We're looking for someone to join our team as a Case Manager LVN ECM who:
Will co-manage and collaborate on patient cases with Care Coordinators (CCs) and Outreach Workers to ensure that patients receive high-quality care, understand their responsibilities as participants (Pediatric/Adult) and that appropriate orders are reviewed and completed in the Electronic Medical Record (EMR).
Essential Functions:
- Perform patient care functions as prescribed by provider following established protocols, policies, and procedures within their scope of education, training and responsibilities.
- Assist medical personnel with special procedures, minor surgical procedures and/or diagnostic exams.
- Assess and monitor patients' conditions and notify provider for disposition.
- Prepare and administer medications as directed by provider order or standing orders, following established protocols.
- If, State certified, initiate and maintain Intravenous solutions as prescribed by provider, following established protocols.
- Maintain equipment and supplies. Identify and arrange for equipment needing repair.
- Apply principles of aseptic technique and infection control.
- Monitor patient flow and assign/direct other personnel as needed.
- Provide medical information and education to patients, following established protocols and policies.
- Document pertinent patient information, assessment and nursing procedures, following established protocols. Maintain patient confidentiality.
- Documentation must be dated, legible and with the appropriate signature to all entries (first initial, last name and title). All pre-printed prompts are to be answered.
- On a temporary basis, may be required to work at any satellite facility.
- Demonstrates clinical knowledge and skill in the care of the newborn, infant, toddler, child, adolescent, adult and geriatric patients ranging from 0-100+ years of age.
- Facilitate the practice of the assigned providers.
- Perform routine clerical functions as assigned (making appointments, telephone calls, etc.

- Graduation from an accredited School of Vocational Nursing.
- Current California Vocational Nurse license required.
- Current I.V. certification preferred.
- Current Basic Life Support card required.

You'll take the lead on proactively preparing patient charts- ensuring all information is obtained and up-to-date for those seen in our facility. **Some of your daily activities will include:**
+ Assisting in preparing patients for examinations or procedures, assisting with procedures, relaying instructions from the physician to a patient or authorized person, and collecting patient data
+ Administering medications specifically directed by the physician and performing accurate vital sign measurements and documentation.
+ Maintaining a clean, organized, and safe environment, and performing patient care services that support the physician / provider's practice under the clinical supervision of the Physician, Registered Nurse, Nurse Practitioner, Physician's Assistant, or Licensed Vocational Nurse.
+ Comfortable managing medical records- charts and EMR
+ Communicate and document any patient concerns to physicians, physician's assistants, or nurses
+ Work with physicians, insurance providers, and pharmacies to obtain authorizations for patient medication, including refills

+ High school diploma (or equivalent) required
+ Satisfactory completion of a formal Medical Assistant program pursuant to the Division of Allied Health Professions or military training that is equivalent to an accredited Medical Assistant program (determination by Dignity Health Medical Foundation H.R. department in conjunction with the State Division of Allied Health Professions) required
+ Excellent interpersonal, organizational, and customer service skills required
+ Medical terminology, familiarity with CPT and ICD-10 coding procedures and reference tools, and pharmacology appropriate to the Medical Assistant scope of practice required
+ Current BLS certification required (must be certified through the American Heart Association)
Preferred Qualifications:
+ Six (6) months of experience in an outpatient setting as a Medical Assistant preferred
+ Certification must be obtained through one of the private agencies approved by the Medical Board of California:
+ American Association of Medical Assistants (preferred)
+ American Medical Technologists
+ American Medical Certification Association
+ California Certifying Board of Medical Assistants
+ Multi-skilled Medical Certification Institute, Inc. (NCCT)
+ Familiarity with an electronic practice management system preferred

We're looking for someone to join our team as a Registered Dental Assistant who:
Under the Direct supervision of a dentist, which means supervision of dental procedures based on instructions given by a licensed dentist who must be physically present in the treatment facility during the performance of those procedures. The duty must be performed pursuant to the order, control and full professional responsibility of the supervising dentist. Such procedures must be checked and approved by the supervising dentist prior to dismissal of the patient from the office of said dentist.
Essential Functions
- Dental Assistant will be proficient in four handed dentistry.
- Assist the dentist in all aspects of chairside assisting. These duties include but not limited to:
- Draping and seating the patient in dental chair. Placing proper tray set up and developing room preparation. Will confirm all treatment is recorded in the chart and all the necessary information of the patient visit is indicated on route slips.
- Operate Dental radiography equipment
- Apply topical agents
- Remove dressings
- Must be an active and enthusiastic team member.
- Will fill in as Dental Receptionist to answer telephone calls, route and assist incoming patients and any other tasks as designated.
- Follows Infection Control policies
- Follows sterilization protocols, and consistently demonstrates adherence to the sterilization process.

You'll be successful with the following qualifications
- Two years’ experience is preferred
- Must possess both California license for Registered Dental Assistant and sealant certificate.
- Preferably bilingual in English and Spanish.
- Willingness to participate in expanded duty program as such program becomes operational.
- Desire to work closely with patients and staff.
- Graduation from high school.
- Possession of State Radiological Certification
