Olive Grove Hospice is seeking a Registered Dietitian on a PRN basis as we grow, to conduct education to our team and function as a member of the interdisciplinary group with expertise in nutritional care for our patients.

The duties and responsibilities for this position as we grow would be:

Develops and implements a nutritional plan of care which is based on an assessment of nutritional needs as part of the patient's total plan of care. Evaluates and reports results appropriately.

The dietitian works with the referring physician and interdisciplinary group to provide appropriate counseling to individuals and families/caregivers. They evaluate and instruct on nutritional principles, dietary plans, food selection and economics, and adapting plans to the individual's illness, as indicated. They will reevaluate nutritional care by providing home follow-up care either at the patient's residence or by phone.

Records subjective and objective information, assessment, treatment plan, and current progress notes in the patient's clinical records including a discharge summary.

Complies with the organization's policies and procedures. Maintains confidentiality and safety of patient records so that no information is divulged to other than proper users.

Actively participates in quality assessment performance improvement teams and activities.

Attends interdisciplinary team case conference, in-service training and the company's QAPI program, as appropriate.

Qualifications:

Must be a graduate of a school approved by the Accreditation Council for Education in Nutrition and Dietetics. (ACEND)

Certified, Licensed or Registered by the state.

Must have at least one year of experience.

The ability to communicate well, both verbally and in writing.

Must be a licensed driver with an insured automobile in good working order.

2. Determines dietary needs of patients from information gathered from patients' charts in consultation with medical staff, and other sources.
3. Calculates special diets ordered by physicians.
4. Calculates weighted diets for patients whose intake must be specifically limited.
5. Authorizes food substitutions to be made by food services staff in diet order for patients.
6. Observes, analyzes, and records diet program and patient response to diets served to evaluate effectiveness of dietary programs, to determine necessary diet changes or supplementary feeding, and to provide dietary information for related medical purposes.
7. Counsels patients regarding normal and therapeutic dietary needs
8. Inspects prepared food, and observes food service for compliance with diet orders and service procedures and for conformity with dietary policies and standards.
9. Confers regularly and periodically with the administrator to review progress, exchange information and discuss future goals.
10. May coordinates menu planning, purchasing, and food preparation.
11. Promotes food production and service procedures which protect quality and appearance of food as served to patients and personnel.
12. Assists in planning menus and nourishment lists.
13. Develops and conduct in-service training for dietary personnel, and others when appropriate.

+ Consults and educates physicians and other healthcare providers regarding member and population specific nutrition/health related issues.
+ Participates in multidisciplinary care teams and committees.
+ Collects data, plans, prepares and implements reports for performance improvement and/or quality assurance audits.
+ Develops grants and proposals for health/nutrition programs.
+ Plans, conducts and evaluates in-service education programs for department staff, the multidisciplinary care team and physicians.
+ Individual Medical Nutrition Therapy (MNT):
+ Provides high quality, evidence-based, efficient, customer-focused MNT, including nutrition screening, assessment, care planning and education for appropriate nutrition intervention.
+ Develops, implements, and monitors ongoing plan of care based on the screening and assessment of the patient specific nutrition care plans/education and based on the patients age, nutrition and psychosocial status, diagnosis, cultural background and religious affiliation.
+ Other duties as assigned.
Basic Qualifications:
Experience
+ Previous experience in providing and coordinating health education services (usually 1 year) and/or hospital experience.
Education
+ Bachelors degree in Food/Nutrition or related field.
License, Certification, Registration
+ National Provider Identifier required at hire
+ Registered Dietitian within 6 months of hire
Additional Requirements:
+ Strong interpersonal and written communication skills.
+ Demonstrates working knowledge of mainframe and personal computing systems.
+ Demonstrates highly effective interpersonal, written and verbal communications.
+ Must be able to work in a Labor/Management Partnership Environment.
+ Must be a Registered Dietitian (RD), as confirmed by the Commission on Dietetic Registration.
+ Pre-RDs may be considered if: 1. candidate has completed the Supervised Practice Program (SPP) for RDs, AND 2. not more than six (6) months has passed since completing the SPP, AND 3. candidate has not previously failed the RD registration exam, AND candidate must become a RD within six (6) months of completing the SPP to comply with California Law Current Registered Dietitian (RD).

**Job Details:**
+ Area: Medical OT-8HR Class: RD Shift: 1 Start: 07:30:00 End: 16:00:00
**Responsibilities (may include but is not limited to):**
+ Providing timely documentation of nutritional care in accordance with policy through nutritional assessment/progress notes.
+ Conducts diet counseling to all patients.
+ Assess the dietary and health needs of patients.
+ Counsel patients on food and nutrition.
+ Develop meal plans for clients and gauge the effects of these meal plans.
+ Attends and participates in team meetings, unit/dietary meetings and nutrition meetings.
+ Observes patients at mealtimes.
+ Provide assistance in the management of food production.
+ Attends professional meetings/seminars as appropriate.
+ Other duties as assigned.
**Qualifications:**
+ Ability to build rapport with patients
+ Compassionate and caring demeanor
+ Familiarity with medical terminology
+ A valid and current CPR card (American Heart Association)
+ Must have up to date credentials in the state of **CA**
