Job Summary

Looking for an experienced medical director to lead our palliative care practice at Baylor Scott & White Medical Center McKinney. The medical director will manage the day-to-day operations of the group, including staffing, schedule oversight, and quality initiatives, while also collaborating with the hospital departments and leadership. The medical director will enjoy the well-established practice and supportive hospital leadership.

The Physician HTPN examines and treats patients with various conditions. They take patient histories, request tests, make diagnoses, and prescribe treatments. They administer vaccinations, perform follow-up exams, and educate patients on disease prevention and health maintenance. The Physician HTPN supervises the clinical activities of allied health and advanced practice professionals.

Essential Functions of the Role

  • Chair the Hospital Palliative Care Committee.
  • Serve as Hospital lead for Palliative Care Consultation Services
  • Develop, review, maintain and execute ongoing multi-year Strategic Plans for Clinical Ethics and Palliative Care (CE&PC) delivery at the Hospital;
  • Serve as working member of BHCS System Ethics and Palliative Care Council (SEPCC).
  • Develop and execute programs for training of physician and non-physician staff to deliver CE&PC services both on inpatient and outpatient basis.
  • Provide leadership for the physicians and staff to enhance clinical expertise in the care of patients suffering with pain and other symptoms associated with the treatments of advanced life-limiting disease.
  • Supervise the clinical activities of a care team that may consist of other providers, and allied health professionals.

The Clinical Excellence Quality (CEQ) Pharmacy Operations team supports Optum's Care Delivery Organizations by delivering patient-centered, clinically collaborative medication consultations to help people live healthier lives. Our team focuses on the design, execution, and delivery of telephonic and digital engagement strategies designed to close Medicare star measure gaps in care. Our interdisciplinary service delivery team is comprised of Pharmacists, Registered Nurses (RNs), Pharmacy Technicians, and Care Coordinators.
Clinical pharmacists perform telephonic, patient-centered clinical consultations focused on the following care opportunities: medication adherence, SUPD/SPC, Care of Older Adults & A1c medication reviews, and polypharmacy measures, including opioids/benzodiazepines and anticholinergic medication use. Clinical pharmacists also consult with providers, pharmacies, internal departments, and/or other external third parties to drive improved patient outcomes. In addition, clinical pharmacists field both clinical and social determinants of health-related needs with members directly or from escalations by other clinical and non-clinical staff.
Clinical pharmacists are required to meet or exceed established productivity and quality metrics, though can work a flexible schedule to support the hours of operation (Monday - Saturday 9:00AM - 6:00PM CT). Clinical pharmacists work a rotating Saturday schedule with approximately 6 shifts per calendar year.

**Primary Responsibilities:**
+ Conduct telephonic medication consultations that meet/exceed production & quality metrics

**Overview**
We are looking for a **Clinical Manager - AIM (Advanced Illness Manager) Program** to join our team based in Plano, TX. This position will directly report to the Director of AIM and is responsible for coordinating and managing all day to day business affairs and other clinical activities related to the administration of the Advanced Illness Management (AIM) and Palliative Care department.
**Position is located in Addison, TX - working in the branch location Monday-Friday.**
+ Ensures clinical care delivery goals are met and exceeded.
+ Manages AIM Clinical Coordinators, AIM NPs and AIM RNs.
+ Ensures scheduling, managing frequency adherence, clinical coordination, medical record maintenance function, and marketing functions are maintained for a team of patients and clinicians.
+ Managesincoming calls from partners and patients.
+ Manages Visit Freq compliance, scheduling PRN visits, post hospitalization follow up with patient/family-expediting discharge home with AIM communication with partners, schedule post hospitalization visits.
+ Orders labs, DME, imaging.
+ Coordinated results with NPs, enters telephone encounters in partner EMR, and enters routine visit med reconciliation in partner EMR.
+ Ensures transitions to HSP referrals to CTC for care type.
+ Assists in appropriate identification of HSP eligible pts.
+ Identifies HH providers that can accept HH referrals from PCC.
+ Identifies and communicates with local partner office for lab drop off.
**About You**
+ Active and unrestrictive RN license to practice in the state without any board action
+ Registered Nurse with Home Health, Hospice and/or Palliative Care Experience preferred
+ Bachelor's degree in nursing, Business Administration, or related field; or equivalent experience and knowledge in Health Care Operations required.
+ Master's degree in business or related field preferred.
+ Minimum of 2 years health care administration/operations management required
+ Must be able to work some nights/weekends on call.
+ Certified in Hospice and Palliative Care - preferred

+ Collects identification and payor related documentation, conducts registration using EPIC workflow.
+ Measures and documents vitals and communicates the resulting data to the consumer and provider, including temperature, pulse, oxygen saturation, respiration, blood pressure, height, weight, and pain level.
+ Documents chief complaint, allergies, health history, and current medications accurately in EPIC for review by provider.
+ Performs and documents laboratory testing and collections per provider orders and/or standard operating procedures, to include venipuncture with prior venipuncture competency check-off.
+ Administers and documents oral and intramuscular medication using the appropriate medication verification workflow.
+ Assists provider with procedures to include, but not limited to maintaining sterile field, laceration repair, I&D, and application of ortho-goods and assists with ortho-glass application.
+ Behave in a professional and empathetic manner to build trust, comfort, understanding, and relationships with consumers and secondary guest(s) and in-line with patient experience standards.
+ Welcomes the consumer and secondary guest(s) to the center (including eye contact, smile, greeting, and personal introduction) and inquires about the purpose and nature of the visit.
