PRINCIPAL DUTIES AND RESPONSIBILITIES:
As a member of the nephrology healthcare team, you will provide safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse. 

•    Sets up, tests, and operates hemodialysis machines for patient treatments.
•    Obtains and documents necessary pre- and post-treatment vital signs and weight for each patient.
•    Evaluates vascular access pre-treatment and performs vascular access cannulation.
•    Evaluates intradialytic problems and provides intervention as prescribed by physician order or as directed.
•    Monitors patients’ response to dialysis therapy.
•    Evaluates patient prior to termination of venous access and discontinues dialysis treatment according to established procedures.
•    Reports any significant information and/or change in patient condition directly to supervisor.
•    Enters all treatment data into the designated clinical application in an accurate and timely manner.
•    Collects, labels, appropriately prepares, and stores lab samples according to required laboratory specifications.
•    Cleans and disinfects dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
•    Assists other healthcare members in providing patient education.
•    Prepares, organizes, and efficiently uses supplies and equipment to prevent waste.
 

EDUCATION AND LICENSES :
•    High school diploma or G.E.D. required.

EXPERIENCE AND REQUIRED SKILLS :
•    Qualities and traits: dependable, compassionate, caring, supportive, collaborative, reliable.
•    Previous patient care experience in a hospital setting or a related facility (preferred but not required).
•    Continued employment is dependent on successful completion of the Fresenius Medical Care dialysis training program and successful completion of CPR certification.

•    Day-to-day work includes desk and personal computer work and interaction with patients, facility staff, and physicians. The position requires travel between assigned facilities and various locations within the community. Travel to regional, business unit and corporate meetings may be required.
•    The work environment is characteristic of a healthcare facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious diseases/materials.
•    The position provides direct patient care that regularly involves heavy lifting and moving of patients and assisting with ambulation. Equipment aids and/or coworkers may provide assistance. This position requires frequent, prolonged periods of standing and the employee must be able to bend over. The employee may occasionally be required to push and/or pull equipment, exerting up to 15 pounds of force. The employee may be required to lift and to lower solutions on a frequent basis of up to 30 lbs., and on an occasional basis lift up to 40 lbs., as high as 5 feet. There is a 2-person assist program and "material assist” devices for the heavier items.

The rate of pay for this position will depend on the successful candidate’s work location and qualifications, including relevant education, work experience, skills, and competencies.

Hourly Rate: $24 - $27

The Clinical Director/Director of Patient Care Services establishes, implements and evaluates goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy. Essential Job Functions/Responsibilities 1. Available on premises or accessible by telecommunications during operating hours. 2. Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel. 3. Provides guidance and counseling to coordinators and Clinical Supervisors to assist them in continually improving all aspects of hospice care services, provided through organization personnel. 4. Assists Clinical Supervisors in managing clinical teams and planning. 5. Provides help in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated. 6. Assists the Executive Director/Administrator in the preparation and administration of the organization's budget. 7. Interprets operational indicators to detect census changes and increases or decreases in volume that could impact staffing levels, revenues or expenses. 8. Evaluates performance of Clinical Supervisors. 9. Assists Clinical Supervisors to develop skills and techniques in evaluating the performance of clinicians. 10. Conducts annual evaluations on clinicians, or more frequently if indicated. 11. Oversees the maintenance of patient clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities. 12. Assures proper maintenance of clinical records in compliance with local, state and federal laws. 13. Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services. 14. Develops, implements and evaluates the orientation program for new organization personnel. Responsible for orientation of new organization personnel, directly or delegated. 15. Plans and implements Inservice and continuing education programs to meet education and training needs of organization personnel at least quarterly. 16. Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of hospice services provided through the Organization. 17. Assists in the development of organization goals. Develops, recommends, and administers Organization policies and procedures. 18. Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and, maintains compliance to the CHAP Home Care standards. Position Qualifications Registered nurse in the State with current licensure to practice professional nursing in the State. Bachelor’s degree in Nursing from an accredited program by the National League for Nursing. Master’s degree in hospice preferred. Three years of management experience within the last five (5) years in a hospice or related health care organization, at least one (1) year of which was in a supervisory or administrative capacity, OR A Registered Nurse with four (4) years’ experience within the last five (5) years in a hospice, home health agency, primary care clinical or health facility, at least one (1) year of which was in a supervisory or administrative capacity. Demonstrated ability to supervise and direct professional and administrative personnel. Ability to market and deal tactfully with customers and the community. Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and is in good working order.
