POSITION SUMMARY: The Community Health Nurse plays a crucial role in enhancing healthcare delivery by reviewing and implementing clinical workflows within physician practices to achieve PCMH level 3 recognition. You will identify a population of individuals with chronic diseases who require tailored healthcare interventions based on a comprehensive stratification process, using data from visits, claims, lab results, and admissions. Collaborating closely with clinical support teams, nursing staff, and physicians, you will help drive the system’s goals of improving health outcomes for vulnerable patients.

EDUCATION/CERTIFICATION: Graduate of an accredited nursing program; an Associate's degree in an applicable field is required, while a Bachelor's degree is preferred. A current license as a Licensed Practical Nurse is required.

EXPERIENCE: Minimum of five (5) years of nursing experience in a community health setting, specifically working with vulnerable populations.

COMPETENCIES: Strong analytical, organizational, and interpersonal skills are essential. You should communicate effectively in English, both verbally and in writing, and possess a solid working knowledge of Microsoft Office products. The ability to multitask and operate independently is essential.

ESSENTIAL DUTIES and RESPONSIBILITIES: (This is not an exhaustive list of all responsibilities.)

• Collaborate with physicians and staff to identify patients meeting PCMH standards.
• Act as a resource for referral departments needing clinical support for authorization calls.
• Support practices lacking LPN staff to ensure patient care processes are executed effectively.

• Review and implement processes necessary for PCMH compliance across practices.
• Engage in ongoing support to maintain adherence to clinical standards.

This position is an on-call position to provide coverage in the APIA Region periodically. Work 7.5 hours per day, plus take call, no benefits.RequirementsHigh school graduate or equivalent, Certified as a Community Health Aide II. Interested in providing health care to the "people of the village." Will be a resident of the village or interested in relocating to the village. Must be 18 years of age or older, unless there is evidence of extraordinary maturity, with a caring attitude and other good humane qualities. As much as possible and practicable, must meet the following education requirements: Eighth grade education. Basic arithmetic skills, including decimals. Sixth grade English reading level as a minimum. Ability to communicate in English and in the local Native language prevalent in the village. • Detects health problems through inspection, observations, interviews and analysis of records.
