Job Description

Under the direction of a Nurse Manager, the PACE Community Health Nurse (PCHN) provides skilled care and routine clinical nursing assessment visits to PACE members and families. The PACE CHN utilizes all elements of the nursing process and ensures compliance with applicable laws, regulations, and agency policies. As a core interdisciplinary team (IDT) member, the PACE CHN is assigned to a PACE member in compliance with CMS PACE regulatory requirements.

Responsibilities

• Carry out the agency’s mission, philosophy, goals, and objectives within agency policy guidelines.

• Interpret and implement the agency’s philosophy to staff and community members.

• Assume responsibility for assignments, seek supervision appropriately, and perform within the limits of education and experience.

• Know and conform to laws governing the practice of professional nursing, providing care using all elements of the nursing process.

• Participate in mandatory PACE comprehensive care planning meetings as required by PACE regulations.

• Collaborate with all IDT members in developing a comprehensive care plan based on member-specific needs, physician orders, UAS-NY CAPS, other IDT assessments, and identified goals.

Work Environment

Community Health Centers, including locations in Bronx and Bushwick. Schedule: Monday – Friday 8:30 am-5:00 pm (37.5 hours), with potential weekend work as required. The work involves traveling and walking between sites and locations throughout the day.

Pay and Benefits

The pay range for this position is $50.00 - $55.00/hr.
