The Clinical Social Worker is responsible for the delivery and implementation of direct clinical services and serves as a member of the treatment team. Provides clinical services to a population of chemically dependent adolescents with co-occurring and mental health problems. Also serves as a member of the interdisciplinary treatment team. The hours for this position are flexible based on need.
What You Will Do:

Facilitate various formats (i.e., individuals, groups, specialty groups) on time and in a professional, ethical and caring manner.

Manage all aspects of a caseload of 20 to 30 from date of entry to date of discharge with special attention to appropriate levels of care and appropriate clinical interventions.

Provide backup coverage in the event of other clinical staff absences.

Demonstrate initiative in task identification and completion; use time efficiently and work with the Site Director and Program Coordinator to effectively handle volume of work.

Address program issues with the Program Coordinator, which pertain to strategic objectives, annual plan and overall delivery of services. Participate in and support agency quality improvement programs; support actions to improve care and services based on outcome data.

Submit weekly schedules, ensure prior approval for offsite activities, monitor and enhance effective utilization of time.

Maintain computer skills that are appropriate to level needed for optimal job performance; proficient with using e-mail, Word Perfect and other programs as necessary.

Exercise due diligence in the delivery of quality care in line with agency Medicaid Compliance Plan and systems of accountability.

Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Psychotherapist (LMSW, LMHC or LCSW) to join our team at the HUB Health Center in Bronx.
Accomplished Psychotherapists (LCSW, LMSW, LMHC) evaluate and treat a diverse caseload spanning all DSM diagnoses at busy outpatient clinic. A strong commitment to community behavioral health and prior outpatient experience required.
Essential Duties and Responsibilities of a Psychotherapist (LMSW, LMHC or LCSW):
- Provide culturally sensitive direct, clinical services, which may include but are not limited to: assessment and treatment planning, individual, family and group psychotherapy, crisis counseling, and community consultation and education.
- Document client progress to maintain a permanent record of client activity according to established methods and procedures.
- Meet monthly target. Contingent on how many hours clinician works for the week.
- Progress notes due within 72 hours of services; preferably within the same day of service.
- Identify case management needs of clients and coordinate with other agency staff and community agencies to have these needs met.
- Perform crisis intervention as necessary.
- Coordinate treatment with other community agencies and services.
- Participate in program development and in training programs.
- Follow the established clinic policies & procedures.
- Maintain appropriate professional standards.
- Master's Degree (MSW or MHC)
- New York State Licensure (LCSW, LMSW or LMHC)
- Must have 2 years of experience
- Bilingual in English and Spanish (preferred)
Full-time annual salary ranges based on licensure are as follows:
- LMHC: $71,653.40- $0,644.00 Depending on Licensure and Experience
- LMSW: 72,982.00 - 84,380.66 Depending on Licensure and Experience
- LCSW: 86,977.80 - 100,493.14 Depending on Licensure and Experience
