+ Perform duties in a manner that will promote and maintain good public relations.
+ Assume other duties and responsibilities as assigned.
+ Drive a company vehicle safely to work locations for the organization.
**WHAT YOU NEED TO HAVE:**
+ Must be able to read maps, identify local tree species and growth rates.
+ Knowledgeable about Utility Vegetation Management practices.
+ Ability to hike ROW's and drive for extended periods of time required.
+ Must be able to work alone, outdoors in various weather conditions and terrain.
+ Must have exceptional interpersonal and communication skills, strong problem solving and multi-tasking abilities and show attention to detail.
+ Must be able to adapt to change.
+ Must have basic computer skills (Microsoft Suite).
**WHAT WOULD BE AMAZING TO HAVE:**
+ Associate or Bachelors degree in Forestry, Environmental Science, or a related field preferred.
+ Six months or more direct UVM experience preferred (or forestry combined with equipment and tree trimming practices).
+ ISA Certified Arborist or ability to gain with 6 months of start preferred.
+ Prior experience with GIS software/technology a plus.
+ Being Bi-Lingual (Spanish) is a plus.
**Working Environment** :
Work alone in an outdoor field environment; vehicle and foot patrol of utility power lines; remote locations as well as urban, city environment; physical demands and travel from site to site, including walking/hiking on various surfaces including flat, dry, wet, slippery, uneven, rough, steep terrain, hills, and slopes; exposure to noise, dust, grease, and all types of weather and temperature conditions; exposure to hazardous traffic conditions. **GENERAL OVERVIEW:**
This job collaborates with the interdisciplinary team of providers, clinicians, health plans, and external partners while advocating for patients and families/caregivers to coordinate care across the continuum. The incumbent is an integral member of the Care Management team that works to improve the quality of care, patient experience, and the health of populations and individuals by focusing on the social determinants of health impacting wellness.
**ESSENTIAL RESPONSIBILITIES**
+ Contributes to and/or completes initial and ongoing comprehensive assessment.Provides interventions and implements recommendations after engaging patients and their caregivers/families.Focuses on the individual's risk related to social determinants of health to assure successful coordination of care across the continuum. (30%)
+ Collaborates to provide the safest transition plan for assigned patients (Inpatient/Observation/ED) to ensure a timely discharge and provide appropriate connection with post-discharge care providers and community-based resources. (25%)
+ Educates patient, family/caregiver and physician regarding most appropriate level of care post discharge and how to access community support.Advocates for the patient, family/caregiver through effectively communicating with interdisciplinary team members, payers and post-acute partners to assure the safest transition. (20%)
+ Serves as a resource to provide counseling and intervention related to treatment decisions and end-of-life issues.Drives collaborative conversations to establish goals of care.Provides crisis interventions in cases involving Child Abuse and Neglect, Domestic Violence, Adult and Older Adult Abuse, Institutional Abuse Sexual Assault, Mental Health Disorders, Substance Use Disorders, and Identification of a Surrogate Decision Maker/Guardianship. **QUALIFICATIONS:**
Minimum

This job collaborates with the interdisciplinary team of providers, clinicians, health plans, and external partners while advocating for patients and families/caregivers to coordinate care across the continuum. + Collaborates to provide the safest transition plan for assigned patients (Inpatient/Observation/ED) to ensure a timely discharge and provide appropriate connection with post-discharge care providers and community-based resources. (25%)
+ Educates patient, family/caregiver and physician regarding most appropriate level of care post discharge and how to access community support.Advocates for the patient, family/caregiver through effectively communicating with interdisciplinary team members, payers and post-acute partners to assure the safest transition. (20%)
+ Serves as a resource to provide counseling and intervention related to treatment decisions and end-of-life issues.Drives collaborative conversations to establish goals of care.Provides crisis interventions in cases involving Child Abuse and Neglect, Domestic Violence, Adult and Older Adult Abuse, Institutional Abuse Sexual Assault, Mental Health Disorders, Substance Use Disorders, and Identification of a Surrogate Decision Maker/Guardianship.
