Summary The position is eligible for Education Debt Reduction Program (EDRP), student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Responsibilities The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) in Home Based Primary Care (HBPC) practices in Extended Care and Palliative Medicine Service to provide a full range of health care services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management. The major duties and responsibilities include, but are not limited to: Independently manages a panel of patients with acute and chronic medical, social, and behavioral conditions, with an emphasis on caring for older, frail, homebound patients. Visits patients in their homes in the community. Assesses, identifies, and diagnoses patients' general medical problems and defines the medical management. Develops and implements an individualized treatment plan to achieve optimal patient health outcomes. Demonstrates leadership and works collaboratively with HBPC interdisciplinary team members.
