The Northeast Health District's Children's Medical Services Program is seeking a Registered Nurse to provide nursing and professional expertise for its program. Duties will include program and staff supervision, oversight of program goals and deliverables, and management of a caseload of patients. A current Georgia license to practice as a registered nurse required. Requires excellent communication, critical decision-making, and interpersonal skills.

Job Summary:
Under general guidance, manages the planning and implementation of nursing and case management services provided through the District's Children's Medical Services Program. Evaluates programs for compliance with state and/or federal guidelines

As a Program Coordinator within the Children's Medical Services at the Georgia Department of Public Health, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways.
Job Responsibilities
  • Oversee and implement services to identify, assess and provide care coordination to eligible infants and children (birth to 21 years of age) at risk for poor health and developmental outcomes and those who meet diagnoses and financial qualifications specific to CMS program guidelines.
  • Support the administration of federal and state funding to ensure alignment with program goals and compliance requirements.
  • Provide day-to-day support, supervision, onboarding and training, and performance reviews for CMS staff.
  • Facilitate CMS/Child Health staff meetings and provide program leadership and supervision. Serve as point of contact and liaison to the State Office CMS Program. Participate in meetings and committees as requested.
  • Assess client needs and local service availability to pinpoint gaps in pediatric specialty care and lead the planning and coordination of specialty clinics.
  • Manage a caseload and make appropriate intervention recommendations to families to ensure optimal health and development.
  • Assist client/families in obtaining care and resources for eligible chronic health conditions. This includes facilitating prior authorizations and referrals for other services to ensure continuity of care.
  • Serve as an advocate and liaison between families and providers/vendors and transitioning families from pediatric to adult care, according to CMS policy.
  • Promote Children's Medical Services through community engagement and events.
  • Maintains knowledge of community resources, availability of services, and delivery options to meet the needs of the children within the special needs population. Identifies new partnerships and services when appropriate.
  • Monitor program compliance with the CMS service matrix, ensuring all prior authorizations are appropriate and allowable.
  • Ensure HIPAA compliance and documentation of care coordination activities, care plans, and client tracking in compliance with CMS Program requirements.
  • Conduct quality assurance and monitoring activities for service delivery and documentation.This includes performing annual chart reviews and completing quarterly reports.

Minimum Qualifications
Bachelor's degree in nursing from an accredited college or university and five (5) years of nursing experience and current Georgia Professional Nurse license and CPR certification;

OR

Three (3) years of experience at the lower-level PH Nursing Prog Consultant 2 (HCP211) or position equivalent and current Georgia Registered Professional Nurse license and CPR certified

We're seeking a **Certified** **Medical Assistant** to assist with office-based procedures.

+ The Peer Recovery Coach is responsible for responding to persons in the emergency department or hospital inpatient setting who have experienced overdose or other challenges because of use or misuse of substances (e.g. opioids, cocaine, methamphetamine).
+ Upholds the culture, principles, and values of recovery in all aspects of services provided to individuals, family members, friends, and allies and will provide connections to local recovery communities surrounding the hospital, as well as treatment and recovery support services in these communities.

+ Provides Peer Support:
+ Provides peer support to persons who have experienced overdose or other challenges with substance use or misuse.
+ Provides information and facilitates development of relationships with individuals, support/mutual aid groups, and service providers when appropriate. Follows up when the peers agree.
+ Manages boundaries in relationships and makes adjustments in accordance with guidance provided by team leadership.
+ Supports Families, Friends, and Allies:
+ Provides support to families and allies of people who have experienced overdose or other challenges with substance use in assigned emergency departments. + Some travel required, primarily to Gainesville, GA for initial training and later within Atlanta.
