We are looking for a Nurse Practitioner or Physician Assistant to join our medical team in the Indiana PA area, covering our primary care office and multiple Nursing facilities in the region. Our providers work in both our primary care office and Nursing Facilities settings providing consultation, primary and follow-up long term care. We collaborate closely with our patients, their families, rehab staff, physicians, and hospital systems. The incoming PA/NP will be covering our primary care office in Indiana, as well as multiple long term care locations in the area surrounding Indiana PA.
Job Responsibilities:
- Perform new admits, consultation, primary and follow-up care for long term care residents
- Round on patients, track patient progress, remove obstacles to functional progress, and improve quality of life.
- Engage with clinical staff on a regular basis and maintain a professional, collegial relationship with other facility physicians, caregivers, administrators, and directors.
- Engage with patients and families during care and communicate clearly to promote understanding of treatment options and procedures. Set expectations with facility, patients, and families.
- Attend facility team meetings as needed
- Documentation: Follows protocol and guidelines for documentation, billing, and coding.
- Regularly meet with facility leadership to understand their needs and build relationships. Report back to Director facility happenings and updates
- Actively collaborates with treating physicians, providers and treating groups(referrals, wound care, Orthopedists, etc.)
- Graduation from an accredited Nurse Practitioner OR Physician Assistant Program
- Active/Current NP/PA license in PA
- Willingness to travel between multiple long term care facilities
Schedule:
- Full time (4- or 5-day contract- incoming physician preferred)
- Up to $125k to start plus production bonuses

- Collaborate with the registered nurses in the assessment of an individual's physical health, making appropriate referrals to community physicians for further assessment and treatment, and in coordination of medical, psychiatric, and substance abuse treatment.
- Provide ongoing support and counseling and education to team members, individuals, and family members to help them become more knowledgeable about substance abuse disorders. Submit written reports and oral testimony and presentation of opinion in commitment and other legal proceedings, as necessary.
- Attending organizational staff meetings and treatment planning meetings as requested to assess patients and assist with staff training and education, as necessary.
Assertive Community Treatment (ACT) Teams are composed of professionals including psychiatrists, nurses, mental health professionals, substance abuse professionals, employment specialists, housing specialists, and a peer specialist, all of whom collaborate to provide complete services including therapy, medication management, and resource linkage to adult individuals with serious mental illness (SMI). Services are provided in the individual's home or the community and include at least three home visits a week. Will be required to utilize personal vehicles for client transportation and community-based work.

Position Summary
We are currently hiring an experienced Rig Moving Crane Operator to join our crew. This role involves operating mobile cranes for rig moves, equipment lifts, and specialized hauling projects across the region.
Responsibilities
- Operate hydraulic and conventional cranes safely and efficiently
- Assist with rig-up/rig-down and job site preparation
- Read lift plans and execute according to project requirements
- Conduct pre- and post-operation inspections
- Communicate clearly with rigging crew and project leads
- Travel to job sites with overnight stays as needed (per diem provided)
- Maintain a strong focus on safety at all times
- NCCCO certification required (TSS, LBT, LBC, or equivalent)
- At least 2 years of crane operating experience, ideally in oilfield or heavy haul
