The Wellness Coach specializes in Exercise Groups and Wellness Programming and is the liaison between Wellness, Rehabilitation Services, and Nursing.

Responsibilities

Manages and maintains residents' wellness/restorative nursing programs and reports any changes to the treatment plan. Conducts exercise groups that meet the MDS criteria for restorative nursing and documents the care provided. Maintains specialized equipment, such as alarms, cushions, and wheelchairs. Collaborates with interdisciplinary team by planning and anticipating long term and/or short term needs regarding residents' admission and discharge.

    Successful completion of a Wellness Training and Restorative Nursing Class.

  • Certified Nursing Assistant in good standing on the State Nursing Assistant registry or appropriate entity
  • Hospital or nursing home experience

  • If the person holding this position leads the program they must meet qualifications as required by federal regulations *483.15(f)(2) The activities program must be directed by a qualified professional who--
    • Is a qualified therapeutic recreation specialist or an activities professional who--
    • Is licensed or registered, if applicable, by the State in which practicing; and
    • Is eligible for certification as a therapeutic recreation specialist or as an activities professional by a recognized accrediting body on or after October 1, 1990; or
    • Has 2 years of experience in a social or recreational program within the last 5 years, 1 of which was full-time in a patient activities program in a health care setting; or
    • Is a qualified occupational therapist or occupational therapy assistant; or
    • Has completed a training course approved by the State.

Essential Functions:

Description

The Wellness Coach is responsible for increasing membership growth and retention. Primary responsibilities include strengthening member commitment through authentic personal relationships, as well as connecting and engaging members, serving as the primary point of support for members who utilize the wellness center and maintaining a clean and safe exercise environment. The Wellness Coach will partner with the Member Experience Representative to create a welcoming, safe and seamless connection experience for new members seeking to develop and maintain healthy lifestyle habits.

ESSENTIAL FUNCTIONS:

Service Skills:
  1. Perform new member uFit sessions in accordance with YMCA of Northwest North Carolina protocol. This could include instructing members in training procedures and exercise techniques.
  2. Provide individual supervision and encouragement during new members' uFit sessions.
  3. Track new members in their wellness program and schedule follow up appointments.
  4. Assist all members on equipment, answering questions and demonstrating proper use.
  5. Ensure health / medical consent form is properly completed prior to a scheduled appointment; ask member about any health concerns as they may be related to exercise program recommendations.
  6. Remain alert to the overall activity in the wellness center, enforcing wellness center policies and guidelines.
  7. Maintain a clean and safe exercise environment. This can include eliminating floor clutter, wiping down equipment and organizing weights to reduce potential hazards.
  8. Report equipment problems to Wellness Director and place "out of service" signs according to YMCA of Northwest North Carolina protocol.
  9. Know and follow YMCA of Northwest North Carolina and facility-specific risk management and emergency procedures.
  10. Refrain from endorsing or encouraging unproven "fad" regimens; refrain from diagnosing injuries, offering medical advice and dispensing or endorsing any and all commercial health products, especially nutritional supplements, vitamins, performance enhancing substances and pain relievers.
  11. Communicate daily with Member Experience Representative regarding new member uFit appointments; schedule new member uFit appointments and determine next steps to reschedule missed appointments.
Member Engagement Skills:
  1. Understand how the Wellness Department partners with the Membership Department to offer a safe and seamless connection to new members.
  2. Motivate and educate members in leading healthy lifestyles; develop wellness plans for various fitness levels and populations.
  3. Build authentic personal relationships with members, volunteers and co-workers; help members and volunteers connect with one another and the YMCA.
  4. Be readily available to assist members and answer questions.
  5. Listen actively and respond to members in a professional, timely manner. Address concerns and complaints quickly and effectively.
  6. Be knowledgeable about all program areas and share information with members.
  7. Take professional initiative to promote programs, personal training, boot camps, classes and clinics during encounters with members.
  8. Demonstrate effectiveness in team building.
  9. Show understanding of individual and group dynamics.

  • Completes mental health assessments and individual classroom observations when challenging behaviors are arise. Works with Early Childhood Education Coordinators and Professional Development Coach on CLASS and CSEFEL classroom observations, results, and improvement plans.

  • Provides in-class interventions to address behavioral and adjustment problems with identified children.

  • Provides stress reducing strategies to staff and parents.

  • Conducts parent workshops to provide relevant information about mental health and social emotional development in young children.

  • Facilitates referrals to community mental health providers, as needed and participates in multidiscipline team meetings to provide social work consultation.

  • Conducts classroom visits to evaluate implementation of IEP’s & IFSP’s.

  • Works with Family and Community Partnership Coordinator and Health & Disabilities Services Coordinator in implementing family goals related to a family crisis event.

  • Works with the Family and Community Partnership Coordinator, Early Childhood Education Coordinator, and Health & Disabilities Services Coordinator in designing, conducting or arranging for mental health and disability trainings for families.

  • Works with a team to implement a coordinated approach throughout the program environment. transition activities for children with special needs leaving the program.

  • Attend all required meetings, pre-service, in-service, and other training as required.

  • Provide sound technical support, guidance and motivate staff in their professional development through coaching and mentoring

  • Adhere to established Work Plan and update as necessary or directed by Supervisor.

  • Adhere to Head Start Standards of Conduct and PathStone’s Action Pledge.

  • Assist in the physical maintenance and care of center facility such as sweeping, mopping, sanitizing and garbage disposal.

  • Implement PathStone’s operational policies and procedures, Head Start Program Performance Standards, state and local licensing regulations. Delegate, observe, monitor, evaluate and provide feedback to center staff and Deputy, Early Childhood Education regarding compliance.

  • Collaborate with professional and community organizations, mental health consultants to incorporate and improve social and emotional opportunities for staff, enrolled children, and their families.

Position Summary

Under authority of the Assistant Director, the Wellness Coach assures that all operational procedures are followed. The Wellness Coach is responsible for utilizing psychiatric rehabilitation principles to promote recovery, community integration and improved quality of life for residents through collaboration with consumers, team members, family members, significant others and collateral resources. The Wellness Coach will assist team members and other providers with an understanding of the Psychiatric Rehabilitation Model and the day-to-day practice of this modality. Wellness Coach is responsible for working in accordance with the values and policies of RHD.

Essential Duties and Functions

  • Utilizing psychiatric rehabilitation principles to promote recovery, full community integration and improved quality of life for residents
  • To maintain a safe, supportive and recovery-focused environment that fosters growth and rehabilitation as well as facilitates the development of natural supports and self-help skills
  • To engage consumers in a trusting and supportive relationship, providing emotional support within the professional boundaries, as needed
  • To support the individualized recovery plans developed with consumers with the aim of achieving and maintaining physical health, mental health and emotional well-being as well as participating in the recovery process.
  • To assist the consumer to reach the goals he or she has set in their recovery plan
  • Practice and encourage advocacy activities to enable self-advocacy efforts.

General Function:

With an emphasis on ensuring the highest quality service to members and guests, the Wellness Coach will work with all staff to welcome, connect, support, and engage all members, guests, and staff.

Qualifications:

1.Minimum of 18 years of age.

2.Must have a minimum of 6 months experience in the wellness field.
• Grade III requirement: minimum of 6 months experience in the wellness field.
• Grade IV requirement: minimum of 2 years in the fitness filed or up to 2 years college level exercise science.

3.Must obtain YMCA Healthy Lifestyle Principles and Foundations of Strength and Conditioning within first 6 months of employment.

4.Certification in CPR/First Aid/AED/O2 required within 60 days of hire.

5.Complete and maintain required trainings to include: New Employee Orientation (NEO), Bloodborne Pathogens, Child Abuse Prevention (CAP), and Sexual Harassment.

6. Satisfactory completion of a criminal background check and Child Protective Services check.

Essential Functions:

1.Support the mission, vision and goals of the YMCA.

4.Supervision of the wellness area.

5.Engage YMCA members through coaching on wellness activities.

6.Provides member training, orientations, and evaluations as scheduled.

7.Provides and documents daily, weekly and monthly equipment checks.

8.Performs daily cleaning in the wellness area and follow documentation procedures.

9.Input maintenance requests in association maintenance software.

10.Attend staff meetings and trainings as scheduled.

11.Wear staff uniform and name tag, or professional attire when appropriate.

12.Act as a leader in emergency situations. Report any accidents or incidents according to Emergency Communication Action Plan.

13.Follow Association standards.

14.Carry out other related duties as deemed necessary by the Wellness Department and center operations to ensure an excellent member experience.
