Looking for an RN to fill our Hospital Liaison

* Coordinate and utilize services by obtaining needed patient information. Communicate required information for referrals to the Home Care office in a timely manner.
* Review patient's clinical record to obtain required demographic information, emergency contacts and specific orders for Home Care and/or Hospice, patient diagnoses, medications and treatments.
* Ensure appropriate continuity of care for al patients. Assess each consult to determine appropriateness for homecare or hospice based on established criteria. Assure that agency can meet the needs of the patient based on competence and availability of staff.
* Collaborate with physician as needed input to determine appropriate patient needs and plan of care.
* Work in conjunction with hospital Clinical Resource Management Department in reviewing the hospital daily census for appropriate patients, providing follow up as needed. Assist in identifying patients who may need home care/hospice, as well as current home care/hospice patients who are re-admitted to the hospital.
* Cognizant of payer requirements for patients, and acts as a resource for hospital staff. Responsible for understanding and communicating and limitations to patients regarding either Medicare or managed care coverage.
* Communication, collaborative relationships and interpersonal skills.
* Maintain a collaborative relationship with physicians and other health care providers to achieve desired patient outcomes and assure as continuous flow of patient care. Effectively promote harmonious relationships within the organization.
* Demonstrate a high level of tolerance, both mental and emotional, when dealing with stressful situations.
* Use tact, sensitivity, sound judgment and a professional attitude when dealing with patients, families, physicians, ad co-workers excelled communication skills. Follow channels of communication.

* Advise Home Care/Hospice management team of potential problems related to referrals or services.
* Demonstrate effectiveness in instructing staff, patients and families regarding appropriateness for home care. Also, able to explain Hospice benefit, including admission criteria, services provided, and levels of care (i.e. routine, general in-patient, respite, continuous care), volunteer services and bereavement counseling.
* Promote Home Care and Hospice services through education of the community and referral sources. Conduct in-services to targeted areas to foster improved understanding of services provided.
* Contribute to the education programs of students, new employee and others who may be receiving experience in this area.
* Participate in agency committees as assigned.

* Improve communication and foster trusting relationships between EHHCH staff and other referral sources.
* Build positive physician relationships through recurring office visits and other contact with physicians and their office staff.
* Gather and assess market intelligence to assist with development of programs and communication materials designed to meet the needs of referral sources.
* Communicate referral source marketing activities to appropriate EHHCH leadership.
* Implement and maintain a customer relationships management tracking system to track and measure effectiveness of program.
* Meet regularly with EHHCH leadership to discuss findings.
* Increase awareness and utilization of Excela Health clinical services and specialties.
* Educate physicians, their office staff and other referral sources on current clinical services and programs available through EHHCH.
* Demonstrate a general knowledge of all Excela Health services and a high degree of knowledge in targeted programs and services.
* Communicate operational changes or newly created programs and services to physicians and their office staff.
* Other duties as assigned.

Competency and Experience (Knowledge, Skills and Abilities)

Required Qualifications/Experience

* Graduate of an accredited Nursing Program required.
* Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice.
* Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.

Preferred Qualifications/Experience

* Minimum of one-year recent experience in home care, hospice, hospital or other related setting.
* Experience in Healthcare setting.

* Current licensure to practice as a Registered Nurse in the State of Pennsylvania required.
* Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program required.

Essential Duties and Responsibilities:
- Manages participants through their period of participation in the Program by monitoring overall program compliance, oversight of recovery activities, recommendations for treatment, and liaison with the Board.

- Conducts remote, telephonic assessment and reassessments of healthcare professional licensees to evaluate their status, overall compliance with program requirements, and progress in recovery.

- Contacts the participant, the appropriate Board/Committee (or their designee) or treatment providers, facilities and labs in response to participant non-compliance with the Program Agreement.

- Meets with applicant/participant telephonically weekly until seen by Review Committee, and monthly thereafter, to review compliance and progress in recovery.

- Responds to incoming calls on the toll-free line, as needed, and after-hour, weekend, and holiday calls on a rotating basis with other Program staff.

- Evaluates incoming information submitted by treatment providers, facilities, participants, and labs to monitor participant's progress and compliance with the Program Agreement.

- Works on issues where analysis of situation and of data requires review of relevant factors: Exercises judgment within defined procedures and policies to determine appropriate action.

- Engages in quality assurance monitoring and evaluation for the Program, as necessary.

Minimum Requirements

- Bachelor's degree from an accredited college or university in Business, Behavioral Science, or related field strongly preferred.
- Clinical licensure requirements are based on program contract requirements and are outlined in job posting

- Minimum of 1 to 3 years of experience in a behavioral health care setting focused on chemical dependency, recovery, and/or mental health.

- Minimum 3 years of experience and a strong level of comfort working in substance abuse and/or mental health treatment settings.

Maximus is currently hiring for a Mobile Medical Unit Driver to join our Veterans Evaluation Services (VES) team. This is a remote-travel opportunity. The Mobile Medical Unit Driver is responsible for maintaining, stocking, and transporting the mobile unit to scheduled clinic locations safely, on time and in accordance with company policies. Additionally, the driver will function as a Clinic Intake Specialist for the mobile unit appointments. The Mobile Unit Driver is a public-facing role providing punctual, solutions-oriented customer experience to Veterans and VES providers with a caring and positive attitude.

- Due to contract requirements, only a US Citizen or a Green Card holder can be considered for this opportunity.

Essential Duties and Responsibilities:
- Travel 100% of the time to mobile unit event sites; site locations could be in any of the 50 states.
- Transport the mobile unit to clinic locations safely, on time, and in accordance with applicable traffic laws and company policies.
- Adhere to DOT hours of service rules and regulations.
- Assist with the loading, unloading, set-up, and tear down of equipment at mobile sites, including displaying snacks/water/flags/veteran material in the waiting area and post VES signs in the vicinity.
- Perform or facilitate basic routine maintenance and interior cleaning services for mobile unit and its associated equipment, including inspecting mobile unit before and after each trip, maintaining a log of vehicle mileage and condition, completing regular medical supply inventory, reporting items to be ordered to Clinic Director/Manager to order. and replenishing stock of clinic/medical supplies.
- Facilitate the operation of the clinic day, including check-in/check-out process and managing sign in sheet, greeting each Veteran, and gathering any medical records or diagnostic results brought in by the Veteran.
- Provide information to the providers (factsheets, medication list, medical records), and assist with vital signs (weight, height, blood pressure) as needed.
- Document and update system comments regarding the Veteran appointment.
- Perform all job functions in compliance with HIPAA policies and adhere to local and externally relevant health and safety laws and policies.
- Work with Domestic Travel Team on route creation and trip timelines.

- Must have an excellent driving record without any serious driving violations for the last five (5) years.

- Due to insurance requirements, driver must be a minimum of 25 years to operate company vehicles

- Must be willing and able to travel out of state with overnight stays along clinic routes

- Must be willing and able to work weekends as required

- Valid driver's license is required

- Must be willing and able to lift, move or carry materials weighing up to 50lbs

- Residence near an airport for travel purposes preferred

- Proficiency with software such as Microsoft Word, Outlook, Adobe Acrobat, & general internet search

- Keeps current on weather forecast, routing, roadwork, or any other event likely to impact the punctuality or safety of transporting the mobile unit and appropriately reports concerns in a timely way to stakeholders

- Follows all company policies regarding safety and upkeep of conditions/equipment and adheres to local and externally relevant health and safety laws and policies

- Facilitates the check-in/check-out process and manages sign in sheet

- Greets each Veteran immediately and gathers medical records or diagnostic results

- Documents and updates system comments for services pertaining to Veterans and providers

- Keeps all Veterans' information private and confidential

- Communicates in a courteous and respectful manner while assisting in any way needed

- Communicates and assists other departments in a collaborative effort to expedite cases

- Adapts to new instructions, requests or procedures maintaining a high sense of urgency

Home Office Requirements Using Maximus-Provided Equipment:

- Internet speed of 20mbps or higher required (you can test this by going to

- Connectivity to the internet via either Wi-Fi or Category 5 or 6 ethernet patch cable to the home router

- Private work area and adequate power source

- Must currently and permanently reside in the Continental US

- In accordance with SCA contract requirements, remote work must be conducted from the location specified at the time of hire. Unapproved travel is not permitted, and you are required to remain at your designated home location for all work activities.

Minimum Requirements

- High School Diploma or GED required.
- Ability to operate large vehicle for extended periods of time.
- Must have an excellent driving record without any serious driving violations for the last five (5) years.

- Must be a minimum of 25 years to operate company vehicles due to insurance requirements.
