Minimum Requirements:
1. Workforce Pathway

+ Relevant roles may include Family and Youth Liaison, Student Advocate, Community Service Coordinator, Behavioral Health Specialist/Technician, or similar positions.
2. Education Pathway
+ Recently graduated from a California college or university with an associate or bachelor's degree in one of the following fields:

The Wounded Warrior Project (WWP) Physical Health & Wellness (PH&W) Specialist develops and executes multiweek programs and events to educate, coach, and empower wounded service members in pursuit of achievement of their physical health and wellness goals.
DUTIES & RESPONSIBILITIES

Identify, screen, and recruit warriors for participation in quarterly virtual and in person Physical Health & Wellness events and coaching programs.

Plan agenda and execute multiday PH&W coaching program kick off events, providing fitness, nutrition, mobility, sleep, and health-focused education to warriors with limited to no supervision from manager.

Create individualized coaching programs to help warriors achieve their physical health and wellness goals.

Manage warrior participation throughout 90-day coaching program, providing encouragement, support, and connection to resources through regular coaching sessions delivered by telephone or Zoom.

Provide instruction, guidance, and coaching using motivational interviewing skills to encourage behavior change and support the progress of warrior’s health focused SMART goals.

Develop and execute in person events, virtual education, competitions, and wellness challenges for warriors and their families.

POSITION SUMMARY: The Health & Wellness Coach will ensure a safe, enjoyable, and positive environment that promotes wellness and engagement under the direction of the Health & Wellness Associate/Coordinator and Vice President of Clinical Integration. Supports the health and wellness needs of the people we serve, Intandem staff, as well as our community. Maintains safety standards for the facility and delivers programming that meets a diverse range of needs.
QUALIFICATIONS:
Minimum Education, Training & Experience:
High School Diploma
Two (2) years experience working with people with intellectual or developmental disabilities supporting health and wellness initiatives.
Preferred Education, Training & Experience:
Fitness or personal trainer certification, a degree in a health related field (Human Service, Social Science (or related Social Service), Exercise Science, Occupational Therapy (or Occupational Therapy Assistant), Physical Therapy (or Physical Therapy Assistant), etc.
One (1) year experience working with people with intellectual or developmental disabilities.
Or
An equivalent combination of education, training and experience deemed acceptable for the position.
ESSENTIAL SKILLS AND ABILITIES:
Valid New York State driver's license that meets the Agency's driving policy
Ability to work outside normal business hours when needed to meet agency needs
Possesses strong knowledge of physiology, exercise technique, and body mechanics
Possesses knowledge of safe use of gym equipment and ability to education/train the people on use of the equipment accordingly.
Ability to multitask and stay organized and maintain cleanliness of the facility
Positive, motivating and effective interpersonal communication skills
Excellent communication and customer attention skills to provide instruction, demonstration, and constructive feedback to gym staff/participants as needed to all fitness levels
Obtain and maintain BLS/CPR as directed
Possesses Functional knowledge of basic computer applications (i.e., spreadsheets, word-processing, e-mail, etc.)
Ability to safely lift and move up to 40 lbs
SALARY:
$18-$20 per hour based on experience

ABOUT THE JOB
The Health & Wellness Coach delivers one to one coaching sessions as well as group programs targeted at the Health and Wellness Center's population of client specific employees. The Health and Wellness Coach facilitates behavior change, life skills development to enable participants to manage their chronic conditions and prevent further disease progression. The Health and Wellness coach will facilitate, develop, monitor, measure, and report on health, wellness, and chronic disease prevention programs that complement the Health and Wellness Clinic. The Health & Wellness Coach will present information about how to improve health, prevent disease, and modify health behaviors to individual patients through one-on-one health coaching, as well as to client groups. These duties are carried out for a specific client at its various locations
ESSENTIAL DUTIES & RESPONSIBILITIES
- Develop, implement, and promote health, wellness, and chronic disease programs that emphasize the importance of healthy lifestyles. Some of these program topics will include, but are not limited to Tobacco Cessation, Diabetes Management, Hypertension Management, COPD Management, Cardiac Management, Exercise and Physical Activity, Weight Management, and Weight Maintenance. The programs will include step-by-step implementation plans, program details, resources needed, goals for success, and all tools needed to successfully complete the program
- Provide health and wellness coaching in both individual and group settings
- The Health & Wellness Coach is clinically integrated and provides enhanced education and coaching to support the clinical care received from the Health Center Providers.
- Develop and implement programs to improve Well-being of client population
- Measure, monitor, and report on efficacy of all health, wellness, and disease management programs
- Review client HRA information and clinical dashboards available, as applicable, and consult on wellness information, compliance program design, and effectiveness of wellness programs
- Participate in education sessions coordinated by clients for their respective, eligible employees
- Consult on wellness and disease management topics at the request of other benefit consultants, brokers and others in the wellness industry, as needed
- Prepare and distribute a list of available community health agencies, hotlines for drug/alcohol abuse, abuse, and crisis intervention to be available to and used by the on-site clinic providers in their contacts with patients
- Coordinate efforts with other vendors (carriers, EAP, etc.) to inform client population of all available resources
- Contribute to and maintain the database to stay current on new, innovative techniques to promote health, wellness, and chronic disease prevention and management
- Perform other duties as assigned
Bachelor's or master's degree in health education, health promotion, exercise science, nutrition, psychology, or other health-related subject and at least 3 years of experience in developing, monitoring, measuring, communicating, and reporting on health- related topics and programs or equivalent combination of education and experience. Certified Health Education Specialist strongly preferred. Completion certificate of National Consortium for Credentialing Health and Wellness Coaches (NCCHWC) from an accredited Health/Wellness Coach Program, strongly preferred or within 1 year of hire date. Eligible to sit for the National Board-Certified Health and Wellness Coach exam. Knowledge of HIPAA compliance and regulations. Experience creating wellness program materials, particularly in employer-based settings. Experience working as a health and wellness coach or integrating wellness/fitness initiatives into the culture of an organization.
