Maximus is seeking a dedicated Clinical Services Manager to support the Colbert Consent Decree Illinois Outreach and Assessment team. In this role, you will act as a liaison to serve as a single point of contact for all transition entities. This would include communicating with Illinois state agencies, MCO's (managed care organizations), court appointed staff, and transitional prime agencies.
**This position requires up to 15% travel annually to Illinois.**
About the Program:
We have contracted with the State of Illinois Department of Healthcare and Family Services (HFS) to manage comprehensive assessment processes for various screening and assessment services, specifically the Williams and Colbert Consent Decrees Outreach and Transitional Assessment. Outreach includes in-person visits with Colbert Class Members in Cook County Nursing facilities and Williams Class Members in SMHRF facilities which provide education on their rights and evaluate their level of interest in potential transition to the community.
Transitional Assessments are conducted in-person with Colbert Class Members in Cook County Nursing facilities and Williams Class Members in SMHRF facilities interested in transitioning to the community. Evaluators assess individuals' functional needs in several areas to determine the level of support necessary for successful community transition.

Replaces pulse oximeter probe on patient’s finger; observes pulse oximeter readings while providing care and notifies nurse or RCP of low readings. Reports pulse oximeter , ventilator or telemetry alarms to the RCP or RN. Performs simple procedures such as non-sterile dressing changes under the direction of a registered nurse. Performs common bedside tests including blood glucose monitoring, and checking stools for occult blood. Reports findings immediately to the patient’s nurse, and documents in Meditech . Observes and reports patient signs and symptoms and responses to treatments. Reports changes in the patient’s condition to the registered nurse and respiratory therapist. Assists with admissions and discharges. Orients patient and family to room, unit and environment. Handles patient belongings carefully and records them upon admission. Collects and packages patient belongings at discharge. Observes or reports the emotional and spiritual needs of patients and assists in providing supportive care. Answers call lights and responds to needs for all patients, not just those assigned. Assists other staff members in providing care to any patient on the unit. Offers assistance to other staff without being asked. Performs related services and duties such as cleaning equipment, obtaining and replacing patient room supplies, and similar related activities. Ensures that all supplies used are charged according to policy. Insures the enhancement of personal knowledge and skills by participating in continuing education programs and reading pertinent literature, etc. Maintains confidentiality of the patient’s clinical and personal information and medical record. Working Conditions Work is performed in a patient care environment. Work is performed in a restricted area which requires adherence to departmental attire policies. Support staff on the unit includes Stroke Coordinator, Respiratory Therapy, physical and occupational and speech therapy, case management, nutritional services, social work, pharmacy, pastoral care, and community nursing service
The unit is managed by the Nurse Manager with a designated charge nurse each shift. The house nursing supervisor provides leadership during off hours and on weekends. Neurologists, Neurosurgery, and hospitalist support is available either in-house or by pager 24/7.
Flexible self-scheduling, with designated third weekend commitment and accommodating three holidays per year.
