DSD Services teammates will have access to all the capabilities and resources DaVita has to offer. Teammates must be comfortable working autonomously and have demonstrated the ability to collaborate with cross-functional resources across the Village.The work location is on site at Alden Poplar Creek (1545 Barrington Rd, Hoffman Estates, IL 60169 ). The dialysis den is growing operations and will be open Monday - Saturday in the future. The PCT teammate is likely to work 3 days per week with no on-call. This person must be flexible to work at other DaVita SNF dialysis den locations.What you can expect:In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. PCTs will work in conjunction with dialysis RNs to provide dialysis care.Patient Care: Spending the majority of your day in direct one-on-one patient care to provide safe, comfortable and hygienic dialysis treatment.Complexity: DSD Dialysis care is a complex but fulfilling service line. Meeting and/or exceeding clinical outcomes are the goals and multitasking is an expectation to meet the needs of the service, the patients and our SNF partners. Autonomy and Critical Thinking: Provide comprehensive patient care in an independent setting. Plan to be on your toes at all times. You use your mind constantly. **Description:**
· Consider bringing your expertise to a dynamic unit. The selected candidate will join a team of Registered Nurses, Patient Care Technicians, Physicians, Residents, and other esteemed members of the interdisciplinary team to provide care to our valued patient population.
Support staff on the unit includes Stroke Coordinator, Respiratory Therapy, physical and occupational and speech therapy, case management, nutritional services, social work, pharmacy, pastoral care, and community nursing service
The unit is managed by the Nurse Manager with a designated charge nurse each shift. The house nursing supervisor provides leadership during off hours and on weekends. Neurologists, Neurosurgery, and hospitalist support is available either in-house or by pager 24/7.
Flexible self-scheduling, with designated third weekend commitment and accommodating three holidays per year.
Performs a variety of direct patient care procedures to assist the unit manager, professional staff and physicians in providing patient care focused on the needs of the patient.
Meets the comfort and hygiene needs of the patient by providing care services such as bathing, oral hygiene procedures, hair care, bed pan/urinal use, skin care and garment, linen and bedding changes. Distributes meals and beverages, takes vital signs, and collects lab specimens.
Assists with completion of patient testing and treatment procedures, sets up and primes IV tubing, 12-lead ECG, simple tracheostomy and care, changes dressings, performs cardiopulmonary resuscitation, obtains wound cultures.
Assists physician and professional staff in equipment supply and set up.
Assists with ambulating patients with assistive devices such as crutches and with transfer/transport procedures.
Performs required documentation and communicates in a competent and effective manner to meet patient and/or family needs and regulatory requirements. Maintains strict confidentiality of patient, employee and department information to ensure patient rights.
Utilizes time and resources effectively to promote quality and cost outcomes.
Other department specific duties as assigned.
**Position Requirements**
**Minimum Education:**
**Preferred:** High School Diploma plus training acquired through work experience or education
**Minimum Experience:**
**Required:** Less than one year of previous job-related experience
**Preferred:** 1-2 years of previous job-related experience
**Licensure/Certifications:**
**Required:** Certified Nurse Assistant, BLS (Basic Life Support) through AHA
**Other:** Completion of a Nursing Assistant Program with placement on the Illinois Department of Health Nurse Aide Registry or verification of an equal or higher educational level in a clinically focused program. Individuals working as Patient Care Assistants at Loyola University Medical Center prior to 1997 are exempt from the above guidelines and are grandfathered into the Patient Care Tech roles upon successful completion of an in-house orientation and training program for the Patient Care Tech position.
