The Clinical Services Manager is responsible for overseeing the Older Adult Behavioral Health Program, Program to Encourage Active and Rewarding Lives (PEARLS), Caregiver Resource Center, Adult Protective Services team and provide short term emergency counseling to older adults living in Oak Park and River Forest. The Clinical Services Manager will provide clinical supervision and direct support to the teams for individual or group counseling, on-site crisis intervention, short-term (emergency) case management, and interface with other providers as warranted by client needs. The Clinical Services Manager will also coordinate presentations for staff and the community on topics of mental health, mental health screening events, and other outreach and presentations, as needed.
Essential Job Functions
- Oversee Older Adult Behavioral Health, PEARLS, Caregiver Resource Center and any additional, applicable programming. Ensures staff productivity and clinical direction of treatment.
- Oversee Adult Protective Services team and offer support on challenging abuse, neglect, exploitation, hoarding, and self-neglect cases where mental health concerns are involved.
- Understand, provide oversight, and carry out work for the programs in accordance with funding guidelines and Oak Park Township policies.
- Provide opportunities for learning, growth and motivation for the clinical services team.
- Hold a small caseload of the most complex cases and provide backup support to the team during absences or staffing shortages.
- 6. Support team in managing participant needs in an effective manner by identifying and addressing gaps in service while utilizing best clinical practices.
- Serves as clinical programs expert for the leadership team.

Minimum Qualifications
Education and Experience:

- Bi-lingual - Spanish preferred
- Clinical supervision provided to team members, others.

Knowledge of:

What You'll Do As a clinical leader and transitional care management subject matter expert, the Medical Director, Central Clinical Services will provide clinical oversight and strategic direction for Strive's transitional care management (TCM), intensive care management (ICM), and utilization management (UM) functions. This role will develop policies consistent with care management best practices and NCQA guidelines while driving Strive Health's mission of delivering exceptional kidney care. This role reports to the Chief Clinical Officer. The Day to Day Leads design, implementation, and ongoing effectiveness of TCM, ICM, and UM clinical programs in partnership with operations SVP of central clinical services and VP of care management in a dyad relationship. Partners with Strive Regional Medical Directors and Specialty Medical Directors to ensure seamless transition of patients between central clinical to market clinical teams. Leads clinically focused rounds, reviews complex clinical cases, facilitates problem-solving, and advocates for patient-level issues while providing daily feedback to individual clinicians on appropriate clinical management. Acts as direct manager for Strive TCM NPs and serves on central corporate clinical committees, such as quality, compliance, or the Clinical Steering Committee as needed. Develops educational content on care management best practices for patients transitioning from acute care settings and participates in onboarding of new hires. What You'll Do As a clinical leader and transitional care management subject matter expert, the Medical Director, Central Clinical Services will provide clinical oversight and strategic direction for Strive's transitional care management (TCM), intensive care management (ICM), and utilization management (UM) functions. The Medical Director will ensure high-quality, cost-effective care delivery aligned with value-based care principles to patients in their most vulnerable states across the entire disease spectrum from CKD to ESRD, with the goal of optimizing outcomes and patient experience. This role will develop policies consistent with care management best practices and NCQA guidelines while driving Strive Health's mission of delivering exceptional kidney care. This role reports to the Chief Clinical Officer. The Day to Day Leads design, implementation, and ongoing effectiveness of TCM, ICM, and UM clinical programs in partnership with operations SVP of central clinical services and VP of care management in a dyad relationship. Partners with Strive Regional Medical Directors and Specialty Medical Directors to ensure seamless transition of patients between central clinical to market clinical teams. Leads clinically focused rounds, reviews complex clinical cases, facilitates problem-solving, and advocates for patient-level issues while providing daily feedback to individual clinicians on appropriate clinical management. Acts as direct manager for Strive TCM NPs and serves on central corporate clinical committees, such as quality, compliance, or the Clinical Steering Committee as needed. Develops educational content on care management best practices for patients transitioning from acute care settings and participates in onboarding of new hires. Engages in various stages of the business development lifecycle, from initial pitches to creation of clinical workflows during implementation phase. Provides consultation to product, tech, and operations teams as a subject matter expert with strong acumen for translating clinical program effectiveness to quantifiable clinical and financial ROI, including impact on total cost of care. Minimum Qualifications 6+ years experience working in cardiology and kidney care management, including experience with CHF, CKD, and ESKD populations. Current state-specific licensure required Current Drug Enforcement Administration (DEA) license required, or eligible to obtain within 90 days of hire. Current BLS or CPR Certification required. Board Certified Nephrologist or Internal Medicine provider. Efficient and reliable transportation, including an active driver's license, allowing for the ability to travel across an assigned region to meet patient needs. Locations may include offices, clinics, and patient homes. Provides in-person patient care which may include standing, sitting, walking, pushing, pulling, and lifting.
