Responsibilities:

GENERAL PURPOSE:

The Mental Health Therapist assigned to the Assertive Community team is responsible for providing psychosocial assessment and treatment to individuals ages 18 and older who are severely and persistently mentally ill. This is provided through the Mental Health Therapist's participation on an ACT outreach team designed to provide comprehensive community based psychiatric care. The Mental Health Therapist functions a general therapist for assigned patients and provides direct clinical service including individual and family therapy, group therapy, crisis intervention, and community

outreach.

ESSENTIAL FUNCTIONS

  • Provides direct clinical services to assigned patients, which may involve individual, family, and/or group therapy, crisis intervention, and diagnostic evaluations.

  • Participates as a member of an interdisciplinary team involving consultation, treatment planning, and periodic care review.

  • Collaborates with family, social supports, community agencies (past and present) to ensure appropriate and effective care. Coordinates referrals and facilitates linkage to other appropriate agencies/supports.

  • Documents interventions, both direct and indirect, in the patient's medical record in compliance with the Division, Department, and Hospital policy.

  • Assists clients in maintaining themselves in the community, including setting up financial budgets, applying for entitlements, learning skills of daily living, and other case management tasks as indicated.

  • Transports clients as needed to appointments and to meet treatment plan goals.

  • Ensures that Hospital and Department and policies and procedures are implemented with regard to professional practice, patient care, and disposition training.

  • Subscribes to the Department's goal of creating a culturally competent environment by treating patients/families, trainees, research subjects and co-workers in a sensitive and caring manner with appropriate attention to cultural differences. Participates in available cultural competence events and trainings.

  • Participates in a client on-call phone rotation with other ACT team members approximately every 3-10 weeks, as needed and according to staffing.

QUALIFICATIONS

Required:

  • Master's degree from a Counsel for Accreditation of Counseling and Related Education Programs (CACREP) accredited program with major course work in mental health counseling required

  • Working with the SPMI (severe and persistently mentally ill) patient population required, preferably in a community-based setting.

  • Or equivalent combination of education and experience

  • NYS licensure as a mental health practitioner (e.g., Mental Health Counselor, Creative Arts Therapist) or limited permit eligible upon hire

  • If licensure has not been obtained upon employment, the individual has 36 months to obtain the NYS license

**Incoming Phone Calls:**

+ Answers incoming telephone calls. Responding to patients' questions, with the ability to assess the patient/support person needs and triage to the appropriate License Social Work staff.
+ Answers phone calls in a timely and courteous manner with an understanding of the social crisis and the distress this can cause for the patients and their supports, as well as an understanding of how to interact respectfully and calmly.
+ Manages incoming social work program calls, assessing and triages to License Social Work staff.
+ Provides information to patients to minimize the need to redirect the telephone call, forwards calls, pages providers, and takes messages.
+ Through verbal assessment and in-person observation conducts assessment of the individual circumstances, to ensure any risks are identified and escalated to licensed social work staff appropriately.
**Incoming service requests:**
+ Manages the Social Work Outlook account for the distribution of patient/support person to the appropriate staff/community resources.
**Service Recovery:**
+ As part of clinical team, assures client satisfaction with information processing and direct point of contact service.
+ Assesses the urgency of a situation and determines the appropriate triage of the patient/family, serves as a focal point for handling complaints, utilizes service recovery concepts, serves as front-line problem solver.
+ Involves Clinical staff, Admin staff or Program Director when appropriate/necessary.

**Care Collaboration:**

**Case Management:**

+ Manages and assesses the need for bus pass distribution for patients needing transportation to and from on-site clinical appointments and hospital discharges.
+ Identifies and coordinates client special needs such as assessing need for other spoken language assistance or ASL Interpreters.
+ Managing incoming referrals for hotel and lodging needs for medical/surgical patients and their support.
**Process Development:**
+ With direct oversight from leadership, carries out actionable items for quality improvement and process development.
+ Recommends process improvements.
+ Maintains the training manual of social work and care management services processes and provides direct training to staff to ensure a standard of care.
**Other:**
+ Continuously provides a clinical presence at the front desks.
+ Subscribes to the Departmental goal of creating a culturally competent environment by treating patient, support person, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competence events and training appropriate to job duties.

Salary: $58,000-$72,000/YR

Responsibilities Include::
  • Providing individual, group, and family therapy

  • Serving as liaison with families and outpatient providers

  • Interact with insurance companies for pre-certification and utilization management

  • Participating in discharge and aftercare planning

  • Therapeutic meal support, while modeling a healthy relationship with food

  • Provides safe, empathetic, and affirming care for each client and their unique circumstance based on the clients' intersecting identities

Qualifications::
  • Master’s degree in clinical counseling or related discipline, at minimum

  • State license (or license-eligible) in a counseling field (e.g., Psychologist, LMFT, LCSW, LPC)

  • Prior experience with eating disorders and higher levels of care is helpful

  • Knowledge of diversity, equity and inclusion practices
