**A Brief Overview**

Demonstrates the ability to develop, implement and evaluate teaching plans for patient populations in unit specialty in accordance with applicable standards; Demonstrates the ability to apply knowledge of growth and development across the life span to the care of patients; Provides direct patient care to patients and families in a culturally, developmentally and ethically appropriate manner; Plans of care address the physical, psychosocial, spiritual and learning needs of the patient/family.
  3. Clinical Scholarship: Participates in QI, C PI and risk management activities at the unit, department or organizational level; Demonstrates the ability to effectively perform and improve all processes in order to achieve excellence with regard to Luminis Health's quality standards and benchmarks; Supports the use of evidence based guidelines and organizational policies and procedures to promote safe patient care and a safe practice environment.
  4. Clinical Leadership: Participates in unit shared governance according to departmental standards; Participates in the education and orientation of new staff; Delegates patient care activities as appropriate; evaluates delegated activities for expected patient care outcomes; Employs real time computer documentation when completing patient record.

Educational/Experience Requirements:
  • Graduate of an accredited school of nursing
  • Adherence to credentialing requirements of Luminis Health as stated in the nursing bylaws.
  • Must obtain Advanced Cardiac Life Support (ACLS) within 6 months of date of hire.
  • Current licensure as a registered nurse by the Maryland Board of Nursing.
  • American Heart Association CPR for Healthcare Providers.

**Overview**
We're looking for a **Hospice Nurse Practitioner** to join our team.You will report directly to the Executive Director or Administrator. You will be assigned to a location to perform Face-to-Face (F2F) encounters and be responsible for gathering clinical information to support the hospice diagnosis and prognosis. These findings are then communicated to the hospice physician responsible for certifying or recertifying the patient's hospice eligibility.
This is a less than full-time opportunity providing face to face visits.
The scope of practice for the hospice NP is limited to those tasks necessary to complete hospice F2F encounter visits.
+ Performs and completes physical assessments for the F2F encounter within the regulatory time frames established by CMS
+ Provides clinical findings, including review of the medical treatment plans, diagnostic tests, lab results, and medication lists to the physician who is recertifying the patient for hospice eligibility
+ Makes appropriate recommendations to the IDG based on findings of the F2F assessment
+ Notifies hospice agency when unable to complete a F2F visit according to schedule
**About You**
+ Master's or higher degree in Nursing.
+ Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
+ Current state license as a registered nurse
+ Current state license or certificate as an advanced practice nurse
+ Current national certification as a Nurse Practitioner. (ANCC or AANP)
+ Must have a collaborative agreement in place.
