We are seeking a highly specialized RN with strong clinical reasoning skills to join our Allergy, Asthma, and Immunology procedure-based clinic in Albany, NY!
Under the direction of the practice physician and/or advanced practice provider, Nurse Manager (RN) and/or Nurse Supervisor (RN), the RN may provide direct patient care, patient triage (in-person and telephonic), assessment, planning, directing and evaluating of a patient’s specific care plan and clinic visit.
Essential Duties and Responsibilities include:
• Obtains and accurately record patient’s vital signs; reports abnormal values to treating provider
• Documents components of patient’s history (Medical, Surgical and Social) in EHR
• Gathers and enters data for asthma control test and for peak flows of diagnosed asthma
• Provides nursing support to all providers/fellows in the clinical setting
• Administers medications and treatments to patients in clinic, under the direction of a physician or APP
• Performs review and triage of incoming test results, patient requests and pharmacy renewals; facilitates call-backs to patients as necessary
• Proficiency of administration (monoclonal agent administration education competency) of specialty medications (Xolair, Nucala, Fasenra, Dupixent)
• Allergy Desensitization Injections (subcutaneous) and Venom Desensitization Injections (subcutaneous)
• Instruction and Administration of specialty specific vaccination (diphtheria/tetanus, pneumococcal) for diagnostic immunology purposes
• Prepares Allergy Serum Compounding (Mixing) paperwork for patient specific established allergy injection patients or new patients
• Assists in the emergency management of anaphylaxis
• Proficient in performing percutaneous and intradermal skin testing in our specialty clinic (environmental, venom, food, specialty drug skin testing)
• Performs oral drug desensitization or food challenge desensitization
• Proficient in nitric oxide testing and spirometry in individuals pediatric through adult with asthma or asthma related conditions
• Teaches/Administers nebulizer treatments, self-injectable medications, spacer and inhaler in the clinic
• Coordinates durable medical equipment (nebulizers, O2, etc.) for patients in home setting
• Assists physician and arrange set up for skin biopsy procedure and rhinoscopy procedure
• Provides material for eczema wraps and bleach baths in the clinic
• Proficient in performing Patch Dermal Skin Testing in the clinic
• RN to Coordinate bed access/team approach (ICU or floor admitting MD/Pharmacy/Nurse Manager/Clinical Nurse Specialist/Consulting MD), prior authorization, protocols for inpatient oral food desensitization or specialty drug challenge or desensitization in the hospital setting under the direction and supervision of the provider

This position will work in close collaboration with a full-time board-certified geriatrician to provide comprehensive, longitudinal care to medically complex, homebound patients.
The NP will manage a personal panel of approximately 40 patients and provide cross-coverage for an additional 160 patients in the practice. The role includes home visits, telehealth support, and the management of chronic and acute medical conditions, with a strong emphasis on geriatric syndromes, care coordination, and diagnostic oversight.
Essential Duties and Responsibilities
Clinical Care:
- Provide primary care services to a panel of ~40 homebound patients through in-person home visits and virtual encounters as appropriate.
- Collaborate closely with the geriatrician to co-manage patient care across the full practice panel (~200 patients).
- Cross-cover the remaining 160 patients, including addressing urgent clinical needs, medication adjustments, and patient/caregiver inquiries.
- Conduct comprehensive assessments and develop individualized care plans that reflect patients' goals, preferences, and functional status.
- Manage acute and chronic conditions, focusing on geriatric syndromes (e.g., falls, dementia, polypharmacy, frailty).
- Coordinate with caregivers, visiting nurses, specialists, and other home-based service providers.
- Provide palliative and end-of-life care consistent with best practices and patient wishes.
Diagnostic & Lab Oversight:
- Order and interpret laboratory tests, imaging, and other diagnostic studies.
- Track and review results for the entire patient population in collaboration with the geriatrician.
- Ensure timely communication of clinically significant findings and follow-up planning.
Team Collaboration & Communication:
- Participate in regular team meetings and interdisciplinary case conferences.
- Maintain thorough, accurate documentation in the electronic medical record (EMR).
- Educate patients and caregivers regarding health conditions, medications, and treatment options.
- Contribute to quality improvement and academic initiatives within the practice.
Qualifications
Required:
- Master's or Doctorate degree from an accredited Nurse Practitioner program
- Current NY State NP license and board certification (Adult-Gerontology Primary Care or Family NP)
- DEA license and BLS certification
- Minimum of 2 years of experience in primary care, geriatrics, or home-based care
- Valid driver’s license and reliable transportation for home visits
Preferred:
- Experience with home visits and medically complex, frail older adults
- Prior experience in academic or teaching settings
- Familiarity with value-based care models and interdisciplinary care approaches
