Our Chicago clinic has been providing veterinary rehabilitation to pets in Chicagoland and training veterinary professionals from all over the world for over 16 years. We are focused entirely on veterinary rehabilitation and work closely with general and specialty practices throughout Chicagoland to give pets the best quality of life through improved mobility and pain management. Our team currently includes 2 Veterinarians certified in canine rehabilitation, acupuncture, and spinal manipulation therapy; 7 Canine Rehabilitation Therapists certified (or pursuing certification) in canine rehabilitation and canine massage; 5 Therapy Assistants, 3 Patient Services Coordinators; and a Practice Manager. Our 6000 square foot facility holds 2 Underwater Treadmills, 5 therapy lasers, a large therapy gym with custom exercise equipment, a Piezowave shockwave, therapeutic ultrasound, and more.

CTS Health Services is a community mental health services center that specializes in a holistic approach to health and wellness for children, parents, and adults of varying circumstances and mental challenges. We provide one-on-one behavioral interventions for our clients and their support circles to help our clients navigate through life within their community at their highest level.

The Psychosocial Rehabilitation Facilitator will facilitate regular PSR Services to ensure participants are provided with active treatment that will improve or maintain the individual's condition, improve functional level, and prevent institutionalization by teaching emotional, cognitive, and social skills. The These programs are aimed to help individuals live as independently as possible and become fully integrated into the communities in which they live. The Psychosocial Rehabilitation Facilitator will be consistently scheduled onsite for the provision of PSR Services per Rule 132.

Requirements:

Educational requirements to be qualified as a Psychosocial Rehabilitation Facilitator:

• Master's Degree in a human service field (Social Work, Counseling, Human Services, Psychology, etc.),
• Behavioral Screening Certification (IM+CANS), (CTS will supply resource to gain certification if not in possession upon hire)
• Bilingual (Spanish or Polish) (preferred)

Responsibilities of the Psychosocial Rehabilitation Facilitator:

• Maintain the minimum hourly billable caseload of 25 per week assigned.
• Facilitate PSR services and its curriculums.
• Work with PSR Director to update monthly calendars pertaining to PSR services.
• Guide clients, individually and in group sessions, to assist in overcoming dependencies, adjusting to life, and making changesby providing support in the development of skills and strategies for dealing with their problems.
• Prepare and maintain all required treatment records and reports.
• Collect information about clients through interviews, observation, and tests.
• Act as client advocates to coordinate required services or to resolve emergency problems in crisis situations.
• Implement program activities based on clinical experience and knowledge, utilizing co-occurring disorder treatment interventions, CBT, problem-solving and social skills, and update as required.
• Collaborate with U/M and staff members to ensure PSR service eligibility on IM+CANS.
• Assess clients' physical or mental condition based on review of client information.

Performs department-based reception and clerical functions.

Responsibilities:

• May act as a receptionist for the department, greeting staff and customers in a polite manner, and referring to appropriate staff when needed.
• Answers department telephone calls in a polite manner, communicating messages appropriately and clearly to all staff members.
• Is customer oriented and able to communicate and work effectively with all levels of staff and department customers. 
• Successfully takes on special projects as needed.

• Completes end of day processing per EPIC policy. 
• Follows Rehabilitation Services and/or CDH Policy and Procedure/Guidelines for scheduling new patient evaluations.
• Accurately identifies patient MRN to avoid duplication of records in system.
• Obtains and updates patient demographic, insurance and billing information.
• May independently access the Unscheduled Orders Work Queues on a daily basis (during normal business hours) and maintains departmental expectations (all new orders will receive initial phone call within 24 -48 hours).
• May independently monitor and maintain other assigned Rehabilitation Work Queues based upon registration. 
• Follows Rehabilitation Services Guidelines and policies in regard to subsequent patient scheduling in order to maximize therapist productivity and time. (Effective use of wait list, scheduling of new evaluations, independently adding paperwork time in instances where a therapist is required to see a high number of new evaluations or has a backlog of documentation).
• May collect cash and credit card payments and ensures all payment collection and documentation meets current standards.
• Follows the necessary requirements for daily reconciliation and balancing.
• May independently manage therapist's monthly schedules/templates.
• Schedules meetings and huddles as requested by therapist or Senior staff member.
• Works with Senior to block scheduled PTO time and/or sick time appropriately.
• May independently make "new evaluation confirmation phone calls" to all new evaluations the day before the scheduled appointment and create "shadow charts" administering all associated paperwork for these patients. 
• Completes order entry and evaluating orders.
• Determines when it is appropriate to consult with technician for interpretative purposes.
• Assists patients to obtain orders not in the EPIC system. 
• Understanding of scanning: possesses thorough understanding of the scanning process. This understanding includes the user's ability to recognize registration and clinical documents and perform accurate indexing within the electronic record. 
• Independently manages Rehabilitation Work Queue 3111 to monitor and maintain "visits not authorized" to ensure proper authorization is received in a timely manner.
• May assist in clinic set up/clean up including hot pack/cold pack machine cleaning and temp logs, eye wash log.
• Assists Manager/Senior Staff in monitoring charges via accessible Work Queues in EPIC. Independently makes corrections to pending Work Queue items with or without guidance from supervisor.
• Manages faxing documentation to insurance companies and/or physician offices for authorization and signatures for Plan of Care as appropriate.
• Works with Coding, Patient Accounts and Financial Clearance Center to ensure all patient accounts are registered accurately for timely billing and management of patient accounts.

Qualifications

Required:

• Two years’ experience in front office with computer experience
• High School graduate
• Successful completion of EPIC training for scheduling and registration
