Summary:
The Community Health Worker (CHW) provides culturally appropriate support and information for the patient and their families in a series of structured visits and follow-up phone calls in a variety of settings, including the patient's home. Knowledge of resources in the community and refers patients to them. Assists patients in their understanding of how to use healthcare and social services and manage their Patient Health Record (PHR). Has a holistic and interdisciplinary approach to care management and health promotion. Builds relationships with the patient, their families, healthcare providers and the community. Assist patients in finding and understanding how to use a Medical Home (patient's primary choice of physician or clinic), pharmacy assistance programs, dental services, diagnostic services, transportation, behavioral health services and other services as needed. Refer patients to healthcare and social services and follows-up on all referrals. Follow care pathways (evidence-based steps) designed and selected to deal with patients' specific health and social concerns and problems. Document activities through appropriate record keeping that follows the organization's policies and procedures. Maintains patient registry. Consult with administrative supervisor and clinical supervisor to coordinate activities and schedule and to address identified health care needs of the patient. Educate patients and their families in healthy lifestyles and promote health behavior change. Encourage patient to set goals, identify barriers and challenges and monitor their own conditions.

Qualifications
In order to qualify, you must meet the specialized experience requirements described in the Office of Personnel Management (OPM) Qualification Standards for General Schedule Positions, Nursing Series 0610 + IOR (Individual Occupational Requirement).
BASIC REQUIREMENT OR INDIVIDUAL OCCUPATIONAL REQUIREMENT:

Education
A graduate or higher-level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Note: You must submit a copy of your transcripts.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Note: You must submit a copy of your transcripts.
Licensure
For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:
SPECIALIZED EXPERIENCE: Applicants must have at least one (1) year of specialized experience at the next lower grade GS-11, or equivalent in other pay systems. Utilizing written communication sufficient to develop research program documents and evaluations, and procedures/policies based on results of special studies and research; and in oral communication sufficient to counsel family members, take medical histories, provide health teaching and crisis intervention. Developing comprehensive nursing care plans, evaluating total health care needs, evaluating intervention outcomes, coordinating health care given, adjusting regimens when needed, and recognizing physical and mental deviations from what is considered normal.
FEDERAL TIME-IN-GRADE (TIG) REQUIREMENT FOR GENERAL SCHEDULE (GS) POSITIONS: Merit promotion applicants must meet applicable time-in-grade requirements to be considered eligible. One year at the GS-11 level is required to meet the time-in-grade requirements for the GS-12 level. TIG applies if you are in a current GS position or held a GS position within the previous 52 weeks. NOTE: Applicants applying as VEOA candidates who are current GS civil service employees or are prior GS civil service employees within the past 52 weeks must also meet time-in-grade requirements.

Duties & Responsibilities
Purpose of Position :
Provides community-based professional nursing care to patients in group and private homes, as well as those without housing across New York City. Diagnoses and treats responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling and the provision of care supportive to and restorative of life and well-being.
Examples of Typical Tasks :
1. Provides services requiring substantial and specialized nursing skill, in accordance with the plan of treatment signed by the physician and makes initial evaluation visits to clients with medical illnesses, chronic diseases, behavioral health disorders, etc.
2. Identifies and distinguishes between physical and psycho-social signs and symptoms. Selects and performs
those therapeutic measures which are essential for effective execution of the nursing regimen for assigned
patients/clients.
3. Develops, implements and continuously evaluates the nursing regimen for assigned patients/clients. May perform assessment of behavioral health needs, wound care, catheter insertion, medication teaching, diabetic care, patient/family education and counseling and aseptic technique.
4. Completes comprehensive patient assessments and develops, prepares and maintains individualized client
care progress records, including required Outcome and Assessment Information Set (OASIS) documentation.
5. Records and maintains nursing care plans and progress notes on patients/clients to ensure continuity of care and initiates, develops and implements revisions as needed.
6. Interprets and reports responses of patients/clients to appropriate members of the health team and makes referrals to other disciplines in collaboration with physician, client and family, as appropriate.
7. Coordinates the total plan of care with other health team members to plan a comprehensive patient care program and initiates client care conferences, when needed.
8. Maintains continuity of client care by liaising with other health professionals assigned to the client including reporting signs, symptoms and changes to the patient's condition to appropriate parties.
9. Determines and assigns those components of the nursing regimen which may be carried out by allied nursing personnel, including home health aides and communicates plan of care changes to aides and schedulers, as needed.
10. Provides teaching, guidance and direction to allied nursing personnel in rendering patient care. May assist with orientation of new personnel.
11. Participates in departmental and interdisciplinary conferences pertaining to policies and procedures affecting nursing practice.
12. Participates in development and maintenance of a system of evaluation of nursing care, practices and procedures, including evaluation of nursing personnel.
13. Interprets philosophy and objectives of agency and Nursing Department to patients/clients, families and other groups.
14. Maintains professional competence through participation in continuing education and other appropriate learning experiences, including agency provided in-service programs.
15. Participates in internal/external quality assurance/performance improvement (QA/PI) activities, programs, and training, as required.
16. Performs other professional nursing activities and related duties, as delegated by nursing service administration, including clinical record reviews.
Minimum Qualifications
1. A valid New York State license and current registration to practice as a Registered Professional Nurse in
New York State; and
2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS)
through the American Heart Association (AHA); and
3. One (1) year of experience as a registered nurse; and
4. Ability to travel regularly to patient/client assignments across New York City via public transportation and/or
private vehicle; and
5. Demonstrated commitment to continued professional development.
Department Preferences
