- Observes patients for changes in condition or behavior and promptly reports these changes to RN, as needed. Follows up as appropriate with supervisor regarding reported complaints, problems, and concerns 100% of the time. Report to RN reddened areas, N/V, amount and color of emesis, diarrhea, or any abnormal BMs, constipation, poor appetite, any complaints of pain, any complaints patient and family has or questions regarding Plan of Care, no urination, appearance of urine if anything other than clear, change in alertness, pain or leaks at IV site, equipment alerts. Complete COA flow sheet with every patient
- Beginning of Every Shift: Receive report from RN. Introduction to the patient, know patient's diagnosis, any recent act of violence, any precautions. Call light and phone is within patient's reach, unless ordered otherwise by the RN, educate patient on use of call light. Tidy the room, bedside table, floor, and keep path to the bathroom free from clutter, gather all anticipated supplies that will be needed. Fill water pitcher, provide fresh cup and linen.
- Providers constant observation for patients being held under the Florida Mental Health Act and other patients as assigned. This includes accompanying patients to all medical procedures. Always maintain a clear exit for safety. It is suggested to sit on the side of the bed closest to the door or near the door depending on your patient
- Daily: Under the discretion of the supervising nurse, assist with bath, assist with oral care (including dentures), brush hair, offer and administer back rub if requested, soak feet, assist and/or monitor shaving. Ask if patient has toothpaste, soap, toothbrush, etc. Assist in changing bed linens as needed.
- Every Shift: Report to the RN the percentage of diet consumed. Assist patient with ambulation to a chair or the bathroom.
- Throughout Shift: Communicates effectively with team members, maintains patient comfort, confidentiality and privacy. Adheres to HIPAA standards. Follows all protocols and policies to ensure patient safety. Documentation including care provided and patient's behavior are recorded on the Constant Observation sheet. Place COA flow sheet in chart when completed. It is part of the permanent medical record. Make sure RN completes safe room checklist with you and all patient belongings are removed by security if under Baker Act.
- Every 2 Hours: Assist patient care staff with turning patients. Observes their skin (heels, back, ears, shoulder blades, elbows, buttocks, back of head) and reports changes to RN. If needed, prop pillows on back and place pillow between knees. Reports complaints of pain to the patient care staff.
- Meal Times: Offer wash cloth so patient care wash hands before meals. Set up food trays, feed patient if needed. Patients with swallowing difficulty may need specific direction form RN prior to feeding patient.
- Throughout Shift: Inspect all aspects of patient's environment to ensure patient's safety, and report correct unsafe conditions or report to RN. Assist patient to the bathroom at least every 2 hours. Keep door propped open at all times. Have a clear visual of patient and arm length distance if a fall risk. Offer assistance with toileting. Offer wash cloth so patient can wash hands after bathroom. Assist RN and PCA with care as needed within COA job description parameters. Maintain clean environment. Place soiled linen in bag and place at door for removal.
- Virtual Safety Responsibilities: Complete virtual rounding a minimum of every hour, alert clinical staff to any patient needs or concerns, activate stat alarm as needed, complete documentation hourly.
- Communication: Active listening to patient. No judgmental statements or opinions. Report any reports of abuse to supervising nurse. Report any signs or statements made by patient about harming themselves or others to supervising nurse.

**Position Summary**
The Associate Manager is responsible for oversight of healthcare management staff including the organization and development of high performing teams. Works closely with functional area managers to ensure consistency in clinical and non-clinical interventions supporting our members.
- Accountable for meeting the financial, operational and quality objectives of the unit
- Oversees the implementation of healthcare management services for assigned functional area
- Implements clinical and non-clinical policies & procedures in accordance with applicable regulatory and accreditation standards (e.g. NCQA, state and federal standards and mandates as applicable)
- Serve as a content model expert and mentor to the team regarding practice standards, quality of interventions, problem resolution and critical thinking
- Ensure implementation and monitoring of best practice approaches and innovations to better address the member's needs across the continuum of care
- May act as a liaison with other key business areas
- May develop/assist in development and/review new training content.
- May collaborate/deliver inter and intra-departmental training sessions
- Protects the confidentiality of member information and adheres to company policies regarding confidentiality. Manages resources responsible for identification of members, development and implementation of care plans, enhancement of medical appropriateness and quality of care and monitoring, evaluating, and documenting of care
- Develop, initiate, monitor, and communicate performance expectations
- Ensures the team's understanding and use of information system capability and functionality
- May have responsibility for their own caseload work
- May act as a single point of contact for the member and the Care Management Team including: participation in care team meetings, implementation and oversight of member cultural requirements, and support implementation of new members
- Participate in the recruitment and hiring process for staff using clearly defined requirements in terms of education, experience, technical and performance skills
- Assesses developmental needs and collaborates with others to identify and implement action plans that support the development of high performing teams
- Establish an environment and work style that promotes the concept of teamwork, cross product integration, and continuum of care thinking that results in strong performance
- Consistently demonstrates the ability to serve as a model change agent and lead change efforts
- Create a positive work environment by acknowledging team contributions, soliciting input, and offering personal assistance, when needed
- Accountable for maintaining compliance with policies and procedures and implements them at the employee level
**Required Qualifications**
- Must reside in the Tampa Florida or Surrounding Area, Specifically Hillsboro, Polk, Manatee, Hardee, or Highland County FL
- Must possess reliable transportation and be willing and able to travel up to 50% of the time. Mileage is reimbursed per our company expense reimbursement policy
- 3+ years of managed care experience
- 3-5 years of experience in a physical or behavioral healthcare setting
- Knowledge of the regulations, standards, and policies which relate to medical management
- Proficiency with computer skills which includes navigating multiple systems and keyboarding
- Time efficient and highly organized
- Exceptional communication skills
- Ability to mentor and support staff to meet business needs
**Preferred Qualifications**
- Bilingual in Spanish
- 1-3 years management experience
- Computer literacy and experience familiarity with Microsoft Excel, Word, and web-based applications desired
- Ability to work with people in such a manner as to build high morale and group commitment to goals and objectives
- Ability to evaluate and interpret data, identify areas of improvement, and focus on interventions to improve outcomes
- Ability to communicate effectively with Providers, Members, Staff and other Leaders both in oral and written format
**Education**
- Bachelor's degree in business or health or human services (e.g. Social Work, Psychology, Nursing, Mental Health, Counseling, Behavioral
Sciences, Sociology, Gerontology or related field required.

**Pay Range**
The typical pay range for this role is:
$46,988.00 - $91,800.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
