Essential Responsibilities

• Provide skill training on targeted goals with measurable outcomes as determined by a Qualified Mental Health Professional.
• Deliver intensive, community-based mental health services using a strengths-based, skill-building approach.
• Collaborate with clients, families, and treatment teams to develop and maintain client-centered treatment plans.
• Help clients understand their symptoms and explore possible solutions.
• Assist clients and families in engaging natural community supports to develop effective coping strategies.
• Coordinate mental and physical healthcare services, facilitating communication among clients, providers, families, and community partners.
• Adhere to privacy and security regulations regarding Protected Health Information (PHI).
• Maintain professional and effective communication with clients, team members, and external agencies.

• Required: Bachelor's degree in Social Work, Psychology, or related field; or Associate's degree plus 2+ years of experience educating clients on self-care and daily living activities; total education and experience must equal 4+ years.
• Preferred: Bachelor's degree in Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics.
• Preferred experience: 3-5 years in mental health services, crisis response, or related fields.

• Required: CPST Case Management training, KHS training, background checks including KBI, child and adult abuse registries, motor vehicle screening.
• Completion of Adult Case Management training within 60 days of hire.
• Valid driver's license, insurance, and reliable vehicle if driving is required.

• Familiarity with community mental health services.
• Strong organizational skills.
• Intermediate proficiency with Microsoft Office.

• Perform a variety of laboratory tests and procedures with precision.
• Analyze and interpret test results, providing findings to healthcare professionals.
• Ensure compliance with quality control and quality assurance standards.
• Maintain laboratory equipment and resolve any technical issues.
• Collaborate with interdisciplinary teams to deliver comprehensive patient care.

• Medical Technologist degree (MLT/MT) from an accredited university.
• ASCP certification is required.
• Open to both experienced and entry-level candidates.
• Experience in a hospital or clinical setting preferred.
• Proficiency in a wide range of laboratory tests and procedures.

The RN Case Manager will coordinate all aspects of patient care in the home setting. This role involves collaboration with various medical professionals and delivering quality care to patients. You will also conduct the initial visit to assess the patient’s eligibility for home care services and develop individualized care plans.

Qualifications:

• Graduate of an accredited nursing program (diploma, Associate, or Baccalaureate).
• Valid state license as a Registered Nurse.
• Current Driver's License.
• At least one year of RN experience; two years preferred.
