**The Assistance Program Counselor Lead:**
+ The Assistance Programs (AP) Counselor Lead is a Master's level licensed provider.
+ The AP Counselor Lead quickly and efficiently gathers basic demographic information, conducts an assessment of the individual's needs, determines level of risk and safety, makes referrals to providers, and when needed makes recommendations or forwards information to assist member in obtaining needed resources.
+ The AP Counselor Lead provides brief solution-focused counseling when caller is in crisis and other services as needed, is knowledgeable about community resources, and maintains appropriate documentation of all contacts according to department procedures.
+ The AP Counselor Lead is able to provide on-site services as needed, including counseling, risk assessment, and adverse incident debriefing and support.
+ The AP Counselor Lead has clinical and documentation oversight of higher level cases including mandatory and fitness-for-duty evaluation.

In collaboration with physicians and the interprofessional health care team, this position sensitizes other health care providers to the social and emotional aspects of a patient's illness to facilitate efficient, quality care and achievement of desired treatment outcomes. The SW I position holds joint accountability with Case Manager for discharge planning and continuity of care, assuring that psychosocial issues are addressed and treated as needed across the continuum of care.
**PEOPLE ESSENTIAL FUNCTIONS**
+ Communicates in an active, positive and effective manner to all health care team members and reports pertinent patient care and family data in a comprehensive and unbiased manner; listens and responds to the ideas of others. Uses therapeutic communication to establish a relationship with patients and families and communicates the discharge plan, facilitating transitions and hand-offs. Supports patients and families in clinical or ethical issues.
+ Uses patient and family-centered approach in collaboration with interprofessional health care team. Facilitates discharge planning activities for assigned patients and collaborates with the case manager and other members of the interprofessional health care team, as well as patient and family. + Develops and implements a comprehensive psychosocial treatment plan utilizing appropriate clinical social work diagnoses, treatments and interventions, including crisis intervention, brief individual, marital and family therapies, and patient, family and caregiver groups. Assists with screening, identification, diagnosis, management and treatment of victims of abuse, neglect, and domestic violence and of mental health and/or substance abuse problems in patients and family members.

Completes screening by patient/family interview, review of the medical record including previous episodes of care, H&P, lab and other test results/findings, plan of care, physician orders, nursing and progress notes. Uses clinical knowledge and screening tools to identify need for case management and/or social work intervention.

+ Uses knowledge of levels of care, working with patient and family, to ensure discharge disposition is to the appropriate level and facilitates transfers. Provides brief, goal-directed counseling services to assist patients/families to cope more effectively with the transition.
**QUALITY/SAFETY ESSENTIAL FUNCTIONS**
+ Modifies care based on continuous evaluation of the patient's condition, using problem-solving and critical thinking, and makes decisions using evidence-based analytical approach. Documentation reflects completed patient screening/assessment and reassessment upon admission and concurrently, as needed. Considers variables that impact treatment plans including diagnosis of emotional, social, and environmental strengths and problems related to their illness, treatment and/or life situation.
+ Continuously reviews the patient for opportunities for care facilitation and needs for discharge planning. Works with case manager for routine discharge and anticipates/prevents and manages/elevates emergent situations with specific focus given to discharge plan and elimination of psychosocial barriers.
+ Collaborates with staff from the interprofessional health care team concerning safety data to improve outcomes and the safe transition of care through effective patient handoffs
**FINANCE ESSENTIAL FUNCTIONS**
+ Completes timely and thorough assessment on all unfunded patients to identify community resources required for effective transition and able to utilize alternative resources to fill gaps in established community resources.
+ Establishes an effective community resource knowledge base and the judgment/ability to effectively select and coordinate available resources, including referrals to regulatory agencies, i.e. CPS/APS.
+ Provides timely, efficient pertinent patient information to healthcare team when coordination with significant or intensive resources is necessary for effective discharge planning outcomes and decreased length of stay.
**GROWTH/INNOVATION ESSENTIAL FUNCTIONS**
+ Provides education to unit-based physicians, nurses, and other healthcare providers on community resources and psychosocial impact on care needs.

The Recovery Counselor will assess the client’s personal, medical, emotional, social, and environmental situation to plan for treatment course through home, community, and office visits. He/she/they will provide individually-based motivational treatment and support to clients to assist them in their recovery from mental illness. The Recovery Counselor will be an advocate for clients, and link them to community services as needed. He/she/they will provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illnesses and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach.

The Recovery Counselor will provide at least 75% of all community support services in the community. He/she/they will complete case management tasks with clients within a shared caseload. The Recovery Counselor will be responsible for documenting all services provided to clients according to agency policy and state requirements. The Recovery Counselor will also provide crisis/on-call coverage as directed.

RESPONSIBILITIES

  • Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
  • Coordinate the establishment of an individual recovery plan with the client, client’s support system, and other care providers.
  • Complete daily progress notes based on services provided to clients in a timely manner reflected in Trilogy’s documentation policy and agency standards
  • Complete all necessary documentation related to client care e.g. IMCANS, LOCUS, tracking forms in the Electronic Medical Record
  • Recognize and act on opportunities to move clients to appropriate levels of care; provide referrals to additional services as indicated.
  • Assist clients in identifying signs and symptoms of de-compensation, assess for crisis situations and or the need for stabilization through hospitalization. Work collaboratively with clients to identify coping skills to assist in managing symptoms and stressors experienced.
  • Provide psychoeducation, medication training and monitoring to clients according to Trilogy policy
  • Coordinate with team nurse/pharmacy to ensure client medication accuracy
  • Documenting in real time on medication administration record (MAR)
  • Assist clients in learning and improving independent living skills; i.e. personal hygiene, housekeeping skills, nutrition, and shopping for food and personal items
  • Educate and assist clients with applying for entitlements; i.e. Supplemental Security Income, Social Security Disability Insurance, Medicare, Medicaid, and LINK
  • Understand the representative payeeship process per agency policy and providing individualized client money management services.
  • Accompany and transport clients to important appointments in the community and provide support with issues related to housing, substance use, budgeting, social support, and medication.
  • Advocate on client’s behalf and empower clients to advocate on behalf of themselves when appropriate
  • Communicate effectively within the team model for a multi-disciplinary approach to client care.
  • Actively participate in team communication via multiple electronic platforms
  • Engage regularly in daily team meetings to consult and collaborate on shared caseload
  • Provide on-call and crisis coverage as assigned.

QUALIFICATIONS

  • Must have one of the following requirements:
    • High School Diploma or GED and two years of supervised clinical experience in a mental health setting. 
    • Associate’s or Bachelor’s degree in any field. 
    • Licensed Practical Nurse under the Nurse Practice Act (225 ILCS 65)
    • Certificate of Psychiatric Rehabilitation from a DHS-approved program, a high school diploma/GED, and two years’ experience of experience in a mental health setting
    • CRSS certification through IAODAPCA
    • Family Partnership Professional certificate from and in good standing with IAODAPCA
    • Occupational Therapy Assistant licensed under the Illinois Occupational Therapy Practice Act (225 ILCS 75) with one-year experience in a mental health setting
    • High school diploma or GED and five years of supervised clinical experience in a mental health setting
  • Valid Illinois driver’s license with daily access to a well-maintained vehicle with $1,000,000/$3,000,000 liab lity insurance

Domestic Violence Shelter

HELP Haven provides emergency services, trauma-informed care, and a safe environment for women, men, and children who are survivors of domestic violence. Our social service, clinical, and housing teams work with each client to locate housing in a safe borough.

Our Domestic Violence Shelter is located in:

Domestic Violence Program

HELP R.O.A.D.S. is a comprehensive, non-residential, individual and family safety program providing a variety of services designed to assist in empowering survivors of domestic violence, other crime victims, and their families. The program addresses the effects of abuse, neglect, and trauma and helps survivors and their families achieve safety and long-term stability.

Our participating Domestic Violence Program is located in:
  • Brooklyn
Mental Health Shelter

Our Mental Health Programs provide therapeutic services to single men who have a mental health diagnosis or are experiencing mental health symptoms. The men reside in a congregate setting or a single unit, depending on their mental health needs. Our clinical, housing, and social service teams collaborate to assist with the stabilization of each individual's mental health needs in order to identify a concrete housing plan.

Our Mental Health Shelter is located in:
  • Wards Island (aka Randall's Island), located between Manhattan & the Bronx (Clarke Thomas).
Supportive Housing Program

Our Supportive Housing Program provides supportive services to formerly homeless single adults with serious mental illness or co-occurring substance use. For over fifty years, New York Junior Tennis & Learning (NYJTL) has honored its founder Arthur Ashe's legacy by celebrating the diversity of children, encouraging them to reach their highest potential through free tennis and academic programs. NYJTL is the largest provider of youth tennis and education programs in the nation, providing services in 100 unique sites across all five boroughs while reaching nearly 90,000 kids. In addition, NYJTL designed, funded (with NYC), built and operates the Cary Leeds Center for Tennis & Learning in the South Bronx. All NYJTL community tennis and after school programs are offered at no cost to its participants.

The mission of NYJTL is to develop the character of young people through tennis and education for a lifetime of success on and off the court. It believes life skills gained through tennis, academics, healthy living, and character development are a catalyst for long-term achievement.

Position Summary:

This internship provides social work students with hands-on experience in school-based settings, supporting middle school youth through NYJTL's afterschool programs. Interns will build skills in group facilitation, crisis response, and family engagement, while working closely with an experienced Social Worker. The role emphasizes developing practical competencies in assessment, intervention planning, and advocacy, all within a trauma-informed and youth development-focused framework. Interns will contribute meaningfully to students' social-emotional growth and school connectedness.

Learning Objectives:
  • Facilitation of SEL Groups : Develop and lead 2-4 small group sessions per week using a Mental Health Skill Building curriculum to promote emotional literacy, self-regulation, and peer connection.
  • Student Support & De-escalation : Identify students in crisis and implement trauma-informed, restorative approaches to support their emotional and behavioral needs within the afterschool environment.
  • Pre- and Post-Survey Implementation : Assist with the collection, organization, and basic analysis of student surveys (e.g., Hello Insight) to assess social-emotional growth and program impact.
  • Family Engagement Strategy : Collaborate with site directors and school staff to build sustainable strategies for increasing caregiver engagement and communication.
  • Collaborative Practice : Partner with afterschool staff, site directors, school leadership, and school-based mental health providers to ensure coordinated, holistic student support.
  • Culturally Responsive Practice : Strengthen cultural humility and trauma-informed approaches to engage students and families from diverse backgrounds in an inclusive and affirming manner.
Core Responsibilities:
  • Facilitate 2-4 weekly small group sessions using NYJTL's SEL and Mental Health Skill Building curriculum.
  • Support enrichment and classroom staff with behavior management strategies and restorative responses to student needs.
  • Assist in collecting, organizing, and analyzing pre- and post-program evaluation tools such as Hello Insight surveys.
  • Identify and engage with students in distress, support the development of student de-escalation plans, and document interventions.
  • Collaborate regularly with site directors, afterschool leaders, and school personnel to align efforts across school-day and afterschool supports.
  • Participate in weekly individual and group supervision to reflect on practice, receive feedback, and continue professional growth.
  • Lead or co-lead intern projects such as creating community resource guides or mental health toolkits for staff and families.
  • Observe group and individual interactions to inform group placement and identify emerging student needs.
  • Document student interventions, group notes, and engagement data in a timely and organized manner.
  • Fulfill additional responsibilities as delegated by the Practicum Instructor to support site and program goals.
Qualifications:
  • Successful completion of DOE and DOH background checks.
  • Enrolled in a social work (BSW or MSW) program.
  • Adherence to organizational policies and procedures.
  • Knowledge of child development, particularly middle school-aged children.
  • Familiarity with behavior management and trauma-informed care.
  • Cultural competence and sensitivity to diversity.
  • Familiarity with program evaluation concepts or willingness to learn.

Job Title: Soaringwords Social Work Supervisor (Licensed LCSW with SIFI Certification)

Position Summary:

Soaringwords is seeking a Licensed Social Worker with SIFI certification to supervise Master of Social Work (MSW) interns who will serve as mental health facilitators leading SOARING Into Positive Wellbeing workshop series. These interactive, empirically-supported workshops are delivered to residents living in 100 long-term care skilled nursing facilities throughout New Jersey. Soaringwords is expanding this program to other states in Fall, 2025. The position can expand as new states sign on to fund this initiative.

The Social Work Supervisor will work closely with Lisa Buksbaum, Soaringwords Founder & CEO to lead bi-monthly Community of Practice Zoom training and supervision sessions where the Soaringwords MSW interns can reflect, learn, and share best practices related to workshop facilitation and group dynamics in long-term care.

Responsibilities:

  • Provide clinical supervision to MSW students (virtually) for 46 hours/week
  • Lead bi-monthly Community of Practice Zoom sessions
  • Offer feedback and guidance aligned with Positive Psychology and Appreciative Inquiry principles
  • Collaborate with Soaringwords CEO to support interns professional development and clinical learning
  • Ensure supervision adheres to academic practicum requirements from several leading Master of Social Work programs (for example - NYU, Hunter College, Fairleigh Dickinson, Columbia, Rutgers)

Qualifications:

  • SIFI Certification (Seminar in Field Instruction) required. If you do not already have SIFI certification you can attend the 12 one-hour online sessions while you are in this position. Just confirm youre open to doing this as it is a requirement.
  • Passion for positive psychology, strengths-based interventions, and bringing positive interventions to benefit aging populations
  • Strong communication and virtual facilitation skills

  • 95% of workshop participants discovered and appreciated their strengths
  • 83% of workshop participants learned and used new skills
  • 88% of workshop participants experienced a heightened sense of community, connection, and belonging

Why there is a Tremendous Need for the SOARING Into Positive Wellbeing workshop initiative: Soaringwords is leading the way to create a new paradigm of Positive Aging:

In many parts of the world, life expectancy has increased, while the quality of life for seniors has diminished. This trend is especially visible in Western cultures with prevalent disparaging attitudes about aging and an obsession with youth. Stereotype Embodiment Theory, (Levy) Stereotype Embodiment Theory proposes that negative age beliefs bring about detrimental health effects, misleadingly characterized as inevitable consequences of aging. Common negative tropes include: incompetent, senile, decrepit, slow, and out of touch. Positive age beliefs add 7.5 years of life. Soaringwords Appreciative Inquiry workshop prompts and content ignites group discussions that foster strengths-based recognition, agency, resilience, and social interaction.

Soaringwords is dedicated to transforming the way our society views aging by introducing positive psychology interventions into long-term care settings. We focus on empowering seniors and caregivers with tools to promote mental, emotional, and physical wellbeing. Our workshops are designed to counter the negative stereotypes associated with aging and offer seniors the opportunity to live more fulfilling lives despite the challenges they face.

Soaringwords approach is built on a strong foundation of positive psychology, appreciative inquiry, and posttraumatic growth. These approaches focus on practices that bring out the best in individuals and help to enliven those facing the natural limitations of aging. Soaringwords empirically-based immersive 7-part workshop series is built on positive psychology interventions designed for older individuals residing in long-term care who have traditionally been overlooked.

The 7 workshop sessions are delivered in person to groups of 15 to 30 residents in long-term care, and also shared with groups of employees working in long-term care. Each session lasts from 60-90 minutes with pre-recorded professional video content interspersed with many immersive appreciative inquiry prompts. The seven topics are taken from the SOARING Into Positive Wellbeing Model . The content is built upon Positive Psychology interventions - including but not limited to: VIA Strengths analysis and strength-spotting among all workshop participants (Niemec); Exploration of The 5 Domains of Posttraumatic Growth (Tedeschi & Calhoun); Growth Mindset exercises (Dweck); Positive Narrative exercises (Pennebaker); Gratitude Visit (Tomasulo) and Gratitude Letter (Seligman); Best Future Self (King) and dozens more.
