Our medical center’s Adult Emergency Department has 52 beds, which includes 4 fast track beds, and 5 behavioral health beds. Nurse/Patient ratios are 3:1 or 4:1. Nursing requirements: At least 1 year recent acute care experience and Associates Degree in nursing from an accredited school. Active RN license required.
Shifts: 12 hour night shift, 36 hours per week
Work location: Columbia, Maryland
Salary Range: Minimum $36.05/hour - Maximum $6.18/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

Job Summary:
The Patient Navigator is responsible for enhancing the patient and family experience throughout the healthcare journey. This role serves as a liaison between patients, families, and the healthcare team to ensure seamless continuity of care during hospital stays and following discharge.
Key Responsibilities
- Serve as a consistent point of contact for patients and families throughout their care experience
- Act as a liaison between patients, families, and clinical/non-clinical departments. Collaborate with multidisciplinary teams to support care coordination
- Document and escalate patient feedback and service recovery needs to appropriate leadership.
- Support the patient experience by providing timely, courteous updates and non-clinical support to patients and families
Daily Duties:
- Coordinate and schedule follow-up appointments with outpatient clinics after ED discharge
- Collaborate with case management and outpatient staff to ensure smooth transitions of care
- Call patients who left Against Medical Advice (AMA) or Left Without Being Seen (LWBS) to offer support and encourage follow-up care
- Round in the ED to Provide non-clinical updates and check-in with patients and families
- Maintain accurate documentation of all patient interactions and follow-up efforts in the electronic health record (EHR)
- Bachelor's degree in progress or completed, with a focus in pre-health sciences, biology, chemistry, or related field
- Experience in a clinical or hospital-based setting is a plus
- Comfort using basic office technology. Experience using electronic medical record (EMR) systems is a plus
- Excellent interpersonal communication and a high level of professionalism
- Strong organizational and critical thinking skills
- The desire to grow and be challenged to learn and improve
- Must have the ability to work in periodically fast-paced environments
- Ability to read, write, comprehend through listening, and speak fluent English
- Must have a computer or access to a computer for virtual/online training

Physical Requirements:
- Able to perform all duties using a computer, keyboard, and other technology
- Ability to bend at the waist, kneel, climb stairs, reach in all directions, fully use both hands and legs, possess good finger dexterity, perform repetitive motions with hands/wrists/elbows and shoulders, and reach in all directions
- Ability to hear (corrected) and understand all information provided by patient or healthcare provider and transcribe into clear written records
- Strong English verbal and written communication skills with the ability to interpret large amounts of information
- Ability to work in an environment with moderate to high levels of noise and exchange accurate information in these situations transcribing the words and activities of the provider-patient portion of the encounter
- Maintain 20/40 vision corrected
- See and recognize objects close at hand and at a distance
- Work in a latex safe environment and work indoors
- Frequently lift and/or carry objects weighing 50 lbs. (+/-) unassisted
- Continuous requirements are to perform job functions while standing, walking, and sitting
