*Position: Travel Nurse Practitioner*

Monogram Health is looking for a skilled Nurse Practitioner eager for the opportunity to make a difference in patients’ lives. The Monogram Travel Nurse Practitioner plays a critical role in providing access to comprehensive care for underserved patients our Monogram Health network reach. Also, helping patients improve their quality of life in the home and slow the progression of kidney disease and multiple other chronic illnesses, enabling positive health outcomes.

*Your Impact:*

As a Travel Nurse Practitioner, you will be responsible for the delivery of personalized and compassionate care to patients primarily with CKD through ESRD. Monogram’s Travel NPs will become nationwide float providers to the members in areas where our local clinicians are unable to reach and in areas where we are rapidly growing. Primary duties include patient health assessment, creating strategies to improve or manage a patient's health, and introducing habits for health promotion. In part of completing this care for patients, your responsibilities will include maintaining accurate patient records, scheduling, and administering follow-up appointments to patients as required across multiple markets.

*Roles and Responsibilities*

* Travels nationwide to conduct in-home assessments, which include comprehensive annual wellness exams, focused screenings, or other visits both in the patients' home and in the virtual environment
* Ability to travel out of town for up to 75% of the month during the work week
* Counsels and educates patients and families about benefits and programs available to help them live healthier lives
* Documents items such as: appropriate chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan
* Responsible for the coordination of care with primary care providers, specialists, and appropriate ancillary services
* Completes all documentation and paperwork in a timely manner
* Maintains quality of care standards as defined by the practice
* This position will not be office-based but will be based in state in which employed and will need to attend periodic training/meetings outside of that state
* Deliver evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction first
* Prescribe medications, order tests, and collaborate with patient’s Monogram physician
* Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily
* Facilitates closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic testing
* Assists patients with enrolling to access educational videos
* Participates in the integrated care team meetings
* Knowledge of disease diagnosis and prevention
* Make assessment of patient's health status
* Implement a plan consistent with appropriate plan of care
* Follow-up and evaluate patient's status

*Position Requirements*

* Active and unrestricted Registered Nurse and Nurse Practitioner
* Board certified for appropriate licensure (NP: ANCC/AANP)
* Current and unrestricted DEA certificate, or eligibility
* Ability to travel extensively by air and utilize rental vehicles
* Ability to work without direct supervision and practice autonomously
* Access to transportation, a valid driver's license, and car insurance
* Must be proficient with medical instruments and equipment required by the work
* Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology
* Ability to communicate effectively in verbal and written form with retail and medical partners at various levels, patients, family members, physicians and representatives of the community
* Sound understanding of all federal and state regulations including HIPAA and OSHA

Qualifications EDUCATION Certified Registered Nurse Anesthetist Completion of Certified Nurse Anesthetist Training Program Graduate of a Nurse Anesthesia Educational Program accredited by the American Association of Nurse Anesthetist Council on accreditation of Nurse Anesthesia Programs. Current State of Connecticut Nursing License, Advanced Practice Registered Nurse (APRN) licensure in the State of Connecticut. EXPERIENCE Prefer clinical working experience as a CRNA within the given specialty/service line recommended, but none required. APRNs, PAs, CRNAs and CNMs are required to be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process. Evidence of current competence to practice as {an APRN, PA, CRNA, CNM} via appropriate reference letters from physicians and other practitioners must be able to be obtained during the Medical Staff appointment process in order to be eligible. LICENSURE Certified Registered Nurse Anesthetist AANA Certification, State of CT RN and APRN License, ACLS DEA Registration and State of CT Controlled Substance Registration SPECIAL SKILLS CRNA's must have sharp concentration and focus, along with the ability to stay calm under stressful conditions. CRNAs must be able to communicate clearly and effectively with surgeons and the surgical support team, as well as with patients and their families. A calm and reassuring manner is one way to inspire trust in patients, who may be apprehensive about surgery, as well as the surgical team. Anesthetic patient management must be immediate and responsive. Flexibility and adaptability are required. PHYSICAL DEMAND A CRNA must be able to demonstrate intellectual-conceptual, integrative and quantitative abilities; skills in observation, communication and motor functions; and mature behavioral and social attributes. Sensory- CRNA must be able to detect and interpret changes in monitoring alarms and equipment. Functional vision, hearing. no impairment of sense of smell and tactile sensation must be adequate. CRNA must be able to observe a patient accurately at a distance and close.Communication-CRNA should be able to speak, hear and observe. CRNA must be able to communicate effectively- includes speech, reading and writing.Motor -CRNA should have sufficient motor function to elicit pertinent information. CRNAs should be able to stand for long periods of time and wear lead as needed for fluoroscopy, X-Ray cases. CRNAs must have the ability to lift 25 pounds and reach above and below shoulder height. CRNA must be able to negotiate patient care environments and be able to move self/patients between anesthesia locations. Coordination of both gross and fine muscular movements, equilibrium, and functional use of the senses of touch and vision is required. Intellectual-Conceptual, Integrative and Quantitative Abilities-Abilities include measurement, calculation, reasoning, analysis and synthesis. Problem-solving, the critical skill demanded requires all of these intellectual abilities. CRNA should be able to comprehend 3-dimensional relationships and understand the spatial relationships. Behavioral and Social Attributes -CRNA must possess the emotional health required for full utilization of intellectual abilities, the exercise of good judgment, the prompt completion of responsibilities. CRNA must be able to tolerate physically taxing workloads and to function effectively under stress and able to adapt to changing environments, display flexibility and learn to function in the face of uncertainties inherent in the clinical problems of many patients. Position Summary:
We are seeking a compassionate and experienced Women's Health Nurse Practitioner (WHNP) to join our OB/GYN Department. The WHNP will provide comprehensive, patient-centered reproductive and gynecological healthcare services across the lifespan, including preventive care, family planning, prenatal and postpartum care, and management of common gynecologic conditions. This position supports the goals of CAMcare's HRSA grant by addressing women's health disparities and increasing access to integrated care.

Key Responsibilities:
  • Provide comprehensive women's health services, including well-woman exams, contraception counseling, STI screenings, prenatal care, and menopause management.
  • Collaborate with physicians, nurses, case managers, and other members of the care team.
  • Ensure culturally competent, trauma-informed, and evidence-based care is provided.
  • Participate in care coordination and health promotion activities in alignment with HRSA and UDS standards.
  • Document all care in the electronic health record (EHR) in a timely and accurate manner.
  • Participate in quality improvement initiatives, including HEDIS measures and clinical benchmarks.
  • Serve as a preceptor or mentor for students as needed in partnership with academic institutions.
Minimum Qualifications:
  • Master's or Doctoral degree from an accredited Nurse Practitioner program

**Duties and Responsibilities**
**Essential Functions:**
+ Obtains/records initial patient histories, assesses current health status, preform physical exams, identifies problems/needs and designs comprehensive management plans for patients.
+ Assesses current health status, real and/or potential health problems and identifies expected outcomes in response to management plans, reviewing data with the physician as necessary.
+ Requests appropriate diagnostic and screening procedures, interpreting the results.

The Advanced Practice Provider (APP) Manager will provide daily clinical and operational management for the advanced practice providers on their service/in their service line in support of institutional and departmental missions. The APP Manager for Cardiovascular services at HUP provides leadership for APPs in the Heart & Vascular ICU, the Progressive Case Unit, the OR and the ambulatory care clinic. Responsibilities:
The individual in this position will be accountable for human resource needs, workforce planning and development of staffing models, orientation, onboarding, professional development, employee engagement and satisfaction, budget planning, and partnership between the clinical service / service line and the Department of Advanced Practice.
The AP Manager will work closely with medical staff leadership in their respective division to maintain oversight of the implementation of performance measures and other operational goals as they relate to individual or team performance. The manager will have direct responsibility for the APPs across the continuum, integrating the boundaries of setting or entity.
The AP Manager, consistent with all nurse practitioners and/or physician assistants, will function under the guidelines as designed by the University of Pennsylvania Medical Center medical staff bylaws and is credentialed through that body. The Manager will be held accountable through a formal job description, the delineation of privileges, and through their agreement with their collaborating / supervising physician.
This position has a dual reporting structure. The Manager will report to the service / service line medical director for direct oversight of the clinical roles and practice, as well as the Director of Advanced Practice who is responsible for the administrative oversight of the position. They are equally accountable both to the clinical service / service line and the organization.
