Job Description
Under the Direction of the Supervisor, the Transition Care Manager is responsible for completing preventative multidisciplinary discharge planning for plan members presenting to the Emergency Room using AmeriHealth Caritas policies and procedures. The Transition Care Manager will assess and process discharge planning needs across all disciplines, including necessary case management referrals.
**Work Arrangement**
- This position is on-site at Reading Hospital Emergency Department
**Responsibilities**
- Review of daily ER census of plan members, receive and process facility referrals
- Face-to-face interaction with plan members, complete a survey to identify plan member needs
- Identify gaps in care, determine members understanding of illness and outpatient management of illness in addition to evaluating and coordinating and implementing discharge needs for all short and long term needs across disciplines including but not limited to PCP, home care, skilled nursing and rehabilitation services, DME, pharmacy, case management and/ or social worker referral for community needs and availability
- Schedule appointments and arrange critical follow-up calls for care coordination, and member education on how and where to seek appropriate care and follow-up services
**Education and Experience**
- Associate in Nursing
- Minimum of 3 years nursing experience in a related clinical setting
- Managed care experience preferred
- Emergency Room or Acute Discharge Planning Experience preferred. Strong Behavioral Health background strongly preferred

**JOB SUMMARY**
The Registered Nurse functions as a vital member of the healthcare team who contributes to the success of Marshfield Clinic Health System by demonstrating customer/patient focus, effective communication, professionalism and self-development, and teamwork. S/he utilizes the nursing process to assess, plan, implement, and evaluate comprehensive care provided to selected patient populations and their families. This individual demonstrates excellent clinical, problem-solving, interpersonal, and leadership skills, and makes decisions consistent with Marshfield Clinic Health System policies, department protocols/guidelines, and the Wisconsin Nurse Practice Act.
**SCHEDULE**
Weekly rotating shift of 7am-3pm week 1 and 9am-5pm week 2, with every Wednesday off.
