**Overview**
We're looking for a **Patient Care Manager Senior** to join our team. You will report directly to the Executive Director or Administrator. You will be responsible for assisting branch leadership with maintaining a high performing hospice program including managing, overseeing, and directing patient care activities to ensure the delivery of quality patient care. This role is responsible for supervising, teaching, evaluating, and developing associates and ensuring adherence to company policies and procedures and compliance with local, state, and federal regulations and regulatory agencies.
**$5,000 SIGN ON BONUS**
+ Preparing for, serving on, and facilitating the Interdisciplinary Group (IDG), as assigned, including presiding over IDG, presentation of patients for review, coordination of minutes, review of patient charts to note and address any changes to patient condition and/or the plan of care, and soliciting input from various care team members.
+ Managing and overseeing clinical and administrative and recordkeeping functions, including documenting referrals, new physicians and facilities, durable medical equipment (DME) pickup, Medicare patient eligibility, obtaining authorization and reauthorization as needed, commercial insurance verification, transfer patient documentation, satisfaction surveys, and patient benefit periods and events.
+ Directing the work of clinical associates including assigning of admit and evaluation visits, scheduling visits at the beginning of benefit period, and ensuring staff are completing visits in a timely manner.
**About You**
+ Graduate of accredited school of nursing; bachelor's degree in nursing preferred
+ Current Registered Nurse licensure in state of residence and requested employment
+ At least three years' experience of direct patient care and two years' experience in hospice or home-based care setting.
+ Demonstrates knowledge and compliance with accepted hospice principles and practice, including industry standards, regulations, and best practices (i.e., Medicare, Medicaid, JCAHO, ACHC), company policies/procedures, and understanding of terminally ill patients and their families.
+ Thorough knowledge of managed care principles, regulatory guidelines (i.e., Medicare, Medicaid, JCAHO, ACHC, and human resource) management principles

Treatment:
+ Welcome assigned patients and inquire as to their wellbeing since their last treatment.
+ Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.
+ Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy.

QUALIFICATIONS

• Masters or Doctorate of Physical Therapy from an accredited program
• Texas license or ability to obtain
• Open to New Grads and Experienced Therapists
• Must be detail oriented but also able to see the big picture

• Servant leader mindset

Role Overview Summary of Job: Enjoy driving a positive, collaborative culture within a CRNA-friendly environment.  Currently offering a schedule of five (5) 8-hour shifts, Monday through Friday, with an opportunity to transition to longer shifts option in the future.  Case mix includes General, Major Vascular, Neuro, OB, Gyn, Endo, ENT, Ophthalmology, Ortho, Plastics, Podiatry, Urology, Regional Anesthesia, and some Trauma. Compensation: $270,000 Salary, 7 weeks PTO, and $90,000 Sign-on Bonus. Role Overview Summary of Job: Join us at Covenant Medical Center to become the third member of a three-provider practice devoted exclusively to adult cardiac, vascular and thoracic anesthesia.  Call is taken from home and is divided equally among the three members of our cardiac group.  Requirements include CV Fellowship or 5 years of CV experience with competence in TEE.  Practice in a very friendly college community of 260,000 with a large patient catchment region where rich western history meets a thriving metropolitan hub with a taste of something for everyone! Cases include: Cardiac, Vascular and Thoracic Anesthesia. Compensation: $625,000 Annual Compensation, 8 Weeks PTO, and $100,000 Sign-On Bonus.
