We re offering a one-time signing bonus to South Carolina-based physicians willing to credential with us, sign our collaborating physician agreement, and fulfill all position requirements. Physician collabs earn a monthly stipend based on the number of Signify Health-contracted nurse practitioners and/or physician assistants they oversee.
You must be licensed in and living in South Carolina in order to qualify.

Our clinicians provide personalized In-Home Health Evaluations, commonly known as Health Risk Assessments, to health plan members.
- Pre-scheduled visits with people in their homes
- Flexible schedule based on your preferences and availability
- Additional compensation opportunities via our pay-per-test model (for visits where the health plan member qualifies to receive additional diagnostic and preventive tests)
- Malpractice coverage during Signify Health-related assignments
Payment is contingent on the signing and execution of the collaborating physician contract/agreement and the fulfillment of all position requirements. The bonus payout is at the sole discretion of Signify Health.
