Qualifications:
- Must be flexible, caring, compassionate, and be a team player
- Minimum of 3-5 years of management or supervisory in the field of medical records, preferably in a long term care setting
- Prior ICD-10 Coding experience of at least 1 year
- Knowledge of PDPM & NTA Point Reimbursement
- Must be organized and have superior communication skills
- Knowledgeable in Microsoft Office
Preferred (But Not Required) Qualifications:
- Prior Skilled Nursing Facility experience under PDPM and MDS completion experience.
- Certification as a Registered Health Information Technician (RHIT), CCA, or CCS
- Active member of the American health Information Management Association (AHIMA).
- Evidence of maintaining continuing education (CE) requirements of the American Health Information Management Association.
Responsibilities:
- Ensuring patients' files are properly maintained and can be retrieved upon request or scheduled appointment
- Performing clerical tasks necessary for the proper maintenance of patients' medical records
- Carry out correct reconciliation of all discharged records and ensure necessary follow up for missing documents, prep/scan/verify/index all patients' records within 24 to 48 hours of discharge
- Coordinate with (NAC) Nurse Assessment Coordinator to Ensure Proper ICD-10 Coding for PDPM & NTA Point Reimbursement (Daily)
- Ensure physician properly signs, dates, and completes patients' records before signing them off as duly completed
- Arrange in proper order records from the skilled nursing facility
- Set up electronic records for storage, & file them accordingly, scan/photocopy records
- Receive and send information through a electronic fax
- Carry out periodic medical record chart audit and review
- Take telephone calls and provide answers to questions regarding patients' medical records.
- Audit Patient Inventory Sheet Completion
- This role will be the department head for HIM & Serve as a department head within the facility's leadership team
- Attend all Department Head Meetings (Usually Weekly)
- Attend Monthly Utilization Review and Quality Assessment and Improvement, as well as any other specific meetings as designated by the center administrators
- Orientation of all new partners involved with documentation process on HIPAA

**What you will bring to the table:**

Project Management & Leadership
- Lead EHR-related projects from inception through execution, including planning, stakeholder engagement, resource coordination, risk mitigation, and post-implementation evaluation.
- Develop and maintain project documentation, including timelines, goals, budgets, progress reports, and KPIs, ensuring transparency and accountability across teams.

- Experience leading cross-functional projects, with a proven ability to manage competing priorities and deliver results on time and within scope.
- Strong knowledge of NextGen EHR system functionality, configuration, and workflow optimization.
