Performs highly complex vocational nursing work. Provides for the care, treatment, and general welfare of individuals residing at the SSLC who have complex medical, mental health and/or behavioral health issues. Performs basic and complex routine and non-routine nursing tasks to promote the health of the individuals within the assigned areas and assigned shift. Collects specimens for lab work and performs health screenings. Instructs direct support professionals and other staff on how to provide service and care that support the individual’s medical needs. Performs technical nursing procedures according to facility and state regulations. Keeps informed of and follows all agency, SSLC, and unit/home policies and procedures. Provides direction and assists with on-the-job training to others such as newly hired LVNs and direct care staff. Assists Registered Nurses (RNs) with implementation of protocols, procedures and programs to improve overall health and safety of staff and individuals. Works under general supervision of an RN with moderate latitude for the use of initiative and independent judgment.

Registrations, Licensure Requirements or Certifications:

Licensed to practice as a licensed vocation nurse in the state of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact.

Or

Issued a temporary permit to perform supervised nursing duties awaiting licensure.

Initial Screening Criteria:

• Graduation from a LVN program.

• Prior employment experience as an LVN is preferred.

• Experience working or volunteering directly with persons with intellectual disabilities or other developmental disabilities is preferred.
• Experience using an electronic health record system is preferred.

Provides direction and assists with on-the-job training to others such as newly hired LVNs and direct care staff. Provides on-the-job nursing instruction and training for new LVNs, within their scope of practice, as requested by RN Supervisor. Monitors, oversees the work of, and assigns work to, LVNs as needed. Evaluates nursing intervention outcomes and provides input to other members of the healthcare team. Assists RNs with implementation of protocols, procedures and programs to improve overall health and safety of staff and individuals. Works under the limited supervision of a Registered Nurse (RN), with considerable latitude for the use of initiative and independent judgment.

Job DescriptionThe Licensed Practical Nurse in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home.ResponsibilitiesClinical Competence Provides high quality clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team, following all applicable infection control procedures. Does not alter the plan of care. Documents patient visits thoroughly and completely per policy and payer requirements and syncs timely per LHC policy. This includes maintaining an accurate and current medication listing.Quality Actively participates in the performance improvement plan, constantly works to reduce patient hospitalizations, and improve outcomes. and process to ensure positive patient outcomes.Professionalism (on time, completes training, compliant) Attends and actively participates in all meetings including case conferences and is punctual. This includes having a camera on for video meetings. Attends work each day with a positive attitude, willing to help the patient and agency in any way they can and is open to learning new skills.Teamwork (on call, productivity) Schedule flexibility is needed in this role to ensure we provide care to the patient as needed. Requires the ability to participate in a weekend rotation and potential availability on observed holidays to ensure we are meeting the demands of our patients.
