**Summary:**
This is highly skilled, rehabilitative work with patients whose abilities to cope with the daily tasks of living are impaired by developmental deficiencies, aging, medical concerns, or psychological and social disabilities. Incumbents provide evidence-based occupational therapy evaluation, consultation and skilled treatment for patients. They document activities, and instruct patients, family, and students. Work is performed under the general supervision of an Occupational Therapist II or Occupational Therapist Supervisor. Clinician is expected to have involvement in the areas of continuing education, departmental and community activities, outreach education and role of preceptor to students.
**Responsibilities:**
1. Diagnostic Testing - Administers basic & complex diagnostic tests and evaluations in the area of Occupational Therapy for the treatment of functional deficits and related impairments.
2. Education - Teach & supervise graduate Occupational Therapy students and/or clinical fellows/residents. Develops or contributes to innovative programs to enhance patient care. Serves as resource consultant to other clinicians in respective specialty area.
3. Evaluation of Care - Analyzes results of diagnostic and or therapeutic testing to establish patient goals and or modify subsequent treatment programs. Identifies expected patient outcomes. Communicates relevant information to promote continuity of care.
4. Implementation-- Selects and applies current and advanced techniques to provide quality and efficient rehabilitative services. Provides habilitation/rehabilitation and maintenance of persons with related impairments. Organizes and prioritizes care according to patient/family needs.
5. Outreach/Community Involvement-Serves as resource to North Carolina government departments and statewide health professionals in specialty areas. Presents at local (university/hospital) or state level, participates in community health screening and events. Participates in the development of new clinical programs to meet emerging needs in conjunction with the clinical specialist and/or department director.
6. Participation - Contributes to quality of clinical services through attendance and participation in departmental meetings and in-services.
7. Quality-Contributes to quality of clinical services through maintenance of valid & reliable records of patient procedures & progress. Participates in quality improvement initiatives with the clinical specialist and other healthcare professionals. Participates in continuous quality improvement efforts in conjunction with the department director.
**Preferred Qualifications:**
Outpatient Ortho and Hand therapy experience strongly preferred.

- Dresses, grooms, bathes, feeds, takes and records vital signs of patients
- Answers call lights, assists in toileting, oral care, nail care, catheter care, assists patients in wheel chairs, transports, turns bedridden patients, reports patient changes, and takes out trash
- Requires certification as a Nurse Assistant; functions may not exceed Standards of Practice as accorded by certification
- Provides simple or uncomplicated patient care
- Cleans room and changes bed linen
- Takes and records temperature, pulse, and respiration rate
- Gives medication as directed by Physician or Nurse and makes notation of amount and time given

- Minimum age requirements is 18 years
- Valid driver's license, with satisfactory driving record, as defined by agency policy
- Certification, if required by state licensure as a CNA or Home Health Care Aide in the state in which the aide is to provide care
- Completion of a recognized home care aide training program or 6 months of recent experience as an aide
- Must be able to utilize proper body mechanics while lifting and may be expected to lift up 75 lbs.; must be able to sit, stand, and walk for periods of time and bend, stoop, push, pull, and reach

To provide consistent and continuous assistance and care to residents and to assist licensed nursing staff with treatments and care of residents as ordered or needed.
Essential Job Functions and Requirements:
Employee work shifts as scheduled. Willing to work other variable hours when necessary.
- Assists residents with bathing, dressing, personal hygiene, and all ADL's as needed during shift
- Changes linen on beds with bath schedule and/or PRN
- Takes and records signs per policy and/or as directed
- Assists residents in and out of bed as needed
- Turns residents while in bed per Facility policy
- Repositions residents while out of bed per Facility policy
- Assists with oral hygiene including denture care when getting up in the morning, after meals, at bedtime, and/or when needed
- Answers call bells promptly following Facility policy
- Completes patient care records at end of shift, including accurate I and O
- At the end of month, fill in Patient Observation Records and Activity Attendance Records for the next month
- Collects urine or stool specimens as directed
- Gives enemas as directed
- Keeps patient unit/halls clean and neat
- Keeps bedside tables, chest of drawers, and closets clean, neat and organized. (Including clothing)
- Cares for and cleans nursing equipment. (BP equipment, thermometers, etc.)
- Follows restraint policy, including release
- Assists with post-mortem care when needed
- Assures residents have fresh ice water by refilling pitchers with ice and/or water at least once every shift
- Cleans water pitchers, cups, bedpans, urinals per schedules
- Rinses personal clothing, sheets, etc. if soiled before placing in appropriate laundry barrel
- Assists residents to bathrooms as needed or assists with bedpan or urinal
- Empties catheter bags at end of shift and records output on appropriate record
- Assists with rehabilitation programs such as ambulation, bowel and bladder retraining, grooming, etc. always charting on Rehabilitation sheet every shift
- Assists charge nurse with resident needs and treatments as directed
- Keeps a clean towel and a clean washcloth for each patient in bathrooms as all times. (Replaces immediately when used.)
- Empties incontinent pad barrel and linen containers every shift and replaces plastic bags in containers
- Reports at end of shift to oncoming CNA's
- Performs other duties as assigned
