Humana Healthy Horizons in Kentucky is seeking a Community Based Social Determinants of Health Professional to join our team in Western Kentucky area.
This associate actively engages with members to help address their health-related social needs. **Essential Role Functions and Responsibilities:**
+ Conduct in-person and telephonic non-clinical assessment(s) to understand enrollee care needs, preferences, and socioeconomic barriers and evaluate the home environment as needed within 50 miles of Paducah or Owensboro, Kentucky.
+ Work collaboratively with other Humana associates as a member of Humana's Care Management Support teams, including case managers, SDOH coordinators, Community Education Representatives (CER), and the Quality Improvement team that conducts HEDIS outreach.
+ Serve as a member of enrollee multi-disciplinary care teams (MDT), as requested.
+ Assist enrollees with scheduling physical health and behavioral health (BH) office visits and addressing barriers to appointment attendance.
+ Assess enrollee barriers to healthy living and accessing healthcare services and arrange needed social services and supports.
+ Act as an enrollee advocate with providers, community resources, schools, and others, including accompanying enrollees to provider office visits as requested.
+ Locate enrollees when they miss appointments to find out why the appointment was missed and problem-solve to address barriers to care.
+ Monitor enrollee compliance with their care plan and provide motivational interviewing to support medication and treatment adherence.
+ Provide social support to help boost enrollees' morale and sense of self-worth, serving as a trustworthy, reliable, non-judgmental, consistent, and accepting member of the enrollee's MDT.
+ Support enrollee self-management through the provision of culturally appropriate health education and health coaching.
+ Serve as a key knowledge source for community services and information for enrollees and other Humana associates.
+ Attend Humana community events to connect with enrollees and provide education on case management services.
+ Conduct research and in-person outreach to locate difficult-to-contact enrollees to increase assessment completion and participation in clinical programs.
+ Build and maintain relationships with providers and community resources to support enrollee identification and referrals.
+ Implement community assessments to identify community resource gaps.

**Required Qualifications**
Must meet **one** of the following:
+ Associate's degree and 5+ years of experience working with individuals face to face and telephonically conducting assessments, and/or professional experience in human services or as a community health worker
+ Bachelor's degree or higher and 3+ years of experience working with individuals face to face and telephonically conducting assessments, and/or professional experience in human services or as a community health worker

Must meet **all** of the following:
+ Certified Community Health Worker in the state of Kentucky or willingness to obtain certification within 12 months of hire
+ Proficient in Microsoft Office (Excel, Word)
+ Ability to use a variety of electronic information applications/software programs
**Preferred Qualifications**
+ Prior experience with Medicaid recipients
+ Bilingual (Language Proficiency Testing required)
**Workstyle:** Combination remote work at home and field based (60-75% travel)
**Location:** Must reside within a 50-mile radius of Owensboro, KY or Paducah, KY
**Schedule:** Mon-Fri between 8:00 AM to 6:00 PM EST, with flexibility for evenings and weekend events, based on business needs

**TB Statement**
This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
**Driving Statement**
This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$5,000 bodily injury per event / 10,000 for property damage or whichever is higher. 
**Work at Home Internet Statement**

Position Summary:

Eventus WholeHealth delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in-house Support Staff.

We are seeking a Therapeutic Behavioral Specialist to join our team! You will provide recovery-oriented support and case management services to residents of the long term care facilities and their families. Opportunities may be present for a future transition to providing counseling services. We care for individuals in long-term care settings, both skilled nursing and assisted living or personal care. We address a wide range of problems from anxiety and depression to severe and persistent mental illness to issues around death and dying. As part of a multidisciplinary team, our clinicians have the chance to make a big difference in the quality of life for these residents. We solve problems, deliver excellent clinical care, and provide a critical link in the care a vulnerable population.

Essential Duties and Responsibilities:

  • Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs getting met.
  • Provide service planning for individualized supports or care coordination of healthcare, behavioral health, and development of a treatment plan.
  • Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
  • Provide services to individual clients or groups of clients.
  • Training of support systems including direct staff on specific client triggers, de-escalation techniques and interventions in client specific mental health treatment plan as developed by licensed clinician.
  • Promote a positive and cooperative relationship with all outside contacts.
  • Meet with Resident Care Coordinator or other staff as designated by the facility upon arrival to and departure from facility to review current resident and facility care needs and communicate findings or concerns.
  • Establish and maintain open and positive communications with facility staff and administration. Working with licensed Provide verbal and/or written instruction or feedback regarding treatment plans and other pertinent caregiver information.
  • Be knowledgeable of and adhere to Eventus WholeHealth standards, policies, and procedures. Be aware of and adhere to all legal and regulatory agencies' rules and guidelines and professional ethical standards. Comply with all regulatory agencies governing health care delivery.
  • Conduct self in a professional manner at all times.
  • Maintain patient confidentiality at all times including appropriate use of cell phone, email, text messaging, patient records, and EMR. Agree to abide by and be knowledgeable of HIPAA rules and regulations.
  • Consult with the clinical supervisor when facing an unfamiliar resident, family, or facility request or recommendation regarding the care of a resident.
Qualifications/Education:

Licensed Professional Counselors and Licensed Marriage and Family Therapists-Master's degree in either counseling or marriage and family therapy. Eligibility for a license to practice in the state in which you will practice, required.

Knowledge:
  • Knowledge of population-specific interventions, and delivering care following treatment plan.
  • Knowledge of rules and regulations of bodies governing behavioral health practice.
  • Knowledge of organization's policies and procedures.
Skills Required:
  • Skill in gathering and analyzing physiological, socioeconomic, behavioral, and emotional patient data.
  • Skills in providing training in social and emotional well being, stress management, anger management and overall life skills.
  • Skill in accurately evaluating patient problems and providing appropriate advice, intervention or referral.
  • Skill in written and verbal communication.
  • Skill in exercising a high degree of initiative, judgment and discretion.
  • Skill in establishing and maintaining effective working relationships.
Abilities:
  • Ability to react calmly and effectively in emergency situations.
  • Ability to work collaboratively with all members of the health care team.
  • Ability to evaluate and make recommendations for continuous quality improvement.
