Job Description Summary

At South Shore Hospital, neurosurgery patients are cared for most often on Pratt 6 North. Patients can be treated for many head or brain related issues such as brain and spine tumors, trauma, degenerative spine disorders, and hydrocephalus. As the only Level II trauma center in southeast Massachusetts, this unit is equipped to perform complex spinal and cranial surgery when every second counts.

Job Description

Pratt 6 North Patient Population:   

As an RN on the Pratt 6 North unit, you can expect to care for patients with critical neuro-related ailments or injuries. Many patients on this floor require round the clock monitoring and have conditions where quick assessment and action is crucial. 

Common procedures and treatments:   

  • Stroke
  • Tumors (including brain, pituitary and spinal)
  • Brain hemorrhage
  • Adult onset hydrocephalus 
  • Head trauma 
  • Chiari malformation
  • Concussion
  • Herniated disc(s) (cervical & lumbar) 
  • Lumbar stenosis 
  • Degenerative disk disease(s)
  • Spinal fracture
  • Sports-related head and spine injury
  •  Advanced chronic pain
  • Trigeminal neuralgia
  • Ulnar nerve palsy

An RN joining the Pratt 6 North team MUST have the following skills, certifications, or specific experience:   

1. RN license from accredited Nursing School

2. ACLS certification (must obtain within 6 months of hire if not already obtained)

3. At least 6 months’ experience practicing as an acute care, bedside RN (new grads without experience are encouraged to apply to SSH’s new grad program) 

4. At least 6 months’ experience with Telemetry (including interpreting rhythms) 

5. TCARR (Trauma Care After Resuscitation) (must obtain within 6 months of hire if not already obtained) 

6. ERT (Emergency Response Training) (must obtain within 6 months of hire if not already obtained) 

Although not required, many skills, certifications, and/ or previous experiences would greatly benefit an RN looking to join this team. If you have any of the following, you would be at a significant advantage for training into this unit at South Shore Hospital: 

1. Previous Neurosurgery experience in an acute in-patient setting

2. Previous use of the Epic healthcare system 

3. Experience with the following treatments or equipment: 

  • --- Brain or spinal surgery recovery 
  • --- Ventriculoperitoneal shunts 
  • --- Stereotactic radiation therapy 
  • --- Spinal cord stimulation for chronic pain 
  • --- Intrathecal Baclofen pump for muscle spasticity

ESSENTIAL FUNCTIONS

1. Test Performance

a. Performs accurate and appropriate testing of specimens received in the laboratory, according to established laboratory protocol and procedures.
b. Evaluates specimens for acceptability for proper testing.
c. Investigates and documents problems with unacceptable specimens.
d. Organizes workload for maximum efficiency in order to complete tests being performed.
e. Accurately performs tests as ordered on specimens received.
f. Verifies accuracy of results as those belonging to the corresponding specimen before reporting results.
g. Collects data as required by supervisor on unacceptable specimens and specimens without orders or with ambiguous orders.
h. Notifies Microbiology supervisor or designee of any preanalytical, analytical, or post analytical variances.
i. Handles specimens according to appropriate storage temperature requirements and as per laboratory procedures.
 

2. Test Result Reporting

a. Reports test results in a timely manner and according to established laboratory protocol and procedures.
b. Reports results within time limits established for test procedures.
c. Documents proper notification process required for test results in the reporting of all critical values that the employee results that meet the established criteria.
d. In the event of an occurrence resulting in incorrect results reported on a patient, employee consistently follows the established policy on Correcting Results including the notification and documentation of corrective actions taken.
e. Notifies the Infectious Disease provider or primary care provider regarding Microbiology alerts in a prompt fashion and documents communication in the LIS system.

g. Notifies the Massachusetts State Laboratory Institute regarding reportable diseases in a prompt fashion and documents communication in the LIS system.

3. Follows established procedures for laboratory quality control and reports discrepancies to the appropriate supervisor.

a. Performs appropriate quality control for the test procedure.
b. Accurately analyzes and evaluates QC results obtained before accepting and reporting patient test results.
c. Records results obtained for quality control testing as defined in test procedure.
d. Appropriately notifies supervisor of discrepant QC results.

4. Performs daily, weekly, monthly or as needed, maintenance on instruments and equipment as appropriate. Performs calibration of equipment as required.

a. Performs maintenance according to the schedule for the instrument/equipment.
b. Documents performance of maintenance procedures for the instrument/equipment.
c. Performs calibration of equipment according to scheduled intervals or receipt of reagents.
d. Documents calibration of equipment.

5. Troubleshoots instruments, equipment, reagents and patient specimen's when problems occur. Notifies the supervisor if unable to solve the problem.

6. Consistently adheres to established Laboratory and Hospital policies for Universal Precautions, Chemical Hygiene and Safety procedures. When handling blood and/or body fluids, Personal Protective Equipment (PPE) will be utilized in order to minimize exposure to infectious diseases. Chemical Hygiene and Safety policies and procedures will be followed when using chemicals.

a. Wears lab coat, gloves, goggles and protective devices as required when handling specimens.
b. Follows safety policies for dress.
c. Follows safety policies when using chemicals.
d. Reports any safety hazards to the appropriate supervisor.
e. Uses protective barrier equipment as required.
f. Disinfects and clean work benches/areas according to established protocol during and at the end of assigned shift.

An Invasive Cardiovascular Technologist, also known as a Cardiovascular Invasive Specialist, plays a crucial role in diagnosing and treating heart diseases. Responsible to perform all required procedures in the Cardiac Catheterization /EP Laboratories and to operate specialized equipment including, but not limited to, the X-ray equipment within the Cardiac Catheterization Laboratory, computer systems, hemodynamic monitoring and have working knowledge of invasive cardiac procedures, perform in all first and second assist duties under the supervision of a physician, prepare patients and equipment for cardiac catheterization and interventions, perform common catheterization interventions including, stent placements, balloon angioplasties, heart valve replacements, and electrophysiology tests, operate intracardiac or intravascular ultrasound equipment, fluoroscopy equipment, or other imaging systems, assisting with handling venous and arterial catheters and wires, monitor blood pressure and heart rate using electrocardiogram (ECG or EKG) equipment during procedures including coronary angiography, pacemaker implantation, aortic stenosis studies, and patent foramen ovale closures, calculate hemodynamic or physiologic values, and recording waveforms for patient records and retrieve and analyze imaging and other data obtained during procedures.

ESSENTIAL FUNCTIONS

1 - Procedure Documentation

   a - Captures All documentation required for all procedures in Mac/Cardio according to standards.

   b - Maintains accuracy of database. Insures all patients and procedures are recorded accurately in database

   c - Verifies procedure log is accurate and assists MD with procedure reports.

   d - Utilizes knowledge of A/P and cardiac procedures to document in Mac/Cardio Lab appropriately and carefully. to include but not limited to the following:

  • Flushing of sheaths and maintaining sheath integrity to ensure proper homeostasis.
  • Will introduce catheters into vessel structures consistent with current CDIC standard operating procedures.
  • Will obtain hemodynamic and angiographic data by operating transducer and manifold. Assists in operation of mechanical atherectomy and thrombectomy devices.
  • Under direct order of physician, will position, inflate, and deflate both balloons and stents inside vessels.
  • Will assist with all balloon, catheter, sheath, and wire exchanges.
  • Expected to be able to demonstrate and perform proper post access care and management (e.g mechanical closure devices and or manual compression.
  • Must be able to convey pertinent patient history to the Physician in a fashion that is consistent with current pre and post care.

2 - EQUIPMENT
Is knowledgeable about and has the ability to operate complex and sophisticated equipment related to a Cardiac Catheterization/EP Laboratory.

   a - Operates electronic equipment to properly monitor patients' heart rhythm and blood pressure during procedure.

   b - Prepares and assists MD with radiographic equipment.

   c - Prepares and assists MD with EP/ Vascular and coronary equipment

   d - Prepares and assists MD with cardiac cath equipment such as IABP and Angiojet.

   e - Prepares and operates and maintains other Cardiac Cath/EP equipment according to policy /manufacturer recommendations to i.e., ACT machine, ablator, OCT, IVUS, RFR, IFR, FFR, Cath Works, Angiojet, CSI etc.

   f - Demonstrates thorough knowledge of location, care and operation of all cath lab equipment. Able to trouble shoot and resolve simple equipment problems or notify appropriate resources for resolution.

3 - SAFETY/QUALITY - Fosters a "Culture of Safety" through personal ownership and commitment to a safe environment.

   a - Verifies patient identification with two identifiers prior to the start of any invasive procedure, including "time out", administration of care, medications, labeled specimens and documents confirming the correct patient, procedure, site, equipment, and consent

   b - Complies with the current CDC hand hygiene guidelines through proper handwashing. Adheres to universal precautions, makes appropriate use of personal protective equipment at all times and appropriately disposes of hazardous materials.  Maintains awareness of MSDS sheets and how to access.

   c - Assesses each patient’s risk of falling using appropriate interventions to prevent falls.

   d - Ensures environment meets regulatory requirements at all times (ie: no cardboard boxes on floor, care of soiled U/S Probe)

   e - Understands and is able to demonstrate individual roles and responsibilities in the event of hospital codes/emergency preparedness.

   f - Holds oneself and others accountable for meeting quality improvement/quality standards.

   f - Uses practical knowledge of anatomy for proper positioning to obtain maximum diagnostic information, as evidenced by image captured.

   g - Selections technical factors to insure high quality, radiographs using the A.L.A.R.A. (as low as reasonably achievable) standards.

   h - Practices radiation safety with all radiographic equipment and overseas that others practice radiation safety.

6 - DATA MANAGEMENT
performs various clerical duties including, but not limited to:

   a - Verification of information and procedures performed are accurately transcribed in cardiac catheterization log book.

   b - Verification of information and procedure performed using computer system.

    c- Assures correct billing of patient accounts at time of procedure and regularly reconciles patient billing as directed by nurse manager.

   d - Maintains accuracy of log book and Mac Lab database.

7 - INVENTORY

   a - Maintains stock inventory and initiates orders of all procedure areas.

   b - Notifies Lead Tech/NM of stock replacement needs.  Orders equipment as needed with Lead Tech/NM.

   c - Properly charges for all medical surgical items used. 

   d - Restocks as needed.
