FlexStaff is seeking a Nurse Practitioner Supervisor (NPS) for our client, a non-profit healthcare organization providing home and community-based healthcare and services for the elderly.
PURPOSE: The NPS collaborates with community primary care physicians (CPCPs) to oversee and coordinate participant care within the Interdisciplinary Team (IDT). This role focuses on participant independence and wellness, acting as a liaison between the participant, CPCP, and plan staff to develop and implement care plans.
RESPONSIBILITIES:
+ Supervise and manage Nurse Practitioners and other clinical staff at assigned PACE site.
+ Conduct performance appraisals.
+ Represent/serve as Medical Director in their absence or as directed.
+ Perform annual wellness assessments (more frequently for high-risk participants).
+ Provide urgent care and same-day access.
+ Manage participants with acute changes in condition.
+ Assess participants returning from hospital/ED within 5 days, reconcile medications, and coordinate follow-up.
+ Review medical history every 6 months, formulate diagnoses, and ensure accurate documentation.
+ Participate in clinical documentation improvement activities and provide feedback to staff.
+ Review symptoms, identify active diagnoses, and inform PCPs of potential care gaps.
+ Co-manage high-risk participants with PCPs and develop care plans to address gaps.
+ Attend IDT meetings and represent CPCP as needed.
+ Communicate with IDT and other plan designees.
+ Educate participants, families, and staff on disease processes, medication adherence, and self-management.
+ Comply with HIPAA regulations.
+ Conduct home visits as needed.
+ Supervise care plan formulation and delivery for community PCP participants.
+ Oversee home care services arranged/provided by CHN.
+ Provide emergency coverage for PACE center PCPs.
+ Participate in on-call rotation.
+ Adhere to scope of practice and meet established competencies.
+ Act as an IDT member.
+ Perform other duties as assigned.
QUALIFICATIONS:
+ Graduate of NYSED-acceptable nurse practitioner program or nationally certified.
+ Minimum 2 years of clinical nursing experience in home care, care management, or related field.
+ Minimum 1 year of experience with frail/elderly populations (or training provided).
+ Excellent communication, interpersonal, analytical, and research skills.
+ Project management skills and ability to meet deadlines.
+ Proficient in Microsoft Office.
+ Active NY Nurse Practitioner license.
+ DEA certification.
+ AANP or ANCC Board Certification.
+ Legally authorized to practice in NY.
+ Medically cleared for communicable diseases and up-to-date immunizations.

Summary

This position will be located at the East Hanover, NJ site and will not have the ability to be located remotely. This position will require 20% travel as defined by the business (domestic and/ or international).

The Director, NPS Lifecycle Management and Process Transformation is responsible for life cy-cle planning for in-line patient support programs and will develop deep forward-looking planning and right-sizing of our programs after launch to LoE. This individual will need to collaborate with the NPS Competitive Intelligence team to understand the external competitive environment and our internal product/portfolio strategies. About the Role

Key Responsibilities:

* Lead the strategic planning for lifecycle management across the Therapeutic Area portfolio.
* Develop and implement focused strategies for brand performance by evaluating insights, category trends, in-market performance, and competitive activity.
* Collaborate with cross-functional teams to develop an LCM framework for key products through active involvement in Governance Framework such as CCRB, Quantum, and PSOC.

The pay range for this position at commencement of employment is expected to be between $185,500 and $344,500 per year; however, while salary ranges are effective from 1/1/25 through 12/31/25, fluctuations in the job market may necessitate adjustments to pay ranges during this period. Purpose:

Stewards Individual Placements is an AmeriCorps Program which provides individuals with service and career opportunities to strengthen communities and protect our natural and cultural resources. Guilford Courthouse National Military Park protects 250 acres of the approximately 1,000 acres of the actual battlefield. Established in 1917, it is the country's first federally protected Revolutionary War battlefield. The purpose of Guilford Courthouse National Military Park is to protect for historical and professional military study as well as the benefit, education, and inspiration of the public, the battlefield and the accounts of the Battle of Guilford Courthouse during the Southern Campaign of the American Revolution.

The Community and Youth Engagement Ambassador is an AmeriCorps Position which will serve as a member of the park's interpretation and education team. The AmeriCorps member will assist park staff in providing a range of services designed to foster meaningful connections to Guilford Courthouse National Military Park and its rich history. During the course of the internship, members will serve alongside experienced National Park Service employees supporting every facet of park operations such as leading informal interpretive activities in the park, delivering existing education programs to youth, and developing new programs for various audiences. The service member will also assist park staff with a large virtual volunteer program, supporting a cohort of over 600 citizen archivists, who transcribe the pension applications of veterans of the battle.

Description of Duties:

As an AmeriCorps Member serving with the Guilford Courthouse National Military Park in Greensboro, NC, your responsibilities will include:

• Deliver existing education programs in schools and in the park.
• Join the Citizen Archivist Coordination team to manage a large program of virtual volunteers.
• Develop a new education program that meets state curriculum standards and fits with the park's culture of hands-on, fun learning experiences.
• Offer informal interpretive activities for guests.
• Assist with the management of a virtual volunteer project, responding to emailed questions, tracking volunteer assignments, and hosting orientation calls.
• Deliver existing interpretive programs and develop new ones to be used in the park and/or offsite for outreach events.

Time Requirements:
