Job Summary


The Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to patients. Responsible for the coordination of care, patient assessment, patient education, triage, and various other nursing interventions. Collaborates with other inter-professional colleagues, including physicians, to plan, implement and evaluate care. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned clinical setting. Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.


Qualifications


Graduate from an accredited nursing program preferred, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor’s Degree in nursing preferred.


Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific licensure, competencies and certifications.

**Job Description**
**Mount Sinai South Nassau seeks a Family Medicine Faculty Physician to join it's growing department!**
Recognized for administering personalized comprehensive patient care, innovative educational opportunities for students and residents and a developing research environment, our goal is to develop long-term healing relationships with patients and help them manage their care within an underserved area. The physician will be involved in direct patient care, following a small panel of patients in the outpatient setting of a full spectrum Family Medicine office. The faculty physician will serve as a preceptor to residents in the Family Medicine Center, provide inpatient pediatric and newborn care, and will be provided with time for administrative responsibilities which allow for teaching and scholarly activities. Care is provided to patients of all ages, including prenatal care. Intrapartum care and deliveries are not required but can be obtained if desired. The ideal candidate is board-certified in Family Medicine with a passion for teaching and striving to promote the values of Family Medicine and the mission of our ACGME accredited residency program. The physician will participate in faculty development, curricular development and implementation, disseminate scholarly activity, and serve as a role model for our residents. The program promotes residency and faculty well-being, as well as patient safety and continual improvement processes. Regular faculty evaluations help with formative and summative evaluations of the resident, and this faculty person will provide this, as well as serve on the clinical competency committee as well as the program evaluation committee. The faculty person may be responsible for the department Performance Improvement (PI) activities, so an interest or background in PI is preferred.
The Icahn School of Medicine at Mount Sinai (South Nassau) Family Medicine Residency Program has been fully accredited by the ACGME for more than 40 years. The Care Coordinator for Family Medicine determines the needs of high-risk patients and provides telephonic and/or in-person education related to positively impacting their quality of life and clinical outcomes. Partners with providers, staff, patients, and caregivers. Visits patients in the hospital, the office, the nursing home, or the home as needed. Acts as an advocate for these patients; links them to other care team members as needed to help patients gain knowledge of their chronic conditions and identifies community resources for continued growth toward a maximum level of independence. Collaborates with providers to coordinate and manage patients as needed. An active member in a patient-centered team approach in the care of patients and families. The physician will be joining a growing Mount Sinai practice that serves this rapidly changing, dynamic and growing neighborhood. Mount Sinai Doctors Midwood is located in the East Midwood section of the borough, serves patients from neighborhoods that include Midwood, Madison, Marine Park, Mill Basin, East Flatbush, Flatlands, Canarsie, Brighton Beach, and Sheepshead Bay. Candidates will work in our Mount Sinai Brooklyn hospital providing GYN services, as well as ambulatory services in our adjacent outpatient office building. There is very little obstetrics work associated with this position.

The **Care Coordinator for Family Medicine** determines the needs of high-risk patients and provides telephonic and/or in-person education related to positively impacting their quality of life and clinical outcomes. Partners with providers, staff, patients, and caregivers. Visits patients in the hospital, the office, the nursing home, or the home as needed. Acts as an advocate for these patients; links them to other care team members as needed to help patients gain knowledge of their chronic conditions and identifies community resources for continued growth toward a maximum level of independence. Collaborates with providers to coordinate and manage patients as needed. An active member in a patient-centered team approach in the care of patients and families. Works collaboratively with all team members in achieving shared goals with a consistent focus on quality care.

+ Participates in daily patient reviews. Maintains communication with other team members as well as office staff and providers. Communicates regularly with assigned patients either by telephone or with face-to-face encounters. Monitors patients' health status and communicates the self-management skills needed to maintain the highest level of function. Communicates issues and concerns back to providers and or office staff.
+ Collects health history data from patient, family, PCP, office staff, and other health care team members as needed. Utilizes critical thinking to identify the patients' bio-psychosocial, spiritual, cultural, environmental, and educational needs.
+ Collaborates with other members of the team as well as outside resources when needed to develop a comprehensive plan of care incorporating current diagnosis, interventions, beneficiary self-management skills, and educational needs. Takes into consideration all chronic conditions when creating the plan of care.
+ Documents all data and findings consistently and comprehensively under regulatory requirements. Completes documentation in the EMR on all patients. Focuses on the Rising Risk population, determines Health Care needs, and enters patients into Disease Coordination programs when applicable.
+ Write Care Plans in EPIC Healthy Planet for high-risk patients being care managed.
+ Other duties and/or projects as assigned.
+ Adheres to HMH Organizational competencies and standards of behavior.
**Education, Knowledge, Skills and Abilities Required:**
+ Graduate of accredited Registered Nursing Program.
+ Minimum 2-3 years experience in managed care or health education/coaching is preferred.
+ Must be able to prioritize, plan and handle multiple tasks/demands simultaneously.
+ Requires communication skills to engage patients/families over the phone and in person.
+ Experience working with large EMR/Health systems
+ Experience using Microsoft Word, Excel, and Outlook.

The Mount Sinai Doctors Hartsdale Medical Practice is a primary care practice serving Westchester County. Hartsdale is one of the few communities immediately surrounding New York City that still has two working farms, both on Secor Road. It also has several parks, including Secor Woods Park, Ridge Road Park, and Rumbrook Park. The town can generally be subdivided into several areas, including the "Village" or downtown part (East Hartsdale Avenue), Manor Woods, Windsor Park, Poet's Corner, Ridge Road, Orchard Hill, College Corners, or more specifically one of the several condominium developments built since the 1970s.
