The Bilingual Crisis Counselor provides brief, solution-focused mental health interventions, crisis interventions, and case management services to children and families that are involved in San Francisco systems including child welfare, San Francisco’s Department of Public Health Child Crisis program and the Crisis Stabilization Unit (CSU), and youth enrolled in San Francisco Unified School District (SFUSD). They work as member of a treatment team and provide services in the community, including hospitals, schools, and family homes located both within San Francisco and up to 90 miles away.

Seneca's San Francisco Hub team strives to provide care and resources to families involved in the child welfare system and foster care services. Our goal is to strengthen permanency for children in their current placement, support them through transitions, and address their mental health and behavioral needs.

Responsibilities

  • Provide crisis management and intervention for youth referred by, but not limited to, the San Francisco child welfare system, San Francisco’s Department of Public Health, SFUSD, and the Crisis Stabilization Unit (CSU)
  • Complete risk assessments, crisis counseling, creation of behavioral plans and safety plans
  • Participate in all treatment reviews, family team meetings and program staff meetings
  • Complete documentation identified by supervisor in a timely and satisfactory manner
  • Collaborate on a regular basis with team members including but not limited to: Child Welfare Workers, caregivers, school staff, therapists, and case managers
  • Participate in weekly individual supervision and group supervision to expand skills and receive ongoing coaching and support
  • Maintain professional behavior at all times, appropriately representing Seneca’s mission and philosophy

Required

  • Education/Experience Requirements:
    • Bachelor’s degree OR
    • Associate’s degree or equivalent + 1 year of applicable experience OR
    • High School Diploma/GED + 2 years of applicable experience
    • Applicable experience can include paid or unpaid experience working with youth or families or working in the social services field.
  • Bilingual in Spanish
  • Strong communication skills and flexibility
  • Must be at least 21 years of age
  • Own vehicle, Valid California driver's license and insurability with Seneca's automobile policy
  • TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements
Preferred

  • Prefer at least 9 months to 1-year experience in working with children with mental health needs in out of home care

Schedule

  • Full time
    • Monday- Friday, 2:00pm - 10:00pm

Review department policies and procedures, at least annually, and participate in making recommended changes. Assist in developing and implementing policies and procedures for identifying the medically related social and emotional needs of the resident. Participate in community planning related to the interests of the facility and the services and needs of the resident and family. Assist residents in achieving the highest practicable level of self care, independence and well being. Assist with providing medically related social services so that the highest practicable physical, mental and psychosocial well being of each resident is attained or maintained. Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Director. Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to the resident. Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs. Record and maintain regular Social Service progress notes indicating response to the treatment plan and/or adjustment to institutional life. Coordinate social service activities with other departments as necessary. Work with emotional problems including assisting resident/family with anxieties and stress caused by illness and admission to the facility, difficulties in coping with residual physical disabilities, fears related to helplessness and death, and the need for institutional and specialized care. Evaluate social and family information and assist in determining plan for social treatment. Meet with department personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident. Assist in making outpatient appointments as ordered and schedule on site ancillary patient services to include optometry, podiatry, dentistry and psychiatric services. Assists in discharge planning with appropriate agencies, entities or individuals to include agency services, equipment and agency referrals. Develop and maintain a good working rapport with other departments within the facility, and outside community health, welfare and social agencies, to assure that social service programs can be properly maintained to meet the needs of the residents.   • Assist Business Office Manager in solving financial needs and promote emotional security for the resident
  • Complete acute care plans as needed and attend care plan meetings
  • Assist Administrator with resident/family/responsible party concern management
  • Attend committee meetings as required.
  • Complete monthly QA reports.
  • As needed, refer residents for vision, dental, and hearing services
  • Assist with room changes by contacting responsible party and resident and completing necessary forms
  • Complete the initial, routine, and as needed psychosocial and socialization assessment, if needed the LSW can refer resident for additional professional psychosocial services.
  • Discuss advance care planning with responsible party upon admission and as per state regulations and complete necessary documents
  • Make morning rounds prior to morning meeting
  • Be prepared to provide staff with any important department updates at the department head daily morning meeting Perform suicidal/homicidal assessments and required follow up Assist in investigations as requested by Administrator
  • Assist with the completion of the MDS as needed
  • Work rotating weekend Manager on duty as scheduled
Qualifications

Educational/Training Requirements
  • Bachelor's or Master's in Social Work
Licensing Requirements
  • Must be a Licensed Social Worker in the state of Texas
Experience Requirements
  • Previous experience working in a skilled nursing facility is preferred

Job Description

Regency Healthcare and Rehab is seeking a compassionate Director of Social Services, to serve our residents, in our fast-paced rehab facility. Social services are services provided by the facility's staff to assist residents in maintaining or improving their ability to manage their everyday physical, mental and psychosocial needs.

A highly qualified candidate is an individual with:

  1. A bachelor's degree in social work or a bachelor's degree in a human services field including but not limited to sociology, special education, rehabilitation counseling and psychology
  2. One year of supervised social work experience in a health care setting working directly with individuals
  3. Licensed in social work preferred, but not required
Key Job Functions:
  1. The social worker, or social service designee, will complete an initial and quarterly assessment of each resident, identifying any need for social services of the resident. Any need for social services will be documented in the medical record under social services
  2. The social worker, or social services designee, will pursue the provision of any identified need for social services of the resident. Attempts to meet the needs of the resident will be handled by the appropriate discipline(s). Services to meet the resident's needs may include:
    • Making arrangement or obtaining needed adaptive equipment, clothing and personal items
    • Working with residents, resident's families and care plan teams on current goals, discharge planning and encouragement to participate in care planning
    • Informing residents and those they designate about available health care choices and their ramifications such as advance directives and code status
    • Making referrals and obtaining services from outside entities (e.g., talking books, absentee ballots, community wheelchair transportation)
    • Assisting resident with financial and legal matters if appropriate
    • Discharge planning services
    • Providing or arranging provision of needed counseling services
    • Through the assessment and care planning process, identifying and seeking ways to support residents' individual needs
    • Promoting actions by staff that maintain or enhance each resident's dignity in full recognition of each resident's individuality
    • Assisting residents to determine how they would like to make decisions about their health care and whether or not they would like anyone else to be involved in those decisions
    • Finding options that most meet the physical and emotional needs of each resident
    • Providing alternatives by understanding and communicating to staff why residents act as they do, what they are attempting to communicate and what needs the staff must meet
    • Meeting the needs of residents who are grieving
    • Fining options which most meet a resident's physical and emotional needs
  3. The resident's plan of care will reflect any ongoing medically-related social service needs and how these needs are being addressed
  4. The social worker, or social service designee, will monitor the resident's progress in improving physical, mental and psychosocial functioning
  5. Preparing and sending death certificates and monthly death report to the state
  6. Completing level ones if needed on admissions and following any required changes outlined by the OBRA office
  7. Completing sections of MDS related to social services
  8. Attending resident care plans
  9. Serving as back-up for Admissions
  10. Visits to hospitals
  11. Marketing with Admissions/Therapy when needed
  12. Other duties as assigned

Establishes course of action by exploring options; setting goals with resident and their families. Assist residents in achieving the highest practicable level of self care, independence and well being. Provide medically related social services so that the highest practicable physical, mental and psychosocial wellbeing of each resident is attained or maintained. Evaluate social and family information, psychological and emotional needs to assist in assessing social services needs as well as develop care plans for social services issues. Provide emotional support and address emotional problems including assisting residents and family with difficulties in coping with physical disabilities, fears related to health conditions as well as grief related to loss of ability and/or death. Assist residents with health care decisions. Assist with personal shopping. Assist in inventory and tracking patient belongings. Coordinate response to reports of missing, lost or stolen belongings. Assist in obtaining resources from community and social services agencies as well as health and welfare agencies to meet the needs of the resident. Assist in securing appropriate prosthetics and assistive devices. Develop and maintain a strong working relationship with other departments in the facility, and outside community agencies, so that social services can be provided to meet the needs of the residents. Document regarding resident social service status. Assist in making outpatient appointments as ordered and schedule on site ancillary patient services to include optometry, podiatry, dentistry and psychiatric services. Assist in discharge planning with appropriate agencies, entities or individuals to include agency services, equipment and agency referrals. Coordinates with interdisciplinary team. Provide in service training to the staff as requested. Counselling residents and family members. Supervisory Requirements This job has no supervisory responsibilities. Qualification Education and/or Experience Bachelor's Degree in Social Work or in Human Services and 2 years of supervised social work experience in a health care setting working directly with individuals. MSW preferred Language Skills Ability to read technical procedures. Ability to read and comprehend policy and procedure manuals. Ability to effectively present information and respond to questions from managers and employees. Mathematical Skills Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations. Reasoning Ability Ability to solve practical problems. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Certificates, Licenses, Registrations Knowledge and experience with PCC preferred. Physical Demands The essential functions of this position require the following physical abilities: Standing and /or walking very frequently. Sitting occasionally.
