Under the direction of the Chief Operating Officer, the Clinical Support Specialist (CSS) will ensure that instED seamlessly integrates into a patient's longitudinal care and does not fragment their care in any way. The CSS will facilitate effective post-visit care and support for instED patients, whether it is to provide basic health education information, ensure they were able to fill prescriptions or secure transportation, make appointments with PCP or other providers, navigate to additional support from their health plan (e.g., care managers, benefit or network information), or connect with community supports to address social determinants of health challenges that may impact their health outcomes and result in additional acute care episodes. instED is not only accountable for the patient's care on the day of the visit, but we want to fulfill our responsibility to link the patient to crucial follow-up care, as well as any non-clinical support they may need to secure that follow-up care.

Essential Duties & Responsibilities and Working Conditions
- Field lab-related questions from several different lab vendors, patients, and providers
- Respond to patient calls regarding test results, prescriptions, and other non-urgent clinical matters
- Monitor and respond to Clinical Support email inbox and escalate to nursing team, as indicated
- Cross-train as a Mobile Health Coordinator to conduct intakes and process requests for referrals
- Review, validate and process medical records requests from patients in a timely manner
- Ensure instED visit notes are consistently sent to patients' PCP and other pertinent care providers and connect PCPs to clinicians when necessary
- Conduct follow-up phone calls to patients after an instED visit, as directed by the nurses and providers
- Manage the administrative aspects of the Athena clinical inbox
- Follow-up on safety concerns reported by mobile health provider workforce
- Maintains knowledge of community resources in MA
- Initiate and facilitate patient referrals to various community organizations and primary care providers in their health plans' networks
- Provide ongoing daily administrative support to the Clinical Resource Center nurses as directed
- Coordinate follow-up care with external care teams, as directed by the Virtual Medical Control providers
- Transfer/escalate issues to licensed independent practitioners as appropriate
- Document and report on successful and unsuccessful patient outreach
- Participate in quality and process improvement initiatives
Required Education (must have):
- High School Diploma required
- Completion of a Medical Assistant, Community Health Worker, or similar training program

The Clinical Support Nurse supports high quality, patient-centered care by offering feedback, guidance, and mentoring to new graduate nurses and/or nurses that are transitioning to a new specialty. The Clinical Support Coord RN Resource supports multiple units, providing guidance and education on the hospital's standard of nursing practice, policies, and procedures to ensure compliance with all accrediting and regulatory agency requirements.
**What You Will Do In Your Role:**
+ You will perform rounds on each nursing unit, continuously observing, assessing the quality of patient care, and identifying nursing education needs
+ You will escalate concerns about nursing skills, patient care, policies, processes, and equipment, as appropriate
+ You will provide leadership by working cooperatively with nursing and other patient care team personnel in maintaining standards for professional nursing practice in the clinical setting
+ You will be responsible for being a resource for RN's with questions and for training purposes, sharing knowledge/expertise
+ You will demonstrate critical care skills for managing acutely ill patients including age specific needs for the geriatric
**What qualifications you will need:**
+ Current Registered Nurse License in the State of Texas or Multi-State Compact License (Employees with RN Compact License are required to obtain Texas RN License within 90 days of hire date)
+ Graduate of an accredited school of professional nursing is required
+ BSN required / Master's Degree is preferred
+ 1-year Leadership role is preferred
+ 2 years of RN experience in Acute Inpatient, ED and/or ICU in an acute care setting is required
+ Current BLS & ACLS (American Heart Association or American Red Cross)

Summary: The Clinical Support Representative (CSR) supports the practice by performing clinical and administrative duties including assisting patients in exam rooms, updating documentation, and cleaning and sterilizing equipment.
Responsibilities:
+ Patient Care Accountabilities:
+ Promptly rooms patients, using EMR to alert provider that patient is ready.
+ If requested by patients, act as chaperone / patient observer during appointments or procedures.

+ Ensures that the patient care areas are kept neat, orderly, well stocked and properly prepared.
+ Cleans and disinfects instruments and equipment according to protocols and checks that the instruments and equipment are in proper working order. Notifies Supervisor of malfunctions. Sets up equipment and instruments as per practice protocol. Cleans and restocks equipment as needed. Authorized to access secured area to obtain items such as intravenous solutions and medication preparation supplies for the purpose of stocking and maintaining pars in the medication carts. Secures stock items in appropriate location.
+ CSRs, if applicable as per regulatory guidelines: Maintains medication storage closet inventory and security checks expiration dates removes outdated items.
Other/Regulatory:
+ Ensures compliance with all applicable federal, state, and local regulatory standards (ex TJC, DOH, FDA, HIPAA, HCFA, DPW, LCGME, SCGME, etc.).
+ Flexible and readily adopts new processes and engages in practice operation changes.
+ Performs duties in accordance with Penn Medicine and entity values, policies, and procedures.
