the PACE Social Work Department assisting participants with various "social determinants of health" (SDOH) needs including housing, food insecurity, benefits and more. Work closely with a supportive interdisciplinary medical team supporting the PACE participants and their families.

Full-time position to act as case manager and support for the Neighborhood PACE Social Work department. Work with older adult participants and families to assist with vital "SDOH" (Social Determinants of Health) issues including housing, food insecurity, benefits access, and more. An ideal candidate will be knowledgeable about community resources, be detail oriented, able to multitask, and possess excellent communication and computer skills. Must hold a Bachelor’s degree in social work or related field OR have two years of relevant work experience in a community setting. Bilingual (English/Spanish) required. Automobile required.

Participant Navigator Responsibilities

  • Builds trusting relationships with participants

  • Provides assistance with housing applications; maintains records of pending applications; provides follow-up on applications

  • Acts as liaison and provides housing advocacy with various housing organizations on behalf of participants

  • Assists with case management tasks including, but not limited to: Food Stamps/DTA applications and re-certifications, Safelink/Assurance phone assistance, scheduling RIDE interviews, Social Security income verifications, calling Social Security to troubleshoot issues

  • Researches and provides participants with resource information, such as food pantries, support groups, immigration assistance

  • Provides money management assistance and act as liaison to representative-payees

  • Implements supportive interventions such as check –in calls under the direction of the social worker

  • Performs community outreach and accompanies participants with exceptional hardship and need to appointments in special circumstances.

  • Consults appropriately with clinical staff

  • Thoroughly documents all communication with participants in the medical record in a timely manner

  • Demonstrates excellent customer services skills and cultural competency with participants and caregivers of diverse backgrounds

  • Demonstrates strong commitment to participants, resolves case management issues as expeditiously as possible to participant satisfaction. As representative of PACE and EBNHC, maintains a highly professional, courteous and respectful manner with participants, caregivers and colleagues

EDUCATION:

BU School Of Social Work (BUSSW) is looking for a full-time Digital Communications Manager to join its Marketing & Communications team. This highly collaborative role supports the school's priorities in reputation building, community engagement, and enrollment marketing through strategic digital storytelling, ensuring alignment with both the school's brand and Boston University's overall brand.

Inpatient Case Management provides psychosocial assessments, evaluations, and referrals for adults, and/or families with psychiatric illness, substance abuse, and medical illness. Casework or therapy takes place in the hospital setting.
Job Details
The Clinical Social Worker provides psychosocial assessments, evaluations, and referrals for adults, and/or families with psychiatric illness, substance abuse, and medical illness. Casework or therapy may take place in the hospital, offices, or community setting, as appropriate. Clinical Social Workers collaborate with multi-disciplinary teams at service sites, and specifically with the team leader to define and provide the services needed by clients. They demonstrate a capacity to work collaboratively in team setting, with an ability to listen to others, participate actively, and take appropriate responsibilities. This position will also collaborate with other caregivers and community agencies as necessary to provide systemically informed, community based treatment. Such collaboration may require participation in caregiver conferences, Patient at Risk meetings, and other meetings as needed. You will complete billings forms, managed care documentation, statistics, and other required paperwork thoroughly and on time.
+ Education/Training: Master's degree, Social Work
+ Licensure: Current MA licensure as LICSW (Licensed Independent Clinical Social Worker)
+ Certifications: Same as above. When required, must be credentialed by the CHA Medical Staff for practice in relevant clinical practice areas.
Previous Experience:
+ Incumbents must have completed at least one successful internship in a medical setting required.
+ Prior experience in a medical setting required.
+ Two years post Masters degree, in medical, clinical or human service setting with relevant populations preferred.
+ All newly hired candidates must successfully complete a job specific physical agility exam
