Maximus is seeking a dedicated Clinical Services Manager to support the Colbert Consent Decree Illinois Outreach and Assessment team. In this role, you will act as a liaison to serve as a single point of contact for all transition entities. This would include communicating with Illinois state agencies, MCO's (managed care organizations), court appointed staff, and transitional prime agencies.

**This position requires up to 15% travel annually to Illinois.**

About the Program:

We have contracted with the State of Illinois Department of Healthcare and Family Services (HFS) to manage comprehensive assessment processes for various screening and assessment services, specifically the Williams and Colbert Consent Decrees Outreach and Transitional Assessment. Outreach includes in-person visits with Colbert Class Members in Cook County Nursing facilities and Williams Class Members in SMHRF facilities which provide education on their rights and evaluate their level of interest in potential transition to the community.

Transitional Assessments are conducted in-person with Colbert Class Members in Cook County Nursing facilities and Williams Class Members in SMHRF facilities interested in transitioning to the community. Evaluators assess individuals' functional needs in several areas to determine the level of support necessary for successful community transition.

Minimum Requirements

Required Qualifications:

Qualifications

We require:
  • High school diploma or GED required.

+ Assists in preparation of the unit, patient rooms, and beds for receiving patient admission, returning surgical patients, transfers, emergencies, or discharges. Provides hygiene care, which includes providing or assisting in bed baths, oral hygiene, hair care and shampoo, back care and rubs, perineal care, dressing patient, and changing bed linen.
+ Prepares patients for meals, serves and removes food trays, and assists with meal or feeds patients. Distributes water and nourishment to patients. Establishes and maintains communicates with the health care team. Performs data collection and documentation of TPR, BP, wt, ht, I/O, activity, diet, and hygiene in an ongoing, timely, accurate, and concise manner.
+ Demonstrates the use of good body mechanics and provides for patient safety when transferring, ambulating, turning, lifting, and positioning patients. Acts appropriately in emergency situations. Performs routine treatments and patient care duties listed on the skills inventory form and under the direct supervision of the RN.
+ Identifies and reports changes in the patients condition to the RN within an appropriate time frame. Maintains verbal and written information in a confidential manner. Assists in straightening patient rooms, cleaning overbed tables and bedside stands, keeping patient care areas clean at end of each shift. Collects and transports specimens for delivery to appropriate departments per hospital standards.
+ Completes work order for equipment that is out of service and leaves copy in the appropriate designated place/unit for follow up. Adheres to Infection Control Policy including but not limited to: a) demonstrates body substance precautions by wearing gloves and other protective attire to avoid body substance contact when appropriate; b) adheres to the KFHPW hand washing policy; c) complies with the proper handling of waste; d) maintains a clean work area. Provides postmortem care under the direction of the RN. Demonstrates initiative and responsibility for improving skills and expanding knowledge through continuing education or on the job learning experience. Consistently demonstrates an appearance consistent with current KFHPW dress code policy.

**Qualifications**
We require:
+ High school diploma or GED required.

Job Summary:


An unlicensed caregiver who maintains a safe and clean environment for patients and Health Care team members. Transport patients with proper technique. Transport equipment, supplies and specimens. Provide environmental sanitation as required.



Essential Responsibilities:



  • Environmental support: Transports specimens to appropriate departments per hospital standards. Cleans all flat surfaces and floors in each operating room between surgical procedures. Cleans all used surgical litters, stretchers, and redresses them for use. Available for spot cleaning or assistance with spills of blood, body fluids, or MSDS listed fluids. Provides environmental sanitation as required by charge nurse. Provides as needed environmental sanitation for common areas (e.g., bathrooms, break room).

  • Daily Surgical Service: Prepares the operating rooms prior to the first surgical procedure of the day. Trnasports surgical patients to the operating room. Demonstrates the use of good body mechanics and provides for patient safety when transferring, turning lifting, or assisting in positioning patients. Performs other duties as assigned by operating room staff. Provides surgical skin preparation using proper equipment, techniques, using standardized guidelines, or physicians orders as guidance. Communicates with patients in a courteous and helpful manner. Establishes and maintains communication with the health care team.

  • Adheres to all infection control policies taking all appropriate precautions to provide a safe environment for patients and all health care team members. Provides direct assistance with the activities required to begin subsequent surgical procedures. a) Return unneeded surgical equipment to its appropriate storage area. b) Obtain required equipment needed for upcoming surgeries. c) Assist with opening sterile surgical supplies. Understands the basic surgical aseptic technique. Transports patients to the operating suite for surgical procedures and to the unit after recovery.

  • Ancillary support: Responsible for daily testing and/or recording of results for a variety of mandated requirements (e.g. cidex sticks, refrigerator charts). Responsible for monthly inventory and control of expired surgical supplies. Restocks all common areas maintaining order, cleanliness, and stock levels. Rotates any expiration-dated supplies restocked. Provides transport of supplies and equipment to and from other departments with the hospital.

  • Ongoing development: Demonstrates initiative and responsibility for improving skills and expanding knowledge through continuing education or on the job learning experience.
