We are seeking Program Supervisors to oversee operations in ourPsychiatric Residential Intensive Treatment Program (PRTF), dedicated to our **adolescent clients** , ages 10-21.Program Supervisors oversee, support, supervise and act as a resource to direct support staff and individuals served. Administrative tasks are also required as needed.
***Starting pay: $24.00 per hour.**
Additional compensation for Experience & Bachelor's degree or higher.
Shifts That May Be Available:
**1st: 6:30am-3:15pm**
**2nd: 2:30pm-11:00pm**
**3rd: 10:45pm-7:15am**
**Floating: 12:00pm-8:00pm (can vary, must be flexible)**

**Qualifications**
**EDUCATION:** Bachelor's Degree in the field of counseling, mental health, social work, psychology/sociology, rehabilitation, special education, or human services field preferred; OR Some college education w/60 college credits in human services preferred.
**EXPERIENCE:** Two (2) years of supervisory experience preferred (degree); or four (4) years of supervisory experience in a mental health related discipline required (non-degree); Direct care experience with the I/DD (intellectual/developmental disabilities) population, specifically children and adolescents preferred.

+ Must be a minimum of 21 years of age.
+ Excellent communication skills, both written and verbal; leadership, relationship management, and multi-tasking skills are required.
+ Strong technical skills and ability to utilize in-house computer applications.

+ On-call, weekend availability and some holidays required.

The duties and responsibilities include performing the day-to-day administrative secretarial and receptionist functions of the facility in accordance with current established procedures, and as directed by the Office Supervisor.

Job Summary:


Works under the direct supervision of the Director of Operations with certain responsibilities to the assistant site manager. Meets with the new residents and follows-up with family members to ensure that their transition is a smooth one. Reviews the requirements and ensures that the residents’ new environment is complete. Renders care to all residents. Ensure that CNAs are following up and caring for the residents. Works with the residents to ensure that their concerns are heard and followed up on. Works with each department to ensure that their requests are addressed. Carries out specialized duties pertaining to the Medication Administration Record (MAR) checking. Reports any medication needs or concerns to management immediately. Responsible for reviewing the schedule and will work with the scheduler to ensure the schedule is done in a timely and accurately manor. Is diligent in following up with any paperwork and record keeping pertaining to their specialized duties. Observes and reports unusual symptoms and conditions of residents immediately (verbally and in writing) to the wellness nurse as well as the assistant site manager. Diligently reads the Log Book each day so that you are familiar with the previous occurrences of the day or previous shifts. Must become knowledgeable of the state, federal, and local guidelines.


Qualifications:

General Responsibilities:

Assisting and treating residents in a pleasant manner conducive to their safety and comfort. Adhering to instructions issued by Management and to the established Assisted Living routine. Performing duties in accordance with these established methods and techniques as outlined under “Duties” and in the Policy and Procedure Manual, providing residents with maximum care.


Responsibilities:

1. Reviews the resident’s file to ensure that you are familiar with each resident so that you will be able to assist them.

2. Checks to make sure that residents have had all meds, baths, etc and that they and their rooms are clean.

3. Assist residents with appropriate activities of daily living.

4. After obtaining any notes or assignment, make rounds and let residents know you are there for them. Inform residents of any special appointments or activity at this time when you meet with them.

6. Make sure residents are ready for and on time for meals when time permits.

7. Assist kitchen staff with serving meals, when time permits.

8. Know Emergency Procedures as listed in Emergency Procedure Book.

9. Never discuss resident’s condition with anyone other than facility staff members. Confidentiality is a Must.

10. Never discuss information regarding the facility, its residents, or staff with anyone other than appropriate facility management personnel.

11. Exercise good hand washing techniques (always). Wash hands before passing medications, before serving meals, (if this happens), and before and after assisting residents.

12. When confronted with conditions, which have resulted in exposure to blood, follow, the instructions as outlined in the Blood Borne Policy guideline posted bulletin board and in the office.

13. Notify the director of operations, assistant site manager and the nurse when a resident needs additional clothes, personal hygiene supplies, protective undergarments, etc. However, you must rely on the caregivers for this information. Each resident is assigned a laundry day; on the laundry schedule, please review this with the team leaders and front office. That schedule should be followed unless there is an emergency. Ensure that staff follows procedures and laundry is done RIGHT and residents get their clothing.

14. Record any pertinent information that occurs on your shift and ensure that this is communicated to the appropriate person prior to your ending you shift.

15. Under No Condition should any caregiver expose him/her to lifting weights (anyone or anything) which would endanger their own physical condition, simply remind them of that policy.

16. Check to see if all forms (waivers/releases etc) are in files.

17. Keep your walkie/talkie with you during the day at all times.

18. Be knowledgeable of the voice communicative system.

19. Be knowledgeable of all drills: fire drills etc…

20. Make sure that daycare participants are on the roster and needs are met.

21. Other duties as assigned.

· Requirements for personal protective equipment include control kit and rubber gloves and smock and non-slip shoes.

· Supervisors work in all areas of the building.

· Supervisors are expected to use personal protective equipment and supplies to prevent burns, falls and infection and follow safety policies and procedures.

**KEY RESPONSIBILITIES:**
1. Knowledge of assisted living policies and procedures regarding all aspects of medication management
2. Remains current in knowledge of state and federal rules regarding scope of practice, medication storage & disposal, infection control practices & bio-hazardous waste disposal.
3. Able to accurately read, and follow medication orders and medication labels.
4. Able to accurately transpose medication orders to the MAR & make any needed corrections to the MAR according to physician orders.
5. Follows the Six Rights of Medication Administration
6. Able to identify concerns related to medication orders, medication instructions or resident reactions to medications and takes appropriate actions.

13. Makes appropriate physician & family contacts in a professional manner when order clarification is needed, regarding resident reactions to medications or other medication concerns.
14. Accurately documents assistance/administration and resident refusal of medications according to policy and professional standards.
15. Follows policy, professional standards, and the law regarding storage, assistance/administration of, documentation and disposal of controlled medications
16. Immediately reports and documents any medication errors to the administrator or his/her designee per policy and takes appropriate actions for resident's well-being.
17. Reports and documents any resident change in condition.
18. Uses good judgment in seeking necessary assistance from peers, supervisor, administrator, physician, family and EMS as needed.
19. Obtain accurate pulse and respiration rate; obtains accurate blood pressure, body temperature, and blood glucose reading
20. Accurately documents the effectiveness of medications given on an as needed basis and does so in a timely manner.
21. Monitors medications for accountability by verifying medications have been ordered appropriately, are received as ordered, and controlled med drug counts are done at change of shift.
22. Communicates effectively from shift to shift & to others (family, pharmacy, physician etc.) as necessary to facilitate the accuracy of any needed follow-up regarding resident medications.
23. Routinely inspects, cleans and monitors the med storage system, equipment and supplies for resident safety. Reports any concerns to the supervisor for correction or repair.
24. Assists direct care partners with their duties as part of the direct care team
25. Completes thorough face to face change of shift report to CMA relieving for next shift.
26. Performs other duties as assigned that are within the scope of the CMAs practice, training, and abilities.

Responsibilities

Home Health with Less Travel!

Our Registered Nurses (RNs) supervise, evaluate, and provide nursing services to clients in various senior living communities, including assisted living and independent living facilities.

Our facility-based RNs enjoy:

* providing care to our patients within the beautiful confines of the assisted living facilities where our patients reside
* 1:1 patient care
* the ability to follow each patient from start to discharge

Enhabit RNs work collaboratively with facility wellness staff on a daily basis to ensure strong lines of communication and overall continuity of care for the residents and their loved ones. Additionally, our facility-based RNs consult as needed with the physician and the office giving details about patient care and act as the point of contact for all disciplines involved with providing care to patients. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of our Registered Nurse RN Home Health.

Qualifications

Registered Nurse RN Home Health must meet the following requirements:

Be currently licensed as a Registered Nurse (RN) in the state of employment A minimum of one year of clinical experience is preferred Demonstrate knowledge and skill in current nursing practice Possess a valid state driver's license and automobile liability insurance

Job Summary: Provide nursing care in accordance with facility and nursing service policy and procedure. Assist with orientation and training of personnel. Communicate and observe the Corporate Compliance Program and comply with Code of Conduct.

Responsibilities:

  • Recognize significant changes in conditions of residents.
  • Administer medications according to procedure and be aware of pharmacological and therapeutic effect.
  • Chart medications and treatments according to procedures.
  • Record nursing information on resident's care plans and clinical records, including nursing notes, MARs, treatment record, and transcription of physician orders.
  • Observe, report and record resident reaction to drugs, tests, treatments, and therapeutic measures.
  • Participate in formal and informal staff development in-service programs, workshops, institutes and conferences.
  • Inform Nursing Supervisor of departmental activities and condition of each resident.
  • Assume responsibility for assigned residents, including regular bed checks, to ensure that no resident is left unattended or permitted to wander from the premises and ensure that resident call lights are answered promptly.
  • Report incidents of reported resident abuse immediately.
  • Report all unusual occurrences (incidents/accidents) that occur during the shift.
  • Supervision of CNAs who give direct resident care on the shift.
  • Ensure rehabilitative nursing procedures are performed.
  • Record monthly resident's assessment and quarterly assessment.
  • Assist in interpreting goals and objectives of the Healthcare Department.
  • Perform compliance rounds at least twice during a shift to ensure residents' needs are met.

Qualifications:

Current Licensed Practical Nurse license in the State of GA

Good standing with the Georgia Board of Nursing

CPR certification

Supervisory experience and experience in assisted living care preferred

Excellent technical assessment and documenting skills

Schedule: Monday - Friday and as needed to ensure staffing compliance 8-4pm.

Pay: $29-$31 an hour
