What You'll Do:

- Facilitate Successful Matches: Monitor the relationships between individuals and their home providers or direct care workers, ensuring a positive and supportive match.
- Develop Personalized Support Plans: Collaborate with the service planning team to create Individual Support Plans that reflect each person's unique wishes and needs, while adhering to policy and regulatory requirements.
- Oversee Contracted Services: Oversee service delivery and contractual compliance and complete visits with the individual and home provider in the home environment or service environment.
- Provide On-Call Support: Be available to provide assistance to Mosaic at Home Providers and Direct Care Workers as needed, ensuring consistent support and problem resolution.

Essential Duties/Responsibilities
Assists in ensuring program quality of Imagine! Residential Wrap-around Services.
- Plans, coordinates, and provides program activities, including scheduling of staff, vehicles, and transportation, ensuring the efficiency and quality of programming.
- Maintains a caseload of clients, including a schedule of billable activities and supports.
- Leads continuous improvement initiatives aimed at enhancing service delivery, efficiency, and client satisfaction.
- Maintains regular contact with team members, individuals, and families to provide updates, address concerns, and support service delivery. This includes, but is not limited to, monthly check-ins and quarterly meetings.
- Coordinates wrap-around services to ensure individuals meet personalized goals, ensuring alignment with Imagine!'s person-centered practices.
- Meets regularly with service leadership and attends staff meetings to stay informed of individual progress and provide individual specific training.
- Ensures health and safety requirements set forth by Imagine! guidelines as well as all regulating agencies.

- Maintains and cares for Imagine! property, including supplies, vehicles, etc., being mindful of safety and cleanliness.

Job Qualifications
Knowledge, Skill, and Ability:
- Demonstrated skill in working with people with developmental disabilities, preferably in a community-based setting.

Summary:
This position is specifically for a Palliative Care Social Worker with the University of Colorado Hospital Palliative Care Service. Positions may include working in the inpatient, emergency department, and clinic settings. Assists in facilitating values-based goals of care discussions, advance care planning conversations, and transitions of care for palliative care service patients. Assesses patients and/or patient's family, caregivers, and/or legal representatives and arranges for needed interventions. Plans palliative care-focused interventions to help patients cope with physical, social, emotional, economic, and environmental concerns and provides short term treatment planning for grief support.
Responsibilities:
- Provides specialty-level palliative care psychosocial support to palliative care service patients through direct counseling and collaborative visits, with cultural humility and sensitivity.
- Performs and documents palliative care psychosocial assessments on the palliative care service patients.
- Collaborates and serves as a resource for social worker team members working with palliative care service patients in the hospital.
- Collaborates and assists the palliative care service in patient-family meetings for values-based goal setting and care planning.
- Participates in resourcing of outpatient services relevant to the palliative care service patients.
- Coordinates educational and outreach services relevant to the social service-palliative care interface.
- Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.
Requirements:
- Master's degree in Social Work.
- State licensure as a Licensed Clinical Social Worker (LCSW).
Preferred:
- Commitment to obtaining Palliative Care certification within 2 years.
- Minimum of 2 years of clinical experience in the hospital setting. Palliative care clinical experience is strongly preferred.
