- Provide various forms of care coordination by relaying medication statuses with patient/counselor and following up on completion of releases, paperwork, and status update reports, and/or applications.

- Respond to calls and walk-ins presenting at the clinic in crisis situations.

- Relate to mentally ill or chemically dependent individuals and their families of diverse ethnic, cultural, and socioeconomic backgrounds, as well as have the ability to interact with them in a positive, supportive, and cooperative manner.
- Experience working in a computerized office environment with word processing, database and spreadsheet skills sufficient to prepare correspondence, reports, and forms, including the willingness to learn new software applications.
- Complete records requests (disability requests/attorney requests/patient request, etc). This may vary by clinic.
- Monitors clinical and non-clinical data via internal Electronic Health Record (EHR), internal dashboards and reports, as well as and regional informational health systems.
- Provide support and triage to patients relative to importance of following up with behavioral health and medical providers, as appropriate.
- Help offer support to patients being seen for medication only services
- Support the clinical treatment team including providers and clinicians including follow up and outreach for engagement purposes.

Compensation
Horizon is consistently evaluating our compensation and benefits packages to ensure we remain competitive. The pay range for this position is $9.50 - 21.00. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.
Physical demands associated with this position include:
- Sitting for extended periods, alternating between standing and sitting
- Mobility required with repetitive wrist, hand, and finger movements

Conducts psychosocial assessment of patients and families upon receiving a referral to the Care Collaboration Unit. GENERAL RESPONSIBILTY:

MINIMUM QUALIFICATIONS:
Masters degree from an accredited school of Social Work, Human Services, Counseling, Public Health or related field. Candidate must have a minimum of 1-3 years experience in developing and implementing health promotion, disease prevention programs. Valid NYS drivers license required. Excellent oral and written communication skills, including public speaking. Highly organized, with the ability to develop organizational tools and track data. Excellent computer skills including word processing, email, internet, spreadsheet and database usage.

Job SummaryThe Clinical Care Coordinator (CCC) works in collaboration with the primary care provider and all other members of the care team to support chronic disease care management for high-risk, high-need patients in a manner that is medically appropriate and cost effective. Division/Program Summary: The Bureau of Mental Health's mission is to improve mental health and wellness for people in New York City, and to eliminate racial and other long-standing societal disparities by providing services, resources, and opportunities to providers that are grounded in accurate, data driven information that fosters community participation, thus reducing the stigma around mental health. This includes procuring and monitoring more than 500 contracted programs that provide mental health treatment, psychiatric rehabilitation, supportive housing, care coordination and advocacy services and operating court-mandated Assisted Outpatient Treatment (AOT), Single Point of Access (SPOA) for mobile treatment and care coordination and NYC Supportive Treatment and Recovery Team (NYC START) for people with first episode psychosis. The NYC Supported Transition and Recovery Team (NYC START) is a specialized program providing services to shorten the duration of untreated psychosis and improve linkage to care and community supports for New Yorkers experiencing a first episode of psychosis. The program collaborates with hospital treatment teams and discharge planners for optimal after care plans, assist with linkage to out-patient services and community supports, provides care coordination services in the community for three months following a hospital discharge, and works with community treatment providers, families, and other supports to promote successful community engagement.

Reporting directly to the Nurse Manager, the Clinical Coordinator III (Registered Nurse III) assists physicians with patient care, health education/promotion, electronic medical record (EMR) chart management, coordination of patient care, and clinical protocols as needed.
This experienced Clinical Coordinator III (Registered Nurse III) duties will include, but are not limited to, medication/immunization administration, point of care testing (POCT), clinical information abstraction, and patient education that can be in person, via our patient portal, or by phone. The Clinical Coordinator III (Registered Nurse III) will work closely with multiple providers and other clinical and administrative staff to deliver an excellent patient experience that contributes to positive health outcomes for patients and a work culture that represents our values. This nursing position will serve as a key member of the patient care team.
Administrative/Operations
- Assesses and collects data related to patient health status and inputs this medical information into the EMR as instructed by the physician.
- Works with the care team to develop the plan and care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools) which includes assisting with medication management (e.g. medication renewals) inclusive of medical prior authorizations as needed. Administer immunizations and medications.
- Executes nursing interventions by collaborating with the care team and following standards of care established by the unit/department.

- Supports on-going clinical functions of the service to provide compassionate patient-centered care.

Responsibilities
- Assesses and collects data related to patient health status and inputs this medical information into the EMR as instructed by the physician.
- Works with the care team to develop the plan and care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools) which includes assisting with medication management (e.g. medication renewals) inclusive of medical prior authorizations as needed. Administer immunizations and medications.
- Executes nursing interventions by collaborating with the care team and following standards of care established by the unit/department.

- Supports on-going clinical functions of the service to provide compassionate patient-centered care.
- Handle telephone, My Chart and in-person triage (for symptom management) as well as patient education
