DEFINITION
Under general direction, plans, organizes and directs the day to day professional nursing activities and related functions on an assigned shift at the in-patient psychiatric treatment facility, and other County outpatient clinic locations; collaborates with other medical staff and interdisciplinary health treatment teams to evaluate and prepare treatment plans; and performs related duties as assigned.
SUPERVISION RECEIVED AND EXERCISED
Receives general direction from assigned supervisory or management personnel. Exercises direct supervision over professional and technical staff.
CLASS CHARACTERISTICS
This is the full supervisory-level class in the Psychiatric Nurse series that exercises independent judgment on diverse and specialized nursing services in support of the County's in-patient psychiatric health facility or out-patient programs with accountability and ongoing decision-making responsibilities associated with the work. Incumbents are responsible for planning, organizing, supervising, reviewing, and evaluating the work of assigned staff and for organizing and overseeing day-to-day nursing, psychiatric, and support services program implementation for hospitalized patients. Employees are responsible for providing professional level support to management in a variety of areas. Performance of the work requires the use of independence, initiative, and discretion within established guidelines.

- Accepts report of previous shift and updates new admissions and medications; makes appropriate nursing notes on patients charts and writes medication orders; assures that patient charts contain accurate and complete documentation.
- Organizes, assigns, directs, reviews and evaluates the work of assigned staff and/or an interdisciplinary treatment team in the delivery of patient care; maintains a safe and secure environment; serves as case coordinator for assigned patients or clients, as needed.
- Performs end-of-shift narcotics count and maintains records of medications in compliance with regulations. Oversees and provides professional nursing care to patients; observes and charts vital signs, including temperature, blood pressure, pulse, and respiration.
- Oversees and provides equipment and supplies required for patient examination or treatment.
- Oversees and administers oral and hypodermic medications and observes and records their effects; obtains and records all orders from physician to facilitate patient treatment.
- Oversees and participates in the preparation of the nursing component of patient treatment plans, including physical assessments on new patients; as a member of an interdisciplinary team, oversees and participates in diagnostic and therapeutic activities.
- Audits nursing assessments, reports, and restraint procedures.
- Performs crisis intervention, in person and on the phone; manages assaultive behavior and provides general counseling; coordinates or arranges for the provision of needed services including emergency detention when patient is in danger to self or others.
- Oversees and provides supportive services including individual, group or family counseling, and various group activities such as exercise and art.
- Confers with medical, behavioral health and inter-agency staff regarding client needs and ways of improving staff's ability to impact on clients.
- Conducts or arranges to have staff conduct case conferences; facilitates decisions on difficult behavioral health cases and problems; and performs chart audits and utilization review.
- Maintains prescribed standards of behavioral health treatment and ensures services are rendered in conformance with policy and procedural guidelines.

Education and Experience:

Environmental Factors and Conditions/Physical Requirements
PHYSICAL DEMANDS
- Mobility to work in a standard office setting, or clinical setting, and use standard office equipment, including a computer, and medical equipment; standing in work areas and walking between work areas may be required, travel to visit outpatient and rural clinics may be required; occasionally bend, stoop, kneel, reach, push, and pull drawers open and closed to retrieve and file information; ability to lift, carry, push, and pull materials and objects up to 25 pounds, in all cases with the use of proper equipment and/or assistance from other staff.

WORKING CONDITIONS
- Employees work in an office, out-patient, clinic, jail, juvenile facility or locked psychiatric facility environment with moderate noise levels, controlled temperature conditions, and no direct exposure to hazardous physical substances.
- Employees may work outdoors and are occasionally exposed to loud noise levels and cold and/or hot temperatures.
- Employees interact with clients with behavioral disorders/erratic and assaultive behavior, including those which require emergency crisis intervention.

EQUIPMENT AND TOOLS UTILIZED

Disaster Service Worker
All Butte County employees are designated Disaster Service Workers through state and local law (California Government Code Section ). Employment with the County requires the affirmation of a loyalty oath to this effect. Employees are required to complete all Disaster Service Worker related training as assigned, and to return to work as ordered in the event of an emergency.

Salary Step Increases:
The beginning salary and the top of the salary range are usually shown on the job announcement. Upon satisfactory service, salary increases of approximately 5% may be given annually until the top of the salary range has been attained.
Cell Phone Allowance:
County issued cell phone or $ 0/month allowance with appointing authority approval.
Boot Allowance:
200.04 annuallly, paid in equal monthly payments through regular pay checks.
Holidays:
12 paid holidays per year.
Vacation:
0-4.99 years: 15 days/yr.; 5-9.99 years: 20 days/yr.; years: 25 days/yr.; 20+ years: 27 days/yr. Note - accrued vacation leave may not be used until after six months of employment.
Administrative Leave:
7 days per year for FLSA Exempt employees.
Sick Leave:
96 hours of sick leave are earned each year with no maximum accrual.
Bereavement Leave:
Maximum of 40 hours for each occurrence in the immediate family.
Health Plan including Dental Insurance and Vision Insurance:
Coverage under the Butte County Health Plan is for employees and their eligible dependents. Depending upon the plan selected by the employee, the County contributes a substantial amount of the total cost of the complete health package, which consists of medical, vision, and dental insurance. Currently, the amount the County contributes for family coverage is /month. For those employees who have health coverage from other sources, and who elect to not participate in the Butte County Health Plan, the County offers a monthly taxable Cash-Back allowance. The current of the taxable Cash-Back allowance is 503.34 per month (prorated for less than fulltime employees). This benefit is 300 per month for Employees hired on or after January 1, 2014.
Life Insurance:
The County provides 25,000 in life insurance with additional coverage available at a minimal cost.
Retirement:
The County is in the Public Employees' Retirement System (CalPERS) coordinated with Social Security.
"Classic Members"- 2% @ 55 . Final compensation based on Single Highest Year. Employee responsible for full 7% employee share effective 1/1/2013.
"New Members" to PERS- 2% @ 62. Final compensation based on Highest Three Year Average. Employee responsible for one half (1/2) the normal cost of the CalPERS Pension.
"New Member" is defined by law (PEPRA) as an "1) individual who has never been a member of any public retirement system prior to 1/1/2013 or 2) An individual who becomes a member of a public retirement system for the first time on or after January 1, 2013, and who was previously a member of a public retirement system, but who was not subject to reciprocity. or 3) An individual who was an active member in a public retirement system and who, after a break in service of more than six months, returned to active membership in that system with a new employer
Deferred Compensation:
The County offers a 457 Deferred Compensation program. Deposits into this plan are strictly voluntary and are made through a payroll deduction on a tax-deferred basis. The County does not make contributions to the employees' Deferred Compensation accounts.
Disability Insurance:
For short-term disability, the County participates in California State Disability Insurance (SDI) fund. County employees are also covered by an employee-paid long-term disability program. The long-term disability insurance benefit is 60% of the disabled employee's biweekly wage.
Flexible Spending Program:
County employees can elect to participate in a Flexible Spending Program. This program allows employees to use pre-tax dollars to pay for certain health care and/or dependent care expenses. In addition, employees can choose to have their portion of the monthly Health Plan premium withheld from their checks on a pre-tax basis as well. Generally, employees who participate in the Flexible Spending Program have less taxes withheld for their payroll checks than if they had elected not to do so.
Tuition Reimbursement:
Up to 500/year.

The County of Butte provides an Employee Assistance Program (EAP) for employees and their eligible dependents. The EAP offers confidential, professional counseling services.

What happens after I submit an application for a lateral transfer or a voluntary demotion:
Once you have completed and submitted your application, Human Resources will review and minimally qualify you for the position(s) you indicated an interest in. When we receive an approved P-2 for recruitment, we will check the transfer list before opening a recruitment, and refer the names of those employees who have requested a transfer and have an approved transfer request to the hiring manager. Summary
The Butte County Behavioral Health Psychiatric Health Facility (PHF) is seeking nurses (RN) to provide direct client care to patients receiving inpatient psychiatric treatment in our facility treating our community members in psychiatric crisis. Care includes, but is not limited to, consultation with providers, participating in multidisciplinary team meetings, medication administration, managing admissions and discharges, oversight of facility functioning and providing quality documentation. Pay does include a 10% site differential and scheduling is based on applicant availability.
The eligibility list created from this recruitment will be used to fill regular help full-time and part-time vacancies, as well as extra help.
Extra Help means this may be a Part-Time or Non-Permanent position. The number of hours worked per week may vary and depend on the needs of the department. For terms and conditions concerning Extra Help with Butte County, see Section 6.13c of the Butte County Personnel Rules.
Employees assigned to the Psychiatric Health Facility (PHF) or Crisis Services will receive a 10% site differential and those assigned as part of the Resiliency Empowerment Team (Rest) will receive a 5% site differential on top of the base salaries listed below.

DEFINITION
Under direction, performs and provides professional nursing and psychiatric care to individuals striving to overcome behavioral health challenges; collaborates with other medical staff and interdisciplinary health treatment teams to evaluate and prepare treatment plans; and performs related duties as assigned.
SUPERVISION RECEIVED AND EXERCISED
Receives direction from assigned supervisory or management personnel. Exercises no direct supervision over staff.
CLASS CHARACTERISTICS
Psychiatric Nurse I: This is the entry-level classification in the Psychiatric Nurse series. Initially under close supervision, incumbents learn and perform routine professional nursing and psychiatric care. As experience is gained, assignments become more varied and complex; close supervision and frequent review of work lessen as an incumbent demonstrates skill to perform the work independently. Positions at this level usually perform most of the duties required of the positions at Psychiatric Nurse II-level but are not expected to function at the same skill level and usually exercise less independent discretion and judgment in matters related to work procedures and methods. Work is usually supervised while in progress and fits an established structure or pattern. Exceptions or changes in procedures are explained in detail as they arise.
Psychiatric Nurse II: This journey level classification is responsible for performing direct nursing services in support of the County's in-patient or out-patient psychiatric health facilities. Positions at this level exercise judgment and initiative in their assigned tasks, receive only occasional instruction or assistance as new or unusual situations arise and are fully aware of the operating procedures and policies of the work unit. This class is distinguished from the Senior Psychiatric Nurse in that the latter performs the more complex work assigned to the series, such as reviewing psychiatric nursing work practices and providing lead technical and functional direction over lower level staff.
Psychiatric Nurse III: This is the advanced journey-level classification in the Psychiatric Nurse series responsible for the oversight of psychiatric nursing services at an assigned clinic and/or performing the most complex and specialized work assigned to the series. Incumbents regularly work on tasks which are varied and complex, requiring considerable discretion and independent judgement. Positions in the classification rely on experience and judgement to perform assigned duties. Assignments are given with general guidelines and incumbents are responsible for establishing objectives, timelines, and methods to deliver services. Work is typically reviewed upon completion for soundness, appropriateness and conformity to policy and requirements.
Positions in the Psychiatric Nurse class series are flexibly staffed; positions at the II- and III-levels may be filled by advancement from the I- and II-levels, respectively; progression to the next level in the series is dependent on (i) management affirmation that the position is performing the full range of duties assigned to the classification; (ii) satisfactory work performance; (iii) the incumbent meeting the minimum qualifications for the classification including any licenses and certifications; and (iv) management approval for progression to the II-level.

- Confers with medical, behavioral health and inter-agency staff regarding client needs and ways of improving staff's ability to positively impact clients; serves as case coordinator for assigned patients or clients, as needed.
- Participates in case conferences and provides input regarding behavioral health cases and problems.
- Provides medication refills per physician's orders and policies and procedures; provides needed information to pharmacies and insurance companies.
- Coordinates care with primary providers, pharmacies, laboratories, schools, medical facilities and personnel, probation, social services, and other government agencies.
- Accepts report of previous shift and updates new admissions and medications; makes appropriate nursing notes on patients charts and writes medication orders; assures that patient charts contain accurate and complete documentation; performs and documents end-of-shift narcotics count.
- Organizes, assigns, reviews and evaluates the work of staff on scheduled shift, as assigned.
- Maintains prescribed standards of behavioral health treatment and ensures services are rendered in conformance with policy and procedural guidelines.
- Adheres to the confidentiality standards of the Privacy Act of 1974 (U.S.C. § 552a), as amended.
- Performs related duties as assigned.

Education and Experience:

PHYSICAL DEMANDS
- Mobility to work in a standard office setting, or clinical setting, and use standard office equipment, including a computer, and medical equipment; standing in work areas and walking between work areas may be required, and travel to visit outpatient and rural clinics may be required; occasionally bend, stoop, kneel, reach, push, and pull drawers open and closed to retrieve and file information; ability to lift, carry, push, and pull materials and objects up to 45 pounds or heavier weights, in all cases with the use of proper equipment and/or assistance from other staff.

Disaster Service Worker
All Butte County employees are designated Disaster Service Workers through state and local law (California Government Code Section ). Employment with the County requires the affirmation of a loyalty oath to this effect. Employees are required to complete all Disaster Service Worker related training as assigned, and to return to work as ordered in the event of an emergency.

Salary Step Increases:
The beginning salary and the top of the salary range are usually shown on the job announcement. Upon satisfactory service, salary increases of approximately 5% may be given annually until the top of the salary range has been attained.

Holidays:
12 paid holidays per year.
Vacation:
0-4.99 years: 15 days/yr.; 5-9.99 years: 20 days/yr.; years: 25 days/yr.; 20+ years: 27 days/yr. Note - accrued vacation leave may not be used until after six months of employment.
Sick Leave:
96 hours of sick leave are earned each year with no maximum accrual.
Bereavement Leave:
Maximum of 40 hours for each occurrence in the immediate family.
Health Plan including Dental Insurance and Vision Insurance:
Coverage under the Butte County Health Plan is for employees and their eligible dependents. Depending upon the plan selected by the employee, the County contributes a substantial amount of the total cost of the complete health package, which consists of medical, vision, and dental insurance. Currently, the amount the County contributes for family coverage is /month. For those employees who have health coverage from other sources, and who elect to not participate in the Butte County Health Plan, the County offers a monthly taxable Cash-Back allowance. The current of the taxable Cash-Back allowance is 503.34 per month (prorated for less than fulltime employees). This benefit is 300 per month for Employees hired on or after January 1, 2014.
Life Insurance:
The County provides 25,000 in life insurance with additional coverage available at a minimal cost.
Retirement:
The County is in the Public Employees' Retirement System (CalPERS) coordinated with Social Security.
"Classic Members"- 2% @ 55 . Final compensation based on Single Highest Year. Employee responsible for full 7% employee share effective 1/1/2013.
"New Members" to PERS- 2% @ 62 . Final compensation based on Highest Three Year Average. Employee responsible for one half (1/2) the normal cost of the CalPERS Pension.
"New Member" is defined by law (PEPRA) as an "1) individual who has never been a member of any public retirement system prior to 1/1/2013 or 2) An individual who becomes a member of a public retirement system for the first time on or after January 1, 2013, and who was previously a member of a public retirement system, but who was not subject to reciprocity. or 3) An individual who was an active member in a public retirement system and who, after a break in service of more than six months, returned to active membership in that system with a new employer
Deferred Compensation:
The County offers a 457 Deferred Compensation program. Deposits into this plan are strictly voluntary and are made through a payroll deduction on a tax-deferred basis. The County does not make contributions to the employees' Deferred Compensation accounts.
Disability Insurance:
For short-term disability, the County participates in California State Disability Insurance (SDI) fund. County employees are also covered by an employee-paid long-term disability program. The long-term disability insurance benefit is 60% of the disabled employee's biweekly wage.
Flexible Spending Program:
County employees can elect to participate in a Flexible Spending Program. This program allows employees to use pre-tax dollars to pay for certain health care and/or dependent care expenses. In addition, employees can choose to have their portion of the monthly Health Plan premium withheld from their checks on a pre-tax basis as well. Generally, employees who participate in the Flexible Spending Program have less taxes withheld for their payroll checks than if they had elected not to do so.
Tuition Reimbursement:
Up to 500/year.

- Complete on-the-job training by a qualified therapist (physical or occupational)
- Orients and instructs new Restorative Aides and Certified Nursing Assistants (CNAs) as instructed
- Assists with resident meal service as assigned.
- Answers call lights, bells, or intercom and provide resident care and services needed.
- Transports resident to and from restorative program.
- Meets with DN or DOR a minimum of monthly to review each resident's performance in their restorative program.
- Answers signal lights, bells, or intercom system to determine resident needs.
- Provides assistance with bathing, dressing, toiletry, and oral hygiene activities of daily living (ADLs).
- Serves and collects food trays and feeds residents requiring help.
- Transports residents, using wheelchair or wheeled cart, or assists residents to walk.
- Turns and repositions bedfast residents, alone or with assistance, and utilizing proper body mechanics, to prevent pressure ulcers.
- Takes and records temperature, blood pressure, pulse and respiration rates, food and fluid intake and output, and weight, as directed.
- Completes documentation (e.g. meal monitor, I&O records, ADL, and Aide flow sheets).
- May be assigned to a specific area of Center.
- May be assigned to clean, disinfect, and store bedpans, bedside commodes, urinals, and other supplies by licensed nurse commensurate with CNA's abilities.
- Changes bed linens, runs errands, directs visitors, and answers telephone, usually on weekends and evening or night shifts.
- Drapes residents for examinations and treatments, and remains with residents, performing such duties as holding instruments and adjusting lights.
- Washes and dresses bodies of deceased persons as needed.
- May be assigned to complete Quality Assurance tasks.
- May be assigned to provide Nursing Restorative Program.
