**Job Summary**
The Extracorporeal Membrane Oxygenation (ECMO) Specialist is an experienced critical care clinician. This specialist works with the healthcare team to provide extracorporeal life support to patients in intensive care. They ensure appropriate patient care by monitoring, troubleshooting, and managing the ECMO circuit and equipment. The specialist partners with bedside RNs, physicians, and other healthcare team members to coordinate complex patient care. They are accountable for assisting daily ECMO program operations under the ECMO Manager or Coordinator's supervision.
**Essential Functions of the Role**
+ Assembles, prepares, primes, and evaluates the ECMO circuit for safe use on the patient. Monitors and adjusts ECMO circuit settings to maintain parameters set by the ECMO physician and guidelines. Ensures the backup ECMO circuit is ready for emergency use at all times.
+ Assesses the patient's physiologic status using monitors, lab results, and physical findings to evaluate ECMO therapy effectiveness. Partners with the ECMO physician and other healthcare professionals to develop and implement an effective care plan. Recommends changes in patient management to the ECMO physician during daily rounds based on the patient's physiologic status and the relationship with the ECMO circuit or lab parameters.
+ Participates in ECMO program education, outreach, and research activities as delegated by the ECMO Program Manager. This includes certification training, routine in-services, didactic, and simulation competencies. Maintains knowledge and participates in current research related to ECMO.
+ Performs ECMO routine circuit assessment and safety checks throughout the shift. Anticipates and prevents potential problems or emergencies. Utilizes ECMO flowsheet for ongoing, complete, and detailed documentation.
+ Monitors and manages anticoagulation levels and fluid status per ECMO guidelines. Ensures appropriate levels for optimal ECMO therapy. Reports current status to providers and ensures care is delivered as ordered.
+ Participates in the development of quality initiatives to audit outcomes and patient follow-up.
+ Provides family/patient education and creates an attentive, helpful, safe and therapeutic environment.
+ Partners in developing, implementing, and maintaining ECMO program policies, procedures, and clinical standards of care. Assists in collecting and maintaining data required by the Extracorporeal Life Support Organization and BSWH System.
+ Ensures patient safety during ECMO therapy. Helps with technical components like CRRT, plasmapheresis, blood sampling, and volume administration. Maintains scheduled pre- and post-oxygenator gases to ensure circuit integrity.
+ Assist ECMO providers with cannulation and starting therapy. Autonomously perform circuit exchanges and emergency interventions. Respond to ECPR hospital calls promptly. Perform intra/inter hospital transport as part of the Rapid ECMO Deployment (RED) Team. On-call participation may be required.
**Key Success Factors**
+ Proficient in the knowledge, beliefs, techniques, and operation of extracorporeal membrane oxygenation equipment and systems like Rotaflow, Centrimag, Cardiohelp, LifeSparc, Protek Duo, Quadrox, Nautilus, and AMG.
+ Detailed knowledge of extracorporeal help physiology, membrane gas exchange physics, and physiology. Understanding critical care disease pathologies and cardiothoracic postoperative management. Knowledge of acute respiratory distress syndrome physiology and mechanical circulatory help physiologies.
+ Proficient in mechanical circulatory help device operations and management strategies (e.g., percutaneous ventricular assist devices, implantable ventricular assist devices, IABP).
+ Advanced knowledge of health care modalities, therapies, equipment, and medications in cardiac intensive care. Proficient in managing solid organ transplant patients.
+ Ability to follow safety, infection control, and isolation procedures at all times. Uses critical thinking and clinical judgment to work independently.
+ Must work under stress using critical thinking and problem-solving skills. Social communication skills are necessary to work with employees, physicians, patients, and family members. The position requires advanced troubleshooting skills.
+ Evaluate ECMO therapy quality by assessing the patient's physiologic status. Use bedside monitors, invasive hemodynamic monitoring, lab results, and physical assessments.
+ Advanced knowledge of ECLS physiology and ECMO modalities not limited to Veno arterial, Veno venous, Veno pulmonary and eCPR.
+ Completion of a two-year AMA approved respiratory care program required.
+ Bachelor's degree preferred.
+ Experience in Clinical Respiratory Therapy in Intensive Care Unit required; Cardiothoracic or Transplant ICU experience preferred.
+ At least 1 year of direct ECMO patient care required.
+ Completion of an ELSO-endorsed training course or another ECMO Training Program based on ELSO Guidelines at a registered ECMO center is required.
+ Adult Critical Care Specialty RRT preferred.

JOB SUMMARY:
The term "ECMO Specialist" is defined for the purpose of these guidelines as "the technical specialist trained to manage the ECMO system and the clinical needs of the patient on ECMO under the direction and supervision of a licensed ECMO trained physician. The individual functioning as the ECMO Specialist should have a strong critical care background in neonatal, pediatric and/or adult critical care.
The Extracorporeal Membrane Oxygenation (ECMO) Specialist (ES) is responsible for ensuring appropriate patient care through the continuous monitoring and assessment of the ECMO circuit, related technology, and patient status. The incumbent identifies and corrects problems associated with the ECMO procedure and initiates and evaluates interventions consistent with protocol. The ES consults with the ECMO physician when patient condition and/or protocol limits or directives are in question. As a member of the ECMO team, this position works closely with other team members and health care providers to provide patient care in accordance with the patient's individualized care plan.
MAJOR DUTIES / CRITICAL TASKS:
The ES systematically and continuously collects and assesses data in collaboration with the ECMO team to provide therapeutic care for the patient and family.
· Monitors and adjusts ECMO circuit settings (e.g. heat exchanger temperature, pump flow, sweep gas) to maintain parameters established by the ECMO physician.
· Recommends administration of blood products, fluids, and/or medications as warranted by ECMO status.
· Assesses the cardiopulmonary status of the patient through test results (e.g. ABGs, CBC, etc.) and physical assessment findings.
· Performs patient assessment to anticipate and prevent potential problems (e.g. variances in blood pressure, perfusion, ventilation, and heart rate).
· Monitors and manages intake and output records to ensure appropriate fluid levels.
· Monitors coagulation results to assess clotting status; adjusts heparin infusion
per ECMO Heparin protocol.
· Evaluates chest x-rays for endotracheal tube placement, cannulae position, and status of the lung fields in determining patient status and weaning guidelines; supports and maintains patency of the cannulae while patient is undergoing ECMO in collaboration with the ECMO physician.
· Supports ECMO team research through the collection of ECMO-related data, preparation, and presentation of research abstracts and/or other documentation.
· Demonstrates expertise in adult, neonatal, and/or pediatric care including patient assessment, technical skills, and psychosocial aspects of care.
· Actively participates in the evaluation of new equipment. Effectively identifies and troubleshoots equipment problems.
The ES will participate as a member of the ECMO Team, working with the health care providers in providing quality care for the patient and their family.
· Initiates multidisciplinary collaboration to positively impact the outcomes of care provided.
· Collaborates with the ECMO physician and other health care providers involved in the management of the patient to coordinate all tests and evaluations (e.g. head and cardiac ultrasounds and EEGs) and provide care in accordance with the patient's individualized care plan.
· Recommends modifications in patient management to the ECMO physician based on the relationship between patient diagnosis and ECMO circuit and/or lab parameters.
· Initiates steps to support and enhance the patient's responsibility and self-determination in decision-making concerning health, treatment, and well-being.
· Evaluates effectiveness of teaching interventions and patient/family level of understanding and adjusts teaching and plan of care
Communication
· Participates in monthly interdisciplinary ECMO case reviews and in-services to share information and determine appropriateness of patient care plans.
· Utilizes the ECMO flow sheet to maintain ongoing complete and detailed documentation of patient history and the amount of blood and other fluids
administered and any complications that may occur.
· Provides family/parental education and reinforces medical counseling initiated by the ECMO physician.
· Provides a clear and concise end of shift report for oncoming ECMO Specialist regarding machine settings, patient status, and patient care plans.
KNOWLEDGE/SKILLS/ABILITIES
· Satisfactory maintenance of clinical competencies: Attendance to quarterly water drills
· Satisfactory completion of the annual ECMO didactic program, satisfactory score of 90% and above on annual ECMO exam
· Satisfactory completion of required special procedure classes (e.g. blood administration)
· Knowledge regarding quality improvement and standards of care within practice area
· Experience teaching patients and families
· Excellent communication skills
· Knowledge of and ability to evaluate and assess arterial blood gas measurements.
· Ability to respond to emergent or stressful situations appropriately, calmly, and in a timely manner
· Will respond to ECMO activation within 45-60 minutes
REQUIRED EDUCATION / EXPERIENCE:
**·** **Licensed as Certified Clinical Perfusionist (CCP) as recognized by the American Board of Cardiovascular Perfusion (ABCP) and State of Texas Licensure as a Licensed Perfusionist**
+ Graduate of Perfusion Program Preferred Baccalaureate degree and certification in care specific area
+ Two (2) years directly related critical care experience in adult or neonatal and/or pediatric care including patient assessment, technical skills, and psychosocial aspects of care.
+ Must have current BLS obtained through American Heart Association (AHA).
**WORKING ENVIRONMENT/EQUIPMENT**
+ May be required to travel in ambulance with patient.
+ Ability to lift up to 50 pounds maximum with frequent lifting/and or carrying objects
weighing up to 25 pounds.
